[CT findings in chordomas of the base of the skull].

Nine chordomas of the skull base and their CT appearances are described. Destruction of the clivus and of the parasellar portions of the middle fossa are characteristic, as is calcification which may be spotty, coarse or ill-defined. The tumour is characterised by marked enhancement following contrast but, on the unenhanced CT, contrary to statements in the literature, it is hypodense or isodense with respect to cerebral tissue. The tendency to recurrence and an altogether poor prognosis is confirmed.

[Mycotic thrombosis as a rare complication of Shaw's central silastic catheter in long-term parenteral feeding in a premature infant].

A 26 days old very-low-birthweight infant with an indwelling silastic-catheter and suddenly occurring symptoms of pulmonary hypoperfusion is reported. At post-mortem examination a mycotic-thrombus originating from the right atrium was found obturating the tricuspid valve. Despite of the very low incidence of complications with central silastic-catheters one should consider thromboembolic occlusion within the heart or the great vessels in any case of an unexplained sudden deterioration with profound hypoxemia.

[Studies on B cell activation in collagenoses].

An inverse correlation between PWM and anti mu responses of peripheral blood lymphocytes, and a close relationship to clinical activity were demonstrated in 9 patients with systemic lupus erythematosus.

[Correlation of clinical and computed tomographic findings for therapy and prognosis of acute pancreatitis].

A prospective study was performed in 156 patients with acute pancreatitis to find out the value of computed tomography for the diagnosis and early prognosis. The CT finding leads to a diagnosis and differentiation in 3 risk groups similar to the clinical staging. The prognostic value of CT depends on the reproducible grading of pancreatic swelling and the grade of extrapancreatic fluid collections. CT finding correlates with early prognostic factors and the clinical staging, leading to different risk groups which allows an adequate stage-depending therapy of patients with acute pancreatitis.

[Significance of hemodynamic sequelae of aortic ligation in infrarenal aneurysms of the abdominal aorta].

The operative mortality in abdominal aortic aneurysm repair is in large part attributable to a high incidence of myocardial infarction. This is a result of cardiovascular instability during aortic cross-clamping and declamping in patients with coexistent coronary artery disease. Therefore cardiodynamics (pulmonary arterial wedge pressure, PAWP; cardiac index, CJ) were studied in 31 patients during abdominal aortic aneurysm surgery. 12 patients (control) with a PAWP mean of 8 mmHg preoperatively showed impaired cardiac function after declamping and a significant fall in arterial pressure. 19 patients were volume loaded to a PAWP greater than 12 mmHg and the cyclo-oxygenase inhibitor Aspirin was given preoperatively. This resulted in improved cardiac performance with no fall in arterial pressure after declamping. Optimal volume loading and cyclo-oxygenase inhibition have the ability to prevent adverse hemodynamic responses to aortic clamping and declamping. Maintenance of optimal cardiac performance will reduce cardiovascular complications and postoperative mortality in abdominal aortic aneurysm repair.

Intrinsic tendon fibroplasia: documentation by in vitro studies.

There has been considerable controversy concerning the capacity of tendons to heal intrinsically without adhesion formation. To help clarify this issue, a tissue culture model was developed to study the potential of tendon fibroblasts to proliferate and migrate. Avian flexor tendons, isolated free of sheath and all synovial components, were maintained in sterile culture plates with Dulbecco's Modified Eagle Medium. After 5 days in culture, the 2 mm tendon biopsies were analyzed for DNA synthesis [( 125I])iododeoxyuridine incorporation) and migration of fibroblasts out of the explant (planimeter analysis). DNA synthesis was maximal on day 5 in culture, and tendon fibroblasts were observed migrating from the biopsy within 48 h. Addition of cells from plasma resulted in a significant increase in fibroblast migration, and the presence of fibrin in the plasma clot was an absolute requirement for the migratory process. These findings further support the hypothesis that injured tendons can heal by an intrinsic healing process in addition to healing by adhesion formation.

[Computed tomography in craniocerebral injuries in childhood].

In accident diagnosis, the computed tomogram (CT) is nowadays decisive for the determination of craniocerebral traumas. This is true for children as well as for adult patients. Besides the rapid handling and the non-invasiveness, the exact diagnosis with direct imaging of the traumatic cranial lesions is a special advantage of computed tomography. The authors present the characteristic CT modifications of the epidural hematoma, the subdural hematoma and traumatic subdural hygroma, the brain contusion, traumatic subarachnoidal hemorrhage and ventricular hemorrhage. Furthermore the injuries to the osseous skull which can be represented by CT and the late consequences of the accident are mentioned. The special aspects of craniocerebral traumas in children are taken into consideration.

