[Suprapubic prostatic sonography in the diagnosis of carcinomas and adenomas of the prostate (author's transl)].

Prostatic adenomas and carcinomas were examined by suprapubic transvesical prostatic sonography. The value of the method is comparable with transrectal prostatic sonography. Adenomas and carcinomas can be differentiated on the basis of their echo patterns and marginal contours. In the presence of carcinomas, sonography is a valuable addition to clinical staging and staging by CT. Compared with CT, the ability to perform longitudinal sections permits good evaluation of the bladder base. Sonography is also valuable for serial observations after treatment and for diagnosing recurrences.

[Problems of Hodgkin's disease in childhood].

Many publications about treatment results of Hodgkin's disease in adults give a rather optimistic outlook. Our experience with the treatment results of 10 children between 1970 and 1980 is rather disappointing due to late side effects of the combined treatment modalities. These complications of therapy and guidelines form the management of various stages of Hodgkin's disease are discussed.

Psychodynamics and therapeutic aspects of anorexia nervosa. A study of family dynamics and prognosis.

38 patients who had suffered from anorexia nervosa were examined and followed up in a catamnestic study, and their biography, family dynamics, eliciting situation and history of illness were studied. The investigation was carried out by means of interviews, questionnaires and personality tests. As to biography and family dynamics, our results are partly complementary and partly contradictory to previously published results of other investigations. Our patients received inpatient psychotherapy over an average period of 3 months and were followed up 2-5 years later. The following prognostic criteria proved to be significant: vomiting as part of the symptoms; chronification (psychotherapy delay), and ability to achieve a constructive therapist/patient alliance. Patients whose weight was largely back to normal by the end of therapy, but were still amenorrhoeic, showed unfavourable development in psychosocial relations. The catamnestic results improved with increasing time lapse between the end of therapy and follow-up investigation.

Use of methylprednisolone in patients following acute myocardial infarction. Hemodynamic and metabolic effects.

Hemodynamic and metabolic effects of methylprednisolone were investigated in a double-blind study of 28 patients with acute myocardial infarction (AMI), confirmed by unequivocal electrocardiographic and enzyme changes. Measurements were performed prior to and at 1.5, 3, 4, 4.5, 12 and 24 hours following infusion of methylprednisolone (13 patients) or placebo (15 patients). Although systemic vascular resistance decreased from 1,750 to 1,420 dynes . sec . cm-5 (p less than .001) and cardiac index increased from 2.77 to 3.10 L/min/m2 (p less than .02) between 0 and 4.5 hours, an abnormal increase in blood lactate was observed in 10 of the 13 patients following administration of methylprednisolone (3.0 vs 1.2 mM/L, p less than .001). Lactate elevation appeared one hour after infusion of methylprednisolone, was maximal at 12 hours, and persisted for more than 24 hours. There was no significant change in blood lactate in placebo treated patients. A transient but significant decrease in plasma volume was also observed following infusions of methylprednisolone. The elevation of blood lactate could not be explained by the reduction in plasma volume since the most striking increases in lactate were observed 12 hours following the initial infusion of methylprednisolone when the plasma volume was returning to the control value. No significant differences in other hemodynamic or metabolic parameters, infarct size or patient survival were observed between the two groups. We conclude that the hemodynamic benefits of glucocorticoids characterized by increased cardiac output and lowered systemic vascular resistance are counterbalanced by the potentially unfavorable conditions of lactate elevation and volume depletion.

Outpatient evaluation of dietary intake and nitrogen removal in continuous ambulatory peritoneal dialysis.

Five patients on continuous ambulatory peritoneal dialysis were studied to evaluate dietary intake, glucose absorption from the dialysate, and urinary and dialysate nitrogen removal. The study was done on an outpatient basis, while the patients ingested their usual diets and went about their usual activities. The results indicate that patients greatly overestimated their caloric and protein intake, which was 0.71 to 0.96 g/kg bodyweight per day. Dialysate glucose contributed considerably to total caloric intake, because approximately 75% of of the dialysate glucose was absorbed independent from total glucose administered, providing 20% to 29% of the total caloric intake. We concluded that protein requirement in clinically stable continuous ambulatory peritoneal dialysis patients still needs defining because the patients may be anabolic with lower protein intakes than previously recommended.

[Flow cytometric determination of DNA distribution patterns of malignant and benign tumors of the prostate (author's transl)].

56 samples of prostates, clinical suspected of being carcinomas, were examined by flow cytometry concerning their DNA distribution patterns. Carcinoma was diagnosed in 25 cases. All histologically benign samples showed a G1-fraction higher than 95%. Accordingly all cases with a G1-fraction lower than or equal to 95% were interpreted to be flow cytometrically positive (= malignant). The results of flow cytometric measurements essentially depend on the relation of the compounds of the sample--carcinomatous and normal tissue. Concerning the DNA distribution pattern, highly differentiated carcinomas do obviously not always differ from normal tissue. Hence they can escape from flow cytometric proof. An improvement of the flow cytometric distinction between histologically malignant and benign samples is achieved by the introduction of correction procedures for background and cell aggregations.

The suitability of carboxymethylcellulose as a vehicle in reproductive studies.

