[Malignant goiter. Diagnostic problems and surgical procedure].

By our own total of 6973 benignant and 188 malignant goitres all problems are discussed concerning finding of the diagnosis preoperatively, during operation and post-operatively and also tactical procedure. Since 1974 the rate of extirpations of the malignant goitres was 87.8% while in 8.7% the only possibility was the reduction of tumor mass, palliatively.

Dissociation between interferon production induced by phytohemagglutinin and concanavalin A in spleen cell cultures of nude mice.

Spleen cells of homozygous nude mice did not react with lymphoproliferation to both phytohemagglutinin (PHA) and concanavalin A (Con A), and there was no detectable interferon production in response to Con A. However, PHA-induced interferon production was equal in spleen cells of nu/nu mice and nu/+ mice. One possible explanation for these findings is that a subset of T cells responds to PHA with interferon production different from that responding to Con A with interferon production.

Superiority of blood cardioplegia over asanguinous cardioplegia--an experimental and clinical study.

This study compares experimental and clinical results using both an asanguinous and sanguinous vehicle for delivering the cardioplegic solution. Animals receiving blood cardioplegia had better left ventricular function after unclamping, significantly less post-ischemic myocardial edema, and a better ability to augment flow and oxygen consumption. Patients receiving blood cardioplegia had also better myocardial performance with high cardiac outputs and lower left atrial pressure, and showed less evidence of myocardial damage. We conclude that oxygenation of the cardioplegic solution provides superior myocardial protection to that seen when the same solution is used in an asanguinous vehicle.

[Early effects of clonidine, epinephrine, and pilocarpine on the intraocular pressure and the episcleral venous pressure in normal volunteers (author's transl)].

In four different groups of eight normal volunteers the intraocular (IOP) and episcleral venous pressures (EVP) were measured before, and at 7.5, 15, 30, 45, and 60 min after topical application of one drop of clonidine (1/4%), epinephrine (1%), or pilocarpine (2%) by means of applanation tonometry or the air-jet method, respectively. The results were compared with a control group. In the control group the IOP decreased by 16% until the fourth measurement. Subsequently it increased to -10% after 60 min. The EVP remained unchanged. After clonidine application the IOP decreased throughout the whole experiment (-31% after 1 h) and after the third measurement the IOP decrease was significantly more marked than in the control group. The EVP I diminished significantly during the first 15 min by 25% and then increased, reaching its initial value after 60 min. IOP and EVP I showed parallel behavior only during the first 30 min. Topical application of epinephrine was followed by a much smaller decrease in IOP (17% after 60 min); compared to the control group the differences were minor. The EVP I was reduced significantly by 15% below the initial level after 45 min while EVP and IOP behaved identically. After pilocarpine application the IOP rose initially (7% after 7.5 min) and decreased markedly after 10 min (33% after 1 h). Compared to the untreated volunteers there were significant differences at nearly all times. At the outset the EVP I increased significantly (29% after 7.5 min), and then decreased again until it regained the initial value after 30 min. Only at the beginning of the experiment were the reactions of IOP and EVP similar.

Diagnostic CSF analysis in neurological practice.

In order to establish the diagnostic value of clinical chemical analyses of C.S.F. samples, the C.S.F. data from seven hundred patients have been evaluated. The authors discuss the influence this investigation has exerted on their differential diagnostic evaluation.

[BCG-infection in chronic granulomatous disease (author's transl)].

A 7 year old boy developed in the newborn period a chronic suppurative process after routine BCG vaccination beginning at the site of the injection and spreading to the adjacent areas on neck and chin. A supraclavicular lymphadenopathy was also noted. Serial histological examinations revealed the typical histopathological pattern of tuberculosis and the boy received a tuberculostatic therapy for five years. During this time he suffered from multiple chronic bacterial infections which led to chronic granulomatous inflammations in different organs and to a fibrous pneumonitis with subsequent cor pulmonale. At the age of 6 years a negative NBT-test allowed the diagnosis of GCD. Consequently therapy with Sulfamethoxazol-Trimethoprim was started and the rate of infections diminished markedly.

[Prolactin in serum and anamnestic data of women with cystic disease of the breast (author's transl)].

