Sex hormone binding globulin binding capacity, testosterone, 5alpha-dihydrotestosterone, oestradiol and prolactin in plasma of patients with prostatic carcinoma under various types of hormonal treatment.

Sex hormone binding globulin (SHBG) binding capacity, the concentrations of testosterone (T), of 5alpha-dihydrotestosterone (DHT), of oestradiol-17beta (Oe2), of oestrone (Oe1), of prolactin (hPr) and the percentual specific binding of T to SHBG (%TB) were measured in plasma of patients suffering from prostatic carcinoma and of a control group of similar age. No significant differences in any of the investigated parameters were found between the control group and the carcinoma patients before treatment although 15% of the latter showed distinctly elevated hPr values. Treatment of carcinoma patients with 1) Antiandrogen (cyproterone acetate, Androcur) resulted in a significant decrease of T, Oe2 and SHBG. The DHT/T-ratio increased. n=5. 2) Orchidectomy caused an even more pronounced fall in T, DHT, Oe1 and Oe2 blood levels. SHBG was not altered. DHT/T-ratio increased. n=32. 3) Cyproterone acetate after orchidectomy led to elevated hPr values. n=5. 4) Oestrogen (diethylstiboestrol-diphosphate, Honvan) after orchidectomy increased SHBG and hPr. n=6. 5) Corticosteroid (Prednisone, Decortin) after orchidectomy decreased T and SHBG below the levels found after orchidectomy alone. n=5. 6) Diureticum (Mefruside, Baycaron) (n=5) or 7) a placebo (n=7) did not alter any of the parameters measured. 8) Treatment with HCG (Primogonyl) of patients suffering from oligozoospermia resulted in a significant increase of T, DHT and Oe2. SHBG was not altered. DHT/T-ratio decreased. n=7.

[Admission and treatment of 592 patients over 70 years in an intensive care unit].

Using an IBM 3741 computer, the authors analysed 592 case records of patients aged over 70 years, admitted to hospital on an intensive care unit, age appears to be an important factor in determining the prognosis of numerous diseases without, however, being a criteria for rejection of admission of these patients to an intensive care unit. The fragility of these patients leads one to consider the following parameters: 1--the degree of resuscitation is assessed by quantification of the therapeutic means. As for other patients, survival varies inversely in relation to the important of the therapeutic means, but in the case of the elderly patient, there is a definite difference in the correlation mortality-therapeutic index towards aggravation, so one may determine a ceiling above which survival is not possible; 2--duration of the intensive care is very important. In most cases, the decisive point is situated on the 3rd or 4th day of care, at which time the decision to continue or stop treatment may be considered in the light of the patient's progress.

[The importance of liver scanning in pancreatic carcinoma (author's transl)].

One of several liver scans were obtained in 29 patients with carcinoma of the pancreas. Scanning of the liver offers several advantages in cases of suspected carcinoma of the pancreas: (1) It may prove malignancy in jaundiced patients without a detectable primary tumor (frequency carcinoma of the pancreas). (2) It may contribute to the differential diagnosis of jaundice when signs of extrahepatic biliary obstruction or metastases can be seen. (3) It is the least strenuous diagnostic method in seriously ill and old patients who are often in very poor condition; sometimes it can spare the patients a laparotomy.

[Morphological and clinical results after combined hormone and radiation therapy for carcinoma of the prostate (author's transl)].

83 prostatic carcinomas were analysed morphologically and clinically before and after combined therapy. The classification of these tumors is based on the proliferation pattern resulting in a group with only one and a group with a variety of growth patterns. The largest groups were carcinomas with glandular structures. On the basis of multiple biopsies the histologic regression of the tumors was determined. In 71% of the cases there was good agreement of morphological results and local palpation findings. Further analyses revealed a correlation between histological growth pattern of the carcinomas and degree of regression: prostatic carcinomas with glandular pattern showed more often a good regression after combined therapy than solid and(or) cribriform tumors. In the groups with a variety of structural pattern the degree of regression was dependent on the prevailing tumor structure. According to these results the organoid-glandular carcinomas of prostate are prognostically more favourable than all the other proliferation patterns. Nevertheless in a single case a prognosis of the therapeutic effect based only on the tumor differentiation can not be made.

Homologous synthesis: a method for the preparation of mixed substrates and individual compounds.

Equimolar mixtures of several homologous or vinylogous alkyl methanesulfonates (mesylates) are converted, by a series of reactions, into equimolar mixtures of compounds having one methylene group more, per molecule, than the starting material. Such mixtures can be used for studying the specificity of an enzymatic reaction in a single experiment in vivo or in vitro. The simultaneous synthesis of a mixture of compounds is also of great advantage in the preparation of a series of radioactively labelled substances. It is more convenient and much more efficient to prepare several labelled compounds of a homologous or vinylogous series in a single reaction, and to isolate each of them by chromatographic methods, than to synthesize the individual substances separately.

[Mediastinal tumors of childhood (author's transl)].

Among 147 operatively treated tumors of the mediastinum there were 14 infantile tumors. 6 of these were malignant. The poor prognosis can only be improved by early diagnosis and early radical surgery. The problems of diagnosis and treatment are discussed.

[Anterior pituitary regulation of testicular function in the elderly (author's transl)].

Pituitary gonadotrophin regulation was examined with the LH-RH test in 15 men (aged between 55 and 90 years), five of them aged between 71 and 90 years. Compared to normal mature males, the plasma testosterone in these subjects was reduced or was in the lower range of normal, while basal LH levels were increased in half the cases. However, basal FSH values were increased in three only. In most of these subjects with increased basal values of LH, the LH-RH stimulation caused a paradoxical temporary reduction in LH release which did not occur with FSH. This paradoxical response also did not occur when basal LH values were normal.
