Esophageal cancer metastatic to kidney: report of 2 cases.

We report 2 cases of renal tumor secondary to an esophageal cancer and discovered during an examination for hematuria. Despite the history the existence of a renal tumor with no apparent metastatic spread and hematuria justified nephrectomy, which led to the confirmation of metastasis to the kidney. Metastases of esophageal cancers represent only 4.8 per cent of secondary renal tumors. The kidneys are the fourth most common metastatic site of esophageal cancers, generally associated with several other secondary localizations. Their clinical latency is common. The difficulty in diagnosing these tumors and the frequent failure of complementary examinations result from their character, that is nodular, small diameter, multiple and cortical.

Short-term prophylaxis with cefotaxime in prostatic surgery.

During a 20 month period 167 patients undergoing transurethral resection of prostate were divided into two groups: a control group without prophylactic antibiotics and a treated group who received cefotaxime 3 g intravenously perioperatively. In the first seven post-operative days 50 patients in the control group had a urinary tract infection (greater than 10(5) cfu/ml) compared with 14 patients in the treated group (P less than 0.0001). Two blood cultures were positive, both in the controls. A full course of antibiotics was administered to 55 in the control group and 14 in the treated group. On the thirtieth post-operative day 24 patients had urinary infection in the control group compared with 5 in the treated group (P less than 0.0001). We conclude that this short-term prophylaxis with cefotaxime significantly reduced the rate of urinary infection after transurethral prostatectomy.

Iridium-192 wiring after partial cystectomy as a treatment of small malignant bladder tumors.

Small tumors of the mobile portion of the bladder may be treated by means of interstitial irradiation by iridium-192 wiring. Forty-four tumors (mostly T1, but also T2 and T3) were treated by this procedure and followed over five years. Good results are attained in 66 per cent of the cases. This method has the benefit of avoiding irradiation to the surgeon's hands, and of delivering to a specific site a dose of 6,000 rads. No bladder contraction was observed. Supplementary treatment by irradiation has to be considered in view of frequent recurrences of malignant bladder tumors after transurethral resection of partial cystectomy. In small cancers slightly infiltrating the bladder wall, many attempts have been made to replace tele-irradiation by interstitial irradiation. The latter delivers specifically high rad dosage to the involved area. Radium needles has been used in France by Darget before 1951 but evaluation of accurate dosage proved difficult. This material was dangerous to the surgeon's hands, and the procedure lead frequently to bladder sclerosis. This technique is currently used by Van der Werf-Messing in Rotterdam. She strongly advises preoperative tele-irradiation (3 X: 350 rads) to prevent tumor implants in the scar. An intravesical balloon filled with fluid radioisotope of gold-198 or cobalt-60 has been used for diffuse malignant papillary epithelioma. It was discontinued because of radiation cystitis and subsequent contraction of the bladder. Recently in well-limited tumors, Bloom and Wallace have used Tantalum-182 needles which have interesting physical properties (high energy and short half-life period) (2).

[Treatment of primary vesico-renal reflux in adults. Apropos of 40 patients].

The authors report a series of forty patients presenting with vesicorenal reflux treated by ureteral reimplantation. Seventeen ureterorenal units were treated by Hutch's procedure, with a success rate of 75%. In 1972, this procedure was abandoned in favor of Cohen's technique, and a further forty two ureterorenal units were treated with a 90% success rate. There was no recurrence of the reflux, but there was one case of stenosis.

[Late anuria in rectosigmoid cancers].

The authors discuss nine cases of anuria by local recurrence after abdominoperineal resection for cancer of the rectosigmoid, with particular emphasis on the frequency, diagnosis, prognosis, and the different methods of treating this type of anuria. The placing of a Gibbons catheter between the renal pelvis and the bladder seems to be the most suitable.

Treatment of malignant bladder tumors by iridium-192 wiring.

Computed tomography diagnosis of renal angiomyolipoma.

Six computed tomography examinations were performed on four patients: two of them had tuberous sclerosis, and the other two had solitary angiomyolipomas. Computed tomography is an easy way of diagnosing tumoral hemorrhages and thus facilitates the supervision of patients with tuberous sclerosis. In two patients, the authors observed dense masses that might have indicated cartilage deposits.

[Hypothyroid myopathy: apropos of a case with a great increase of creatine phosphokinase, myoglobinemia and transient kidney failure].

Clinical, usually moderate myopathy, frequently develops in patients with hypothyroidism and may reveal the thyroid dysfunction. Biological signs of myolysis, essentially a, usually moderate, rise in creatine phosphokinase plasma levels, are often found. On the other hand, rhabdomyolysis seems to be exceptional in this context. We report a case where an extremely severe clinical and biological myopathy associated with transient renal failure revealed hypothyroidism due to prolonged intake of amiodarone.

[Is there still a role for radical cystectomy in localized, infiltrating bladder cancers in 1998?].

The role of radical cystectomy as the standard treatment of localised infiltrating bladder malignancy is challenged by the development radiotherapy and chemotherapy combinations. The published studies are difficult to compare because of large differences in the patients selection criteria and in the assessment of local involvement (only clinical in combined treatment, based on pathology and therefore unquestionable in surgical series). The advantages of radical surgery are its precedence and a well-established technique, simplified follow-up procedures and seemingly higher survival rates acquired after 3 years. Conversely, the combination of radiotherapy and chemotherapy allows bladder preservation in about 40% of the patients (at 5 years), but only in 23% without bladder relapses. A complete initial endoscopic resection is the best and only prognostic factor of these results which nevertheless needs a very carefully and endless follows. The comparison of the quality of life achieved by both treatment modalities remains insufficiently documented.

[Adaptation and validation in the French language of the International Score of Symptoms of Benign Prostatic Hypertrophy].

The International Prostate Symptom Score, which was proposed during the First Consultation on Benign Prostatic Hyperplasia in June 1991, was culturally adapted and linguistically validated in French. In a first step, the English version, adapted from the questionnaire developed by the American Urological Association, was translated into French. In a second step, the French version was submitted to a multidisciplinary group who made linguistic improvements. In a third step, the refined version was tested in 30 persons and amended. Finally, the questionnaire was submitted to a linguistic validation study in a representative sample of 100 men aged 65 to 80. The use of this index will allow standardized measurements of urinary symptoms status in patients with benign prostatic hyperplasia.
