Unusual retinal pigment epitheliopathy and choroidopathy in carcinomatosis: a rare case of cancer-associated retinopathy.

BACKGROUND: Cancer-associated retinopathy is a syndrome causing ocular symptoms. It is a rare entity and only a few cases have been reported. METHODS: A 67-year-old woman with small-cell endometrial carcinoma suffering from deterioration of visual acuity is presented. RESULTS: The patient presented with extensive mottled changes of the retinal pigment epithelium, accompanied by diffuse subretinal fluid in the posterior pole and exudative retinal detachments inferior in both eyes. CONCLUSION: This patient suffered from a rare variety of cancer-associated retinopathy.

Research utilization.

The gap that persists between research and practice has been highlighted by nurse practitioners. In South Africa the research-practice gap is maximized by the lack of research facilities and research utilization projects in nursing education programmes. This paper considers conceptual, empirical and pragmatic issues in an attempt to explore the complexity of the influences that may affect the integration of research into nursing practice. Issues involved in a process of transferring research-based knowledge into innovative protocols for nursing practice are also dealt with.

Ocular manifestations of relapsing polychondritis. Three case histories.

Relapsing polychondritis is a rare, presumably inflammatory autoimmune disorder affecting cartilagenous structures throughout the body. The ears, nose, joints, eyes and the respiratory tract are most frequently involved. The main ocular manifestations are episcleritis and scleritis, conjunctivitis, iridocyclitis and chorioretinitis, cataract and corneal infiltrates and melting. Extraocular signs and symptoms often are indicative of the diagnosis. Therapy includes systemic steroids, immunosuppressive drugs and dapsone. Three case histories are reported and the literature of ocular manifestations of relapsing polychondritis is reviewed.

Echographic classification of intraocular tumours. A 15-year retrospective analysis.

The data on echographic routine diagnosis of intraocular tumours (prominence equal to, or larger than 2 mm) during the years 1976-1990 in the authors' department, were analyzed by multivariate statistical methods, using the final diagnosis either from pathology after enucleation or from the confirmed clinical diagnosis. The material consisted of melanomas (n = 325), metastases (n = 44), haemangiomas (n = 19) and other intraocular tumours (n = 16). The best set of echographic parameters in descending order of significance was: reflectivity (A-mode), choroidal excavation (B-mode), shape (B-mode), and regularity (A/B mode). Echographic differentiation by computer analysis of data on the three main kinds of tumours (melanoma/metastasis/haemangioma) separate from all the remaining tumours was expressed by the correct fraction: melanoma 89%, metastasis 80% and haemangioma 97%. The clinical echographic classification for these cases was 89%, 93% and 99.5%, respectively. The simultaneous differentiation between the three classes was found to yield a correct fraction of 85% by computer statistics and 95% by routine echography. The results of the present study might be used for prospective classification through the use of the parameter 'knowledge base' contained in the statistical classification procedure.

Choroid Neoplasms

The electrooculogram in heterozygote carriers of Usher syndrome, retinitis pigmentosa, neuronal ceroid lipofuscinosis, senior syndrome and choroideremia.

Electrooculographic studies were performed in 77 carriers of tapetoretinal dystrophies: Usher syndrome (20), retinitis pigmentosa (32), neuronal ceroid lipofuscinosis (6), Senior syndrome (2), and choroideremia (17). The carriers were matched for sex and age with normal controls. In carriers of Usher syndrome the EOG Lp/Dt ratio was significantly lowered with 30% of the recordings having a subnormal value. There was a trend in carriers of retinitis pigmentosa to a subnormal EOG. In contrast to previous studies there was no decrease in the EOG Lp/Dt ratio in carriers of neuronal ceroid lipofuscinosis. Two carriers of Senior's syndrome had a normal EOG. Carriers of choroideremia did not differ significantly from normal controls; however, the Lp/Dt ratio decreased with increasing age. An abnormal EOG may be indicative of the carrier state in relatives of patients with tapetoretinal dystrophies.

Academic nurse leaders' interpretation of concepts and priorities related to the examination of scientific short papers, dissertations and theses--Part 1.

The purpose of this study was to establish whether there is agreement among academic nurse leaders in their interpretation of aspects and criteria related to the examination of scientific short papers, dissertations and theses. The Delphi technique was selected as the most appropriate method of data-collection for this type of study. The target population identified for this study consisted of the Heads of Nursing Departments or their delegates from the 18 universities of the four provinces of the RSA, self-governing and independent states of Southern Africa and Namibia, which offer nursing degrees. Three rounds of questionnaires were sent to all heads of the 18 identified nursing departments. Participation was poor (nine participated in first round, nine in second the third round, no further rounds of questionnaires were sent due to the waning interest and poor participation. Many divergent responses were received to most of the aspects included in the study. This indicates that with a few exceptions there is little or no agreement among academic nurse leaders in their interpretation of aspects and criteria relating to the examination of post-graduate scientific or academic treatises.

The science of nursing: current issues and dilemmas.

The question regarding the nature and direction of nursing science, how it is derived or why and whether nursing is a science is a much debated one. This article focuses on an examination of the literature with regard to conceptualizations of science in general and nursing in particular with special emphasis on the nature, purpose, methods and domain of science and nursing science. It looks at the various positions taken, examines the consequences of holding any special position and makes suggestions for achieving excellence in Nursing Science.

On empathy: fundamental issues regarding its nature and teaching.

In nursing empathy is generally regarded as an essential component of nurse-patient interaction, but there appears to be a lack of clarity among nurse educators and professional nurses regarding its nature and how it may be taught and measured. This paper focuses on an analysis of selected theoretical works related to the nature and teaching of empathy, and an attempt is made to identify fundamental issues regarding the phenomenon "empathy" and its place in clinical nursing practice.
