[Mucocutaneous lymph node syndrome in Austria -- Report of four cases; one with cardiac arrest. First of two parts: clinical observations (author's transl)].

Since October 1978, four cases of mucocutaneous lymph node syndrome, a disease previously not yet described in our country were observed in our childrens hospital. This particular disease has been described quite frequently in Japan since 1967. It has a characteristic manifestation being mostly benign and self limited. In a few cases however acute cardiac arrest has been described. The first case we observed expired as a result of acute coronary infarction on the twentyfifth day of illness although clinical improvement had been previously noted. At post mortem examination the coronary arteries showed changes consistent with arteriitis. The following three cases presented the more typical benign course of this disease one of them with deformities of the coronary arteries. The clinical synopsis describes the above mentioned cases.

Results of computer tomography on chronic alcoholics.

Sixty adult patients with chronic alcoholism were examined by CT. Psychiatric and neurologic syndromes were given equal consideration. Pronounced brain atrophy was found in 75% of the cases. Cortical, as opposed to subcortical, atrophy was apparent in 64%; it was found especially in the frontotemporal region and cerebellum. Cases of Wernicke's encephalopathy as well as epileptic manifestations revealed global signs of atrophy, not restricted to the brain stem. Further correlations to problems of mental deficiency in alcoholism are discussed together with earlier results.

Computed tomography in detecting calvarial metastases: a comparison with skull radiography and radionuclide scanning.

A clinical study comparing the relative sensitivities of computed tomography, skull radiography, and radionuclide scanning in the detection of skull metastases indicated that CT was the least sensitive of these three modalities. CT could however detect a majority of lesions if scans were viewed at appropriate window settings. Phantom studies showed that the limitations of CT can be related to limited spatial resolution, the density of the lesions, partial volume averaging, and plane of section.

[Preoperative preparation in colorectal surgery: antibiotics (author's transl)].

Out of a total of 2727 operations of the large bowel because of tumors and inflammatory disease, performed over a 14-year period, 897 were one-stage resections of the colon and rectum without relaxing colostomy. Standard preoperative preparation of the bowel consists of a balanced diet, laxatives, and enema supplemented by 9 g Neomycin and 1.8 g Achromycin, within a 2-day period. Disturbances in wound healing occurred in 12.5%, seroma included. Anastomotic insufficiency occurred in 4%, and fatal fecal peritonitis due to tumors in 1.3% and due to diverticulitis in 1.2%. Total mortality was about 5.7%. Postoperative hospitalization after resections because of tumors was 15 days and because of diverticulitis, 19 days.

Primary repair of flexor tendons in the hand without immobilisation-preliminary report.

A new approach to the problem of flexor tendon repair within the fibro-osseous canal is presented. Using a technique of bevelling the tendon ends and suturing with a fine suture material, under magnificaiton, a sufficiently strong junction is obtained, which enables immediate active mobilisation without strangulation of the blood supply. The junction can resist gap formation up to tensions of 4 Kg. It is postulated that under these conditions tendon nutrition is minimally interfered with, adhesions do not form, and the tendon heals by its own intrinsic healing ability.

[Internal carotid artery supply of juvenile angiofibromas (author's transl)].

Twelve juvenile angiofibromas of the paranasal sinuses are described which were partly supplied by branches of the internal carotid artery. Five vessels were found to be involved; with increasing frequency they were: the middle cerebral artery, the dorsal meningeal artery, the ethmoidal arteries, the artery of the inferior cavernous sinus and the artery of the pterigoid canal. Intracranial extension of tumour occurred only once, but the paranasal sinuses were involved in all cases. Occasionally, early filling of veins was seen. The branches of the internal carotid artery played a small part in the arterial supply when compared with branches of the external carotid artery. In spite of this, up to 80% of the tumour could be opacified through internal branches. Bilateral internal artery supply stresses the necessity for selective demonstration of both internal and both external carotid arteries.

[Emphysematous gastritis (author's transl)].

The clinical and radiological features of emphysematous gastritis are described. This is a rare, but dangerous, disease of the stomach. The diagnosis is immediately obvious radiologically, particularly when one keeps the clinical findings in mind. The cause is assumed to be penetration of gas-forming organisms through a previously damaged gastric mucosa.

The base of the skull: a comparison of computed and conventional tomography.

The diagnostic information obtained from 35 examinations of the skull base by computed tomography (CT) was compared with the findings furnished by conventional or blurring tomography (BT). Computed tomography yielded diagnostically valuable information not only on the soft tissues but also on the bony structures. Compared with BT, CT was superior in 40%, equivalent in 40%, and inferior in 20% of the cases in assessing pathological findings involving the bony structures of the skull base. Obviously, CT's additional capability of accurate display of soft tissue changes represents a fundamental diagnostic advantage.

Gastrointestinal somatostatin: extraction and radioimmunoassay in different species.

