[Reactive histiocytosis variant of tuberculous lymphadenitis].

Paraffin blocks from 38 cases with superficial lymph nodes exhibiting reactive histiocytosis with debris-like necrosis and neutrophil infiltration were studied by means of polymerase chain reaction for amplifying the repetitive DNA sequence (123pb fragment) specific for mycobacterium tuberculosis (M.TB-PCR), BCG immunohistochemistry (BCG-IHC) as well as acid fast stain (AF) for M.TB/MOTT (mycobacterium other than tuberculosis). The comprehensive positive rate of the three methods was 52.6% (20/38 cases) and the separate positive rates for M.TB-PCR, BCG-IHC and AF being 50%, 26% and 0.8% respectively. The results prove that (1) half of the reactive histiocytosis of lymph nodes selected in this paper are related to tuberculosis infection, i.e., some tuberculous lymphadenitis may appear as a variant of reactive histiocytosis; and (2) PCR technique is a very valuable method for making the etiologic diagnosis of tuberculous lymphadenitis.

[Pleural malignant mesothelioma complicated with disseminated intravascular coagulation].

Three cases of MPM complicated with DIC were reported in a total of 6862 autopsies (0.04%) from 1950 to 1989 year in our university hospital. All the three cases showed hemorrhagic patches, shock, decreased platelets and fibrinogen, and prolonged prothrombin and thrombin time. Hyaline thrombosis was observed in the pulmonary interstitial microvessels. We have also discussed the diagnosis and pathogenesis of DIC.

[A clinicopathological and immunohistochemical study of 49 cases of necrotizing lymphadenitis].

A clinicopathological and immunohistiochemical study of 49 cases of necrotizing lymphadenitis was made. Twenty-four patients were male and twenty-five female with the M:F ratio of 0.96:1. The patients ranged in age from 9 to 62 years with a mean of 26. Forty-nine patients had superficial lymphadenopathy, and twenty-two were accompanied by fever. The results also showed that there was multifoci necrosis present in the involved lymph nodes, particularly in the cortex or/and paracortex, with variable numbers of small lymphocytes, immunoblasts, histocytes and phagocytes, the latter with phagocytized nuclear debris derived from necrotizing T lymphocytes. However, granulocytes and plasmacytes were generally absent, and B lymphocytes rare. The pathogenesis of this disease remains unknown and the pathogen has not been found by Gram, Giemsa, PAS, Ziehl-Neelsen and Warthin-Starry stain. The diagnosis, differential diagnosis and etiology of this disease are discussed. We support the suggestion that this entity be called "Kikuchi-Fujimoto's disease" rather than "Histiocytic necrotizing lymphadenitis".

[A comparison study of immunohistochemical detection of ras P21 and N-ras mRNA in situ hybridization in hepatocellular carcinoma and pericancerous nontumor liver tissue].

30 cases of hepatocellular carcinoma and pericancerous nontumor samples were studied by examining the serial sections for immunohistochemical detection of P21 product accompanying identification of N-ras mRNA by in situ hybridization with biotinylated N-ras cDNA probe and a significant correlation was noticed between the results obtained (P less than 0.02). It was suggested that immunohistochemical detection of ras oncogene P21 product may indicate the expression of ras oncogene. The higher expression of N-ras mRNA of the carcinoma and the non-tumor tissue peripheral to the cancer by in situ hybridization were 53.33% and 28.57% respectively. The positive ratio of P21 were 96.67% and 90.48%.

[Etiological diagnosis of tuberculous lymphadenitis by polymerase chain reaction].

By using polymerase chain reaction (PCR) technique in amplifying a reparative DNA sequence specific for Mycobacterium tuberculosis, the pathogenic bacterium in paraffin-embedded tissue was identified. A 123-base-pair fragment, specific PCR product, was obtained from all 12 cases of tuberculous lymphadenitis and 4 out of 10 cases in which tuberculosis was suspected. In the 4 suspected cases, 3 cases showed good results when treated by anti-tuberculosis therapy. The remaining case was lost from follow-up. Comparing acid-fast stain and PCR results, we confirm that PCR method is more powerful and more sensitive than acid-fast stain in the etiological diagnosis of tuberculous lymphadenitis.

[Thymus pathological changes in perinatal babies and children].

The most outstanding pathological changes of perinatal babies and children, based on the review of the autopsy files in the 1st Affiliated Hospital of West China University of Medical Sciences, are lymphocytic depletion, and reticuloepithelial cell swelling and/or fusiform malformation in the cortex, while in the medulla apoptosis is more prominent. We suppose that these alterations are due to immaturity of cortical thymocytes, and in the diseased condition, they are easily affected by extraordinary factors, especially the influence of corticosterone inducing acute severe necrosis, so the number of lymphocytes are obviously diminished. But, in the medulla, as intact mature lymphocytes exist, physiological phenomenon such as apoptosis is rather prominent in it. While in the medulla, Hassall's corpuscles have various characteristic changes, such as cornification, calcification, fusion, cystic change and disintegration. Besides, we observed a new alteration in the thymus defined as vacuolization. All the above pathological changes reached the peak in the 28- day group; there after, they might become either worse or better according to the condition of the disease and growth of the body. However, these are still problems to be further studied separately.

Porcine endogenus retrovirus transmission from pig cell line to mouse tissues but not human cells in nude mice.

The continuous shortage of human donor organs for transplantation has led to new interest in the application of pig organs. However, reports of pig endogenous retroviruses (PERV), which are able to infect human cells in vitro, have raised concerns about the transmission of PERV to the recipients or even to other community members. In this study, PK15 and human 7721 cell lines were implanted into each flank of nude mice and tumors appeared several weeks postimplantation. PERV infection was detected by PCR, using cytochrome oxidase B sequences as the specific marker for pig DNA. The results showed that PERV gag sequence were positive in mice livers, kidneys, hearts, and lungs, but no cytochrome oxidase B sequences detected, which indicates the absence of pig-mouse microchimerism. The results also showed that PERV did not infect human 7721 tumors in mice. This study confirmed the presence of PERV transmission from pig-to-mouse tissue and strengthened the concern of the risk of transmitting PERV through pig cells xenotransplantation.

The characteristics of tolerogenic plasmacytoid dendritic cells stimulated with interleukin-3.

This study focused on whether bone marrow would differentiate tolerogenic dendritic cells (DCs) by comparing interleukin-3 (IL-3) DCs (induced by IL-3+IL-4) with GM-CSF DCs (induced by GM-CSF+IL-4, generating myeloid DCs). Mouse bone marrow progenitors were induced in vitro to differentiate into DCs by exposure to IL-3. The morphology and yields of IL-3 DCs and GM-CSF DCs were compared by scanning electron microscopy and laser scanning confocal microscopy. Immunohistochemistry and flow cytometer (FCM) were used to identify immune phenotypes. The results showed that IL-3 DCs and GM-CSF DCs both expressed the specific marker DEC-205 with about 80% and 70% purity, respectively. IL-3 DCs had similar morphology as plasma cells, but a different immune phenotype, namely, CD80(-), CD86(-), CD14(-), and MHC class II(+). IL-3 DCs remained in the costimulatory molecule-deficient state when TNF-alpha was supplemented into the culture. We therefore propose that IL-3 induces a high purity and yield of DCs from bone marrow. Since IL-3 DCs possess a tolerogenic phenotype (MHC class II(+), B7(-)), they may induce alloantigen-specific hyporesponsiveness in vitro.