78 women with cysts of the mammary gland--proved by aspiration of cystic fluid--had significantly more hyperprolactinaemia than 42 control-persons. Hyperprolactinaemia was twice as frequently found in 46 women with microscopic cystic disease--proved histological in biopsies--than in control-persons. On the average there was a moderate elevation of prolactin. Individual prolactin-levels were up to four or five times higher than normal. The comparison between prolactin and gynecologic data and anamnestic known endocrine and metabolic lesions was without significance. There was no relation between use of prolactin-stimulating drugs (psychopharmacas a.o.) and cystic disease of the breast. Patients with cystic disease used significantly less oral contraceptiva and more estrogens than control-persons. There was no familiar-genetic disposition significant. Our results show prolactin as an etiologic factor of (gross-) cystic breast disease.

A constant tension dynamic splint.

We have presented a new method of dynamic splinting that allows a greater range of protected motion under constant tension. The device has been designed for use with newer methods of tendon repair in situtations where early controlled motion is allowed.

[Bioavailability of cyproterone acetate after oral and intramuscular application in men (author's transl)].

Cyproterone acetate was administered orally and intramuscularly to 5 male patients. The level of the active substance in plasma was monitored at intervals during the period of application using radioimmunoassay specific for cyproterone acetate. The areas under the drug level curves were similar after oral and intramuscular application of cyproterone acetate when taking into consideration the different doses used. Accordingly, cyproterone acetate is not dependent upon a "first-pass" effect. The bioavailability of the active substance was complete following oral application.

Effects of methylprednisolone on P50, 2,3 diphosphoglycerate and arteriovenous oxygen difference in acute myocardial infarction.

In a double-blind randomized study, 30 mg/kg of methylprednisolone sodium succinate (MPN) or 15 mg/kg of mannitol placebo (PL) were infused in 28 patients after acute myocardial infarction. Measurements were obtained immediately before and after for 24 hours after the initial infusion. The partial pressure of oxygen at 50% saturation of hemoglobin (P50) did not change significantly in vitro or in vivo after MPN, whereas 2,3 diphosphoglycerate (2,3 DPG) increased from 13.2 to 14.2 mumol/g Hb (p < 0.05) in the group receiving PL. The arteriovenous oxygen difference (Ca-VO2) remained constant after MPN or PL. The cardiac index (CI) increased after MPN (p < 0.02) associated with an increase in the oxygen consumption index (CI X A-V O2) from 146 to 170 ml/min/m2 (p < 0.05). These data show that MPN increases CI after acute myocardial infarction, but has no specific effects on P50, 2,3 DPG or Ca-VO2.

[Improvement in nitrogen sparing by branched amino acids during parenteral nutrition].

Two groups of ten patients each, staying in a Intensive Care Unit, received exclusive parenteral alimentation with different concentrations of branched chain amino acids (BCAA). Group A received studied solution containing 12 g N (852 mmol) with 41,1 p. 100 BCAA. Group B received control solution containing 18 g N (1278 mmol) with 21,1 p. 100 BCAA. Every patient received 60 nonprotein Kcal x kg-1 body weight/24 hr. The study was performed on five days. Measured parameters were: daily (D0 to D5) N balance, blood urea N and total protein and urinary alpha amine N, and after therapy (D6) amino acid levels. Results have shown no statistical significant difference between both groups. N balance became less negative during the therapy. Amino acid levels were in the normal range and alpha amine urinary elimination remained low in patients with parenteral nutrition. These results indicate that 12 g (852 mmol) N in BCAA rich solution are as efficient as 18 g (1278 mmol) N in standard solution, allowing a N sparing.

[Hypophysectomy in the advanced stage of carcinoma of the prostate (author's transl)].

22 patients with advanced prostatic cancer underwent transnasal-transsphenoidal hypophysectomy. 2 patients died immediately after the operation, when this method was introduced. Of 19 patients, who were followed at short time intervalls, 13 = 68,5% showed a good remission, which lasted on an average of 5.5 (3-8) months. The survival time of these patients with a good remission was 11.5 months, whereas in patients without any remission it was 4.8 months. We recommend the hypophysectomy in individual cases as a possible form of treatment, which might lead to relief of pain and to objective tumor remission.

New development in surgical andrology. Alloplastic spermatocele.

We developed a cup-shaped silicone-dacron prosthesis that may be used as a sperm reservoir. Spermatozoa aspirated from this reservoir and stored as pellets in liquid nitrogen for 6 weeks induced pregnancies and normal births in 50 per cent of inseminated cows. Because of aplasia or long stenosis of the vas deferens 26 prostheses were implanted upon the epididymis in 14 patients. Percutaneous aspirations up to 7 months postsurgery showed 0.3 to 105 million spermatozoa the quality and quantity of which decreased with time. Insemination of patients' wives has not resulted in pregnancy.