A radioimmunoassay capable of detecting 300 fg somatostatin has been developed and levels of the polypeptide in gastrointestinal tissues from man, dog, and rat have been measured. Rapid freezing of collected samples and careful control of extraction is necessary. Concentrations in different regions of dog antrum (425 +/- 50 to 773 +/- 254 ng/g tissue) are similar to those in antrum from duodenal ulcer patients and control subjects: 614 +/- 125 and 465 +/- 104 ng/g tissue respectively. Levels in histologically normal human pancreas (253 +/- 43 ng/g tissue) are comparable with those in dog pancreas (333 +/- 66 ng/g tissue), whereas in two cases of neonatal hypoglycaemia the concentration exceeded 3000 ng/g tissue. On gel chromatography the majority of immunoreactive somatostatin elutes as the synthetic tetradecapeptide and a small fraction as a larger species.

Absence status in the adult. New results including computerized transverse axial tomography.

7 adult cases with petit mal status are described; most of these patients had clinical recurrences of petit mal seizures after time intervals ranging from 15 to 50 years. Electroencephalography (EEG) and computerized transverse axial tomography (CT) findings are described and correlated. All CT studies showed frontal lobe abnormalities. When speaking of absence status in the adult, it is useful to distinguish between such an event within the course of primary generalized epilepsy and its reactivation after a very long seizure-free period at an older age. Such reactivations occur preferentially in women. CT supplies more structural information on the living brain and thus we could identify a dysplastic configuration of the skull and hemispheres in a 28-year-old patient and frontocortical and moderate subcortical lesions in the older age-group. The role of the frontal lobe in releasing spike-and-wave discharges of different degrees of generalization appears to be a new aspect worthy of further investigation.

[Extraneural metastasis of an oligodendroglioma in ventriculo-peritoneal shunt].

The clinical and pathological report presents a seven-week old girl suffering from signs of raised ICP. A ventriculo-atrial shunt was inserted and replaced by a ventriculo-abdominal shunt due to occlusion at 15 months of age. At 16 months, craniotomy and resection of an oligodendroglioma was performed and the shunt was preserved. The child died at 18 months due to extensive local recurrence. Autopsy revealed massive peritoneal metastases of the tumor. This is the eleventh published case of canalicular metastases of glioma.

Longitudinal studies of plasma aldosterone, corticosterone, deoxycorticosterone, progesterone, 17-hydroxyprogesterone, cortisol, and cortisone determined simultaneously in mother and child at birth and during the early neonatal period. I. Spontaneous delivery.

In order to obtain the still lacking reference data of individual plasma steroids in the immediate postnatal period needed for the assessment of adrenocortical function in various neonatal maladaptation syndromes, aldosterone (A), corticosterone, deoxycorticosterone (DOC), progesterone (P), 17-hydroxyprogesterone (17-OHP), cortisol, and cortisone were simultaneously followed in the same human newborn in a single 250-500 microliters peripheral plasma sample obtained at constant times during the first week of life using a mechanized Sephadex LH-20 multicolumn chromatography and standardized RIAs. Mean concentrations in 12 spontaneously delivered full term newborns of either sex and in paired umbilical (UV) and peripheral maternal (MV) venous plasma are given in the table. Besides significant maternoumbilical gradients in each steroid, DOC, P, 17-OHP, and cortisone, originating predominantly from the fetoplacental unit, disappear rapidly with steadily increasing half-lives. A, corticosterone, and cortisol, however, remain elevated in comparison with later infancy, with the exception of a marked "glucocorticoid dip" in cortisol and corticosterone levels between 2 and 12 h after birth.

Investigation of reference values of components of cerebrospinal fluid.

In order to obtain reference values, random samples of CSF from 70 women and 69 men were analyzed for various components, including total protein and protein fractions. Using statistical methods, data were also collected for sex differences and the influence of age. Small variations in the values of total protein, albumin and gamma-globulin between males and females were found, and slightly higher values for the gamma-globulin were found with increasing age.

Dose-response study with a new LH-RH analogue, D-Ser (TBU)6 LH-RH 1-9 (EA)10 during the follicular phase of the menstrual cycle.

D-Ser (TBU)6 LH-RH 1-9 (EA)10 (HOE 766) a highly active LH-RH analogue, was studied with regard to its effects on the release of follicle stimulating hormone (FSH), luteinizing hormone (LH) and oestradiol-17beta (Oe2) during the follicular phase of the menstrual cycle. Forty-two regularly menstruating women were allowed to five different treatment groups with different doses (1.25 microgram; 2.5 microgram; 5.0 microgram; 10.0 microgram; 20.0 microgram) of HOE 766 given as intravenous bolus injections and the plasma concentrations of FSH, LH and Oe2 were measured up to 24 h after injection using specific radioimmunoassays. In the majority of cases, peak values of both FSH and LH occurred 4 h after injection being significantly different from pre-injection levels (P less than 0.02 in the 1.25 microgram treatment group, P less than 0.005 for the other treatment groups). Statistical analysis of maximum values as well as the absolute and relative increase in the different treatment groups revealed a dose-dependent effect of HOE 766. Maximum values of Oe2 occurred 8 h after injection and were found to be significantly different from pre-injection levels (P less than 0.005). However, no dose dependent effect was observed. It was concluded that HOE 766 is a potent and long-acting stimulator of FSH, LH and OE2 release in women. The effect of HOE 766 is dose dependent for FSH and LH but not for Oe2.
