In an attempt to characterize immunological reactions in patients with progressive systemic sclerosis (PSS) mononuclear cells were tested for their ability to secrete interleukin-2 (II-2), and for their response to the T-cell independent B-cell mitogen anti-mu sepharose. Compared with patients with systemic lupus erythematosus (SLE), patients with PSS showed a marked enhancement in their response to anti-mu. With regard to II-2 secretion mononuclear cells from PSS patients tended to show increased responses as well. Our data provide evidence for hyperreactivity at both the T- and B-cell level in PSS.
A Blalock-Taussig-anastomosis was performed at the age of 2 years in a boy with transposition of the great arteries, ventricular septal defect, and pulmonary atresia. Nine years later he developed a transient aphasia. Cranial computed tomography (CT-scan) revealed a structure compatible with brain abscess. The boy was conscious and no neurological deficit was found. Initial therapy consisted of Ampicillin, Tobramycin, and Metronidazole, 12 days later an acute hemiplegia developed. 30 ml of pus were aspirated from the brain abscess, and the boy's condition dramatically improved. Streptococcus milleri was found bacteriologically and antibiotic therapy was continued over six weeks. Serial CT-scans during and after therapy demonstrated disappearance of the brain abscess. The presented case shows that after a shunt procedure in cyanotic heart disease right-to-left-shunting and therefore chronic oxygen desaturation and polycythemia are still present as facilitating factors for focal encephalomalacia, cerebritis, and brain abscess. In case of short duration of neurological symptoms and a size of abscess less than 4 cm in diameter antibiotic therapy without total excision may eliminate the infection.

The authors report on a patient suffering from centrocytic non-Hodgkin's lymphoma. Double contrast examination of the colon showed multiple, partially pedunculated polyps in the terminal ileum and colon. As cause of these lesions involvement of the bowel by lymphoma could be proved by biopsy. The differential diagnosis against other types of polyposis is discussed and the variable radiographical manifestations of the disease known as "multiple lymphomatous polyposis" are pointed out.
The permanent tissue expander has many advantages over a fixed-volume implant. Chest-wall skin can be expanded, enabling safe reconstruction of an aesthetically pleasing breast without the use of flaps. Excessive pressure on compromised skin flaps is avoided, and larger breasts can be achieved postoperatively by staged expansion. It appears that early capsular contracture can be treated by overexpansion of the implant and subsequent removal of saline to decrease the pressure. A second operative procedure to change an implant that is too small is unnecessary, and greater symmetry of the breasts can be obtained by adjusting the volume of the implant postoperatively. The expander has proven to be of great benefit in augmentation mammaplasty, the creation of symmetric breasts that are of a size and shape that completely satisfies the patient's demands. This is facilitated by the use of an implant that permits the patient and surgeon to effect expansion and volume adjustments. The patient can be more confident that her conception of an acceptable physical appearance will be realized, and the surgeon can thus decrease the incidence of patient disappointment. The implant has been used to reconstruct breasts following mastectomy (secondary reconstruction), at the time of mastectomy (primary reconstruction), following subcutaneous mastectomy, in Poland's syndrome, in breast augmentation, and even in buttocks asymmetry.
In a case report, idiopathic segmental infarction of the greater omentum is demonstrated for the first time by means of computed tomography. This rare disease of unknown etiology can mimic the clinical presentation of acute appendicitis or cholecystitis.
The essential oil of anise (fruits and shoots) was investigated focusing on the composition of the hydrocarbon fraction. Several sesquiterpenes were identified by GC-MS and the relative composition of the fractions was established by GC analysis. gamma-Himachalene and the diterpene neophytadiene were isolated by TLC and column chromatography at low temperatures. Their structures were determined by MS and NMR including 1H-1H correlated COSY and NOE experiments.