Ultrafiltrable plasma testosterone and 5 alpha-dihydrotestosterone as measured across the human testes.

It has been confirmed that within the human plexus pampiniformis a venous-arterial transfer of testosterone (T) (5.95%) and of dihydrotestosterone (DHT) (16.58%) occurs. It could be established that in the testicular artery the percentage of ultrafiltrable DHT was significantly lower compared with the venous compartment. It is assumed that by this means a biological recycling of the active androgen DHT into the testis is achieved.

[Intrauterine ultrasonic tomography (IUT): comparison with macroscopic sections].

Clinical gynaecological examination and the imaging techniques in present use are insufficient for an accurate diagnosis of changes of the myometrium. The intrauterine ultrasonographic tomography (IUT) permits the differential diagnostic imaging of normal myometrium, leiomyomata, uterine carcinomata and sarcomata . There was good correlation between intra-uterine ultrasonograms and sections at the same level of selected normal and pathological hysterectomy specimens.

[Intrauterine ultrasonic tomography].

The imaging methods used to date yield unsatisfactory information on the fine structure of the myometrium. Intrauterine sonographic tomography enables imaging of the normal myometrium, of myomas and carcinomas, with an exact determination of location, extent and above all infiltration depth of pathological changes.

[Intravenous digital subtraction angiography in the detection of spinal angiomas].

In every case in whom the myelogram suggests abnormal vascularity, a spinal arteriogram by means of intravenous DSA is indicated. This should be regarded as a screening method in order to confirm the suspected diagnosis of a spinal angioma and to ascertain its extent. After embolisation or surgery, intravenous DSA is a suitable method for observing the result of the treatment.

Breast reconstruction using an inflatable breast implant with detachable reservoir.

An inflatable breast implant having a detachable filling reservoir is described. Once in position, the implant can be filled under controlled conditions postoperatively. The implant functions either as a tissue expander or as a delayed-filling implant in the initial stages of the procedure. Once the desired breast size is achieved, the reservoir is removed, leaving the filled implant in position. This implant has been used successfully in various types of breast reconstructive procedures, including both primary and secondary reconstruction following modified mastectomy, immediate reconstruction following subcutaneous mastectomy, secondary reconstruction following radical mastectomy combined with the latissimus dorsi flap, and following the removal of silicone gel implants with associated capsular contracture. A total of 23 patients, representing 34 breasts, are reported on. The results have been encouraging over a period of 20 months of follow-up.

Antihypertensive effect of the GABA receptor agonist muscimol in spontaneously hypertensive rats. Role of the sympathoadrenal axis.

The antihypertensive action of central GABA-ergic stimulation was investigated in conscious stroke prone spontaneously hypertensive rats. Injection of the potent GABA agonist muscimol (0.01-1 microgram) into the lateral brain ventricle (icv) lowered mean arterial blood pressure (192.1 +/- 8.4 mm Hg) dose-dependently in stroke prone spontaneously hypertensive rats with a maximal fall of -52.7 +/- 5 mm Hg lasting for about 90 minutes. This was accompanied by bradycardia and sedation. Pretreatment with atropine (2 mg/kg, ip, or 15 micrograms/kg, icv) did not significantly influence the muscimol-induced fall in mean arterial pressure. In normotensive (109.3 +/- 1.9 mm Hg) Wistar-Kyoto controls, the maximal decrease in mean arterial pressure was -12.1 +/- 1.6 mm Hg from 109.3 +/- 1.9 mm Hg, and the duration of the effect was much less than in stroke prone spontaneously hypertensive rats, Following 1 microgram muscimol, icv, plasma noradrenaline did not fall significantly in stroke prone spontaneously hypertensive and Wistar-Kyoto rats, but in stroke prone spontaneously hypertensive rats, plasma adrenaline was fully suppressed (from 118.1 +/- 24.2 to 22.8 +/- 5.7 pg/ml) throughout the depressor response. The efferent sympathetic nervous activity as directly recorded from the n. splanchnicus was similar in conscious stroke prone spontaneously hypertensive and Wistar-Kyoto rats, and was moderately reduced in both strains by 1 microgram muscimol, icv.(ABSTRACT TRUNCATED AT 250 WORDS)