The 2% aqueous solution of a sodium carboxymethylcellulose (CMC) was administered by gavage to 20 male and 40 female albino rats at the daily volume of 1 ml/100 g body weight. The males were treated for at least 60 days and the females for at least 14 days prior to mating and during a 6-day mating period. In one half of the females treatment was continued until sacrifice on day 14 of pregnancy; in the other half of the female treatment was continued until weaning of the progeny. A variety of tests was used to examine the behavioural performance of the pups. The overall results of this study were comparable for both test groups, CMC treated and untreated. Consequently, the CMC used here is considered to be a suitable vehicle for suspending tests materials for oral administration in long-term reproductive studies in rats.

Critical importance of ensuring cardioplegic delivery with coronary stenoses.

This study examines the relative importance of the duration of ischemia versus the adequacy of cardioplegic distribution and protection in hearts with coronary stenoses. Of 18 dogs on cardiopulmonary bypass, 12 underwent critical narrowing (greater than 90%) of the left circumflex artery (LCA) and total occlusion of the anterior descending coronary artery (LAD). In six dogs (control) the coronary arteries were patent. A 16 degrees C blood cardioplegic solution was given at 20 minute intervals of aortic clamping. In control dogs and in six dogs with stenoses, the aorta was clamped for 60 minutes. In the latter group, the stenoses were removed after 20 and 40 minutes to simulate sequential completion of grafts and better cardioplegic distribution. In the remaining dogs with stenoses, the aorta was clamped for only 30 minutes, with stenoses removed after the heart had been returned to the beating empty state for 30 minutes to simulate doing distal grafts with cardioplegic protection and proximal grafts during reperfusion (traditional technique). With sequential grafting, myocardial temperature was lower (16 degrees C versus 22 degrees C) and incidence of reperfusion fibrillation less than with the traditional technique. Despite a greater ischemic interval, sequential grafting with adequate cardioplegic distribution resulted in less lactate washout (5 +/- 15 versus 35 +/- 6 cc/100 gm/min), greater recovery of compliance, and higher stroke work indices (1.32 +/- 0.12 versus 0.75 +/- 0.15 kg-m/min). We conclude that the success of myocardial protection with potassium cardioplegia in hearts with coronary stenoses is related more to ensuring its distribution than to limiting the duration of ischemic arrest with the false assumption that the heart is reperfused adequately while proximal grafts are completed in the beating empty state.

[Cell hybridization: a monoclonal marker as a diagnostic help in haematology and oncology (author's transl)].

This is a communication on the introduction of the first monoclonal marker at Graz University Medical School. Human peripheral blood mononuclear cells were used for immunisation of BALB/c mice by injecting 4,5 X 10(6) cells s. c. Boosting consisted of i.p. injection of 5 X 10(6) cells 4 times in monthly intervals. Spleen cells were taken 4 days after the last boost, fused with NS-1 myeloma cells, using PEG as fusogenic agent. After growth in HAT selective medium, antibody secreting clones were identified by testing the supernatants. Cultures with activity against lymphocytes were closed on normal BALB/C peritoneal cell feeder layers. One of them secreted IgG 1 with strong activity against all lymphoid cells and was named HLy D 1. Further testing showed activity with band cells, polymorphs, eosinophils, macrophages but not with tissue sections from anaplastic undifferentiated cancers and erythroid leukaemias. Since he was named H Le D 1 and introduced for differentiating rare undifferentiated carcinomas from malignant tumors of the lymphoid system.

[Intrapericardial rhabdomyosarcoma in infancy (author's transl)].

At the age of three months an infant rapidly developed signs of cardiac failure as well as in- and exspiratory stridor, caused by an intrathoracic tumor. Thoracotomy and biopsy revealed an intrapericardial tumor, histologically myxosarcoma. In spite of chemotherapy and radiation the infant died at the age of seven months due to multiple intracerebral metastases now histologically rhabdomyosarcoma. This is one of the rare cases of primarily malignant intrapericardial tumors in infancy, and also shows the possible pleomorphism of childhood rhabdomyosarcoma. We know only one further case of pericardial rhabdomyosarcoma where similar histologic changes have been observed.

[Reactive lymphadenopathy-enlargement-pseudolymphoma--prelymphoma].

In this first of two articles on reactive and lymphomatous lymph-node enlargement, based several own cases an overview of reactive lymphadenopathies is given. A short presentation of the subpopulations of the lymphatic system and its function is followed by tables and case reports illustrating the individual reaction forms. The include abnormal storage, inflammatory and infctious processes, immunological disorders and hamartomatous lesionsl. Certain reactive disease entities which clinically and pathologically closely resemble lymphomas are designated as pseudolymphomas. Prelymphomas include those processes which in part may lead to a malignant lymphoma. Finally, practical aspects of lymphode diagnostics (localization factor, clinical-pathological correlation) are discussed.

A comparison between abdominal ultrasonography and computed tomography.

Computed tomography (CT) and ultrasonography are based on entirely different physical principles. In the abdomen the two techniques have similar diagnostic applications. Whereas CT is in many instances more informative, it is also a more time-consuming and much more costly procedure. For clinicians who practise in areas where these techniques are available, this article may be useful as a quick guide to the choice of procedure.

The importance of early ultrasonography in jaundice.

The ultrasonographic findings in 3 patients with longstanding jaundice of clinically unsuspected obstructive nature are presented and discussed. Attention is drawn to the importance of early ultrasonography, even when the clinical and biochemical picture is not necessarily in keeping with a diagnosis of obstructive jaundice.
