[Development and application of a new polyurethane plastics support bandage (author's transl)].

Experiences with a newly developed plastics support bandage on 1007 patients are reported. A polyurethane hard foam composition developed specially for medical purposes was used, which is created by blending the two components A (polyol) and B (isocyanate). The principle of our support bandage lies in the fact that after being mixed in a 1:1 ratio, the two highly viscous components are packed into a cotton stocking having a high transverse elasticity. After being rolled out to the desired thickness, the stocking is wrapped around the extremity which it is to support, where it hardens out into a polyurethane hard foam composition. By reason of the moisture content of the scoured cotton stocking, a 1 to 2 mm thick layer of foam forms in the contact region and renders any further padding unnecessary. Correspondingly, by using a double chamber stocking, circular bandages have been produced which can be removed, by means of incorporated zip and interengaging hook fasteners. Reclining shells, abduction bandages and body corsets can be made in the same way. The physical properties of the support bandage are illustrated: apart from its low weight, the bandage is particularly advantageous by reason of its high strength, insensitivity to water and ideal permeability to X-rays. The support bandages are easily made up and entail negligible cost.

Anxiety, task-experience, and vocal-style of judges as determinants of performer confidence.

To examine the effect of umpiring vocal-manner on a performance-measure of confidence, 30 baseball players viewed a series of videotaped pitching-catching-umpiring sequences and rated confidence in their ability to hit each pitch. Matched sequences were presented under each of two controlled conditions: for Conventional, the umpire called "strikes" emphatically and "balls" in an indifferent manner; for Modified, this vocal-manner was reversed. A single Conventional (or, Modified) sequence embedded within a series of contrasting sequences and a "bad" pitch (clearly out of the strike-zone) were included as tests of the effectiveness of the experimental manipulation. Results indicated: (1) Modified umpiring enhanced confidence; (2) this effect was most prominent among highly experienced players; (3) low-anxiety players showed higher confidence under both conditions; (4) Modified sequences embedded within a Conventional background elicited significantly greater confidence than in the background sequences.

[Teratogenic action of ethyl-nitrosourea in Göttingen miniature pigs (author's transl)].

By i.p. application of 70 or 100 mg/kg ethyl-nitrosourea, resp., to gravide Göttingen miniature pigs on days 14 or 16, resp., of gestation massive malformations of the skeletal system were produced. The experiments demonstrated the teratogenic effect of ethylnitrosourea on non-rodents, ascertained the period of determination of the skeletal system in pigs, confirmed the concentration effect in teratogenesis as well as the ineffectiveness of biological barriers with adequately high dosages.

Acoustic neuroma surgery: an eclectic approach with emphasis on preservation of hearing. The New York University-Bellevue experience.

During the past 10 years, 157 patients have been operated on at the New York University-Bellevue Medical Center for acoustic neuromas and other cerebellopontine angle tumors. We describe our diagnostic protocol with joint neurosurgical evaluation leading to either translabyrinthine (otology only) or suboccipital-transmeatal (combined otology-neurosurgery) surgery. The decision is made on the basis of tumor size, level of hearing, age, and health of the patient. Using these criteria, 105 suboccipital and 59 translabyrinthine operations were performed with eight patients having had two-stage operations. In 12 of 29 patients, hearing was successfully preserved. Of 18 patients with good hearing and extracanalicular tumors less than 2 cm, hearing was preserved in 11. We describe the surgical technique used in this suboccipital-transmeatal operation and present illustrative cases in detail.

[Differences in the health and disease conditions in the rural and urban-suburban populations in Nuevo León state].

This work shows the results of two surveys taken in the state of Nuevo León, Mexico during 1985. The first survey, which covered morbidity and health service use, was taken from 759 households in urban/suburban Monterrey (a city of two million inhabitants and capital of the state of Nuevo León) and from 751 households from a stratified sample from the rest of the state. The second survey, which had to do with the different aspects of quality health services available, was given to 752 patients in 93 distinct health units in Monterrey and all over the state. The objectives of this publication are: 1) to describe the morbidity of the populations in question, as well as to describe the different aspects of the supply and demand of health services; 2) to compare the conditions of life reflected in the state of health between the rural and urban/suburban zones of Nuevo León. The results obtained show that those in the urban/suburban zones of Monterrey possess certain advantages over those in the rural areas in the state of Nuevo León such as: a higher level of schooling, lower emigration to the U.S.A., a lower fertility index, lower morbidity from severe illnesses and a lower death rate in children, wider coverage from health insurance, better communication between doctor and patient, more frequent use of health services, and greater participation in prevention programs. Rural area life has the following advantages: a lower number of cesarean sections, and a prolonged period of maternal suckling. In this work, we present the opinion that the primary health care currently centered in the urban zones, where the population is much greater, should not impair the required attention in the rural areas (areas that are more extensive and often unattended).

Circadian and circannual variations of FSH, LH, testosterone, dehydroepiandrosterone-sulfate (DHEA-S) and 17-hydroxy progesterone (17 OH-Prog) in elderly men and women.

A group of 63 men and 86 women 77 +/- 8 years of age institutionalized at the Berceni Clinical Hospital, Bucharest, Romania, on a diurnally active living schedule with rest at night, eating 3 comparable but not identical meals a day, were investigated over a 2-year span in 278 profiles extending over a 24-hour span each and distributed over all our seasons. On each day of sampling six samples of blood were collected at 4-hour intervals beginning at 0800 of the first day of the study to 0400 of the next. Plasma testosterone, dehydroepiandrosteronesulfate (DHEA-S), 17-hydroxyprogesterone (17-OH Prog), FSH and LH concentrations were determined by radioimmunoassay. Statistically significant circadian rhythms were detected and quantified by population mean cosinor for the men and women of the entire group and for the age groups of 51 to 70 (I), 71 to 80 (II) and above 80 years of age (III) separately. Circannual rhythms of the circadian means were sought and evaluated by one way analysis of variance. Statistically highly significant circadian rhythms were found in plasma testosterone, 17-OH Prog and DHEA-S, concentrations in men and women of all three age groups with a phase advance of over 2 hours in DHEA-S with advancing age. Circannual cycles were found for plasma testosterone concentration and for DHEA-S in men and women, with a likely phase difference between men and women in testosterone. In LH a circadian rhythm was found in the women and in FSH in men only and a circannual variation in LH only in men.
