[Comparison of laparoscopic and scintigraphic findings in chronic hep-titis, liver cirrhosis and liver tumours (author's transl)].

Results of laparoscopy and scintigraphy were compared and contrasted with ultimate clinical diagnosis in 94 patients with chronic hepatitis or liver cirrhosis and 38 with liver tumours. The findings agreed in 77 cases of chronic hepatitis and cirrhosis and 33 of liver tumour. In the remainder either laparoscopy or scintigraphy gave the wrong results. In one case each of cirrhosis and liver metluded that (1) laparoscopy and scintigraphy are methods complementing but not replacing other clinical tests; (2) if diffuse liver disease is suspected clinically the diagnosis can essentially be done only by laparoscopy and selective biopsy or blind liver biopsy but not scintigraphy; (o) if a circumscribed condition is suspected, scintigraphy is among the most important screening tests, but cannot replace laparoscopy, biopsy or angiography for confirming the diagnosis; (4) liver scan is an important method, of little stress to the patient, in following the progress of chronic hepatitis and liver cirrhosis, additional to clinical examination, enzyme tests, laparoscopy and histology.

[Endocrinological and histomorphological studies of testicular atrophy in myotonic dystrophy (author's transl)].

Testicular atrophy in myotonic dystrophy is caused by a primary and progressive atrophy of the seminiferous tubules. Endocrinological studies with synthetic LH(ICSH)-RH in two patients demonstrated the complete absence of FSH feed back mechanism. Glycogen content was markedly diminished or completely absent, as demonstrated histochemically, both in the morphologically intact and in damaged tubules. The endocrinological and morphological results both demonstrate that insufficiency of the Sertoli cells is the primary damage in the pathogenesis of testicular atrophy in myotonic dystrophy.

[HL-A, W 27 and serum amino-acids in ankylosing spondylitis (author's transl)].

Tests for HL-A, W 27 were positive in eight of twelve patients with ankylosing spondylitis. The four negative cases, among them three members of the same family, had a different antigen pattern. Also positive were two cases with Reiter's disease. Semi-quantitative determination of serum levels of aminoacids in this group gave an increased alanine concentration in all but two instances. Leucine and valine levels were also raised, but less characteristically than alanine. These immunogenetic studies enrich diagnostic possibilities in clinical medicine, in addition to suggesting a possible common aetiology and(or) pathogenesis of Reiterś disease and ankylosing spondylitis. The significance of quantitative amino-acid modifications is not clear.

Microscope fluorometric investigations on the reticulocytic maturation distribution as diagnostic criterion of disordered erythropoiesis.

A microscope fluorometric technique is described which permits not only the visual identification of reticulocytes under the fluorescence microscope but also the determination of their relative stage of maturation to normocytes. The technique is based on a specific staining procedure which results in a fluorescent complex between the reticulocytic RNA and acridine orange. Thus, the relative mass of RNA in the individual reticulocytes can be measured by means of mciroscope fluorometry. As the reticulocytic RNA content decreases and finally disappears during the final maturation process of reticulocytes after their release into the peripheral blood stream, the fluorescence signal indicates the relative degree of this maturation. A characteristic frequency distribution of this parameter can be obtained for a given blood sample by microscope fluorometry measuring 200 to 300 reticulocytes. The preliminary use of this technique for following up the course of two cases of hemolytic anemia and one of pernicious megaloblastic anemia during their treatment demonstrates the potential diagnostic value of this technique of identifying the change of the reticulocyte maturation distribution in addition to the reticulocyte count. Satisfactory agreement between the microscope fluorometric results and those obtained by counting separately the four reticulocytic maturation stages according to Heilmeyer and Wesbäuser has been achieved. The possibility of obtaining quantitative and comparable results by use of this method may be considered a general advantage and a promising basis for the development of an automated technique.

[Lobar emphysema: differential diagnosis and treatment (author's transl)].

Lobar emphysema with acute severe respiratory distress is predominately seen in early infancy while the few cases after one year of age usually show signs of recurrent respiratory infection. Several conditions causing increased transparency on one side of the chest x-ray must be be differentiated. In case of emergency the simple chest x-ray must be sufficient for diagnosis. In this situation even the performance of a diagnostic thoracotomy can become necessary. The removal of the affected lobe is the treatment of choice. 6 cases of lobar emphysema are reported.

[Deformities of the spine and ribs in embryologically related malformations of the heart with cyanosis].

Three cases of severe costovertebral deformities together with embryologically related cardiac malformations (Fallots Tetralogy and Truncus arteriosus communis) are presented. Two patients died because of their thoracic deformities and limited respiratory function. Relationships to similar bizarre veretebral anomalies are discussed. The cause is probably a teratogenic agent, acting between the fifth to eight embryologic week.

[O2 pressure on liver surface following portacaval anastomosis and administration of vasoactive substances in dogs with normal livers].

1) Following acute portocaval anastomosis in healthy dog liver tissue - pO2 drops by 23%, an extent regulary occuring in hour long laparotomy. 2) Intravenous octapressin lowers splanchnic blood flow, concomitantly liver blood-flow and tissue -pO2 is decreased. 3) In contrast to octapressin orciprenalin improves liver blood-flow and tissue-pO2. 4) By shunting of arterial blood through the gut norepinephrine causes a rapid and lasting elevation of liver tissue-pO2.

[Report on 21 uterine ruptures and the influence of single row uterotomy stitching on rupture prevention (author's transl)].

Wound closure by 1 row of interupted sutures provides optimal healing conditions. As a result scar tissue formation is cut down. This general surgical principle is valid in stitching an uterotomy as well. Increasing frequency of cesarian section could end up in a rising number of scar ruptures. This drawback for a subsequent pregnancy should be met by the single row technique. It was applied in 1434 women from 1962 to 1974. All of them had lower segment cesarian sections. Only 3 harmless ruptures occurred in this group. The clinical course of 12 scar ruptures after terraced uterotomy closure was less favourable. In 6 cases a pregnant uterus ruptured without previous operation. Clinical results are compared. This backs the technique practised. In 5 patients the ruptured uterus could be repaired in the same way as in cesarian section. Hysterectomy, however, was the usual treatment. The risk of rupture after classical cesarian section is lowered by the technique described. This applies to the surgical treatment of uterus duplex as well.
