Nipple-areola reconstruction using intradermal tattoo.

A technique of nipple-areola reconstruction with intradermal tattoo is presented. By using various colors, it is possible to tattoo a nipple-areola complex onto the breast that will have an illusion of projection. This technique alone is acceptable to older patients. In younger patients, or in those in whom nipple projection is desired, a simple technique of using a subcutaneously based nipple pedicle is found to be highly effective.

[3-dimensional cranial and spinal computed tomography].

3-D CT images can be reconstructed with special software programs. Testing the importance of 3-D imaging for cranial and spinal CT was the aim of this study. The images were computed in 81 patients on the base of the CT data. 3-D views provided essentially the same diagnostic information as the conventional CT images but in a more useful form. The conditions are more apparent and the relationship between normal and abnormal structures is more easily understood. From this point of view, 3-D imaging makes it possible to improve communication between the radiologist and the attending physician.

[Liver diseases in children and their relatives with homozygous and heterozygous alpha 1-antitrypsin deficiency].

Over a 6-year period 26 infants and children with homozygous (2 Z and 6 ZZ) or heterozygous alpha 1-antitrypsin deficiency (12 MZ, 6 MS) were observed prospectively and their families investigated. 7 of 8 homozygous patients had neonatal hepatitis, whereby 3 of these showed maximum transferase activities during the 5th to 9th months of life. At the age of 7 years 2 of these patients were clinically normal, but only one patient had normal transferases. One patient had cirrhosis with portal hypertension at the age of 16 years 6 months; her nephew showed hypersplenism. Family studies revealed a further 5 relatives of phenotype Z, 16 of phenotype MZ, 3 of phenotype SZ and 1 of phenotype MS; 6 of these had slightly elevated serum transferase activities. 6 patients of phenotype MZ and 2 patients of phenotype MS had neonatal hepatitis but generally with a much better prognosis than in homozygous patients. The other heterozygous patients (6 MZ and 4 MS) had a variety of additional factors determining the disease and the prognosis. Family studies showed a further 7 family members of phenotype MZ, and 2 of phenotype MS; 2 of these had slightly elevated transferase activities, 3 parents had hereditary hyperbilirubinaemia.

Influence of the spread and period of retention of Iotrolan in the subarachnoid space on the side effects rate in myelography.

Fifty patients underwent myelography with the newly developed CSF-isotonic, dimeric, non-ionic contrast medium Iotrolan. Repeat spinal and cranial computerised tomographic studies with measurement of the attenuation values were conducted to demonstrate the administered contrast medium in the spinal canal and intracranial subarachnoid space. The patients were examined neurologically, observed clinically and asked about concomitant symptoms. The period of retention and the spread of the contrast medium in the subarachnoid space was not found to have any influence on the side effects rate. In particular, no association was confirmed between intracranially demonstrable contrast medium and the occurrence of side effects.

Concentrations of unconjugated 5 alpha-androstane-3 alpha, 17 beta-diol and 5 alpha-androstane-3 beta, 17 beta-diol and their precursor in human testicular tissue. Comparison with testosterone, 5 alpha-dihydrotestosterone, estradiol-17 beta, and with steroid concentrations in human epididymis.

The concentrations of testosterone and its tissular metabolites were determined in testicular and epididymal tissue obtained from eleven male subjects (aged 65-85 years) after orchiectomy for prostatic cancer. The steroids were measured in different tissular compartments, i.e. testis, caput, corpus and cauda epididymis. The values (mean +/- SD; ng/g wet weight) were: Testosterone 724.0 +/- 286.0, 32.08 +/- 2.56, 41.45 +/- 1.77 and 32.24 +/- 2.14; 5 alpha-dihydrotestosterone 6.95 +/- 1.99, 9.76 +/- 2.33, 16.87 +/- 0.21 and 15.79 +/- 2.67; 5 alpha-androstane-3 alpha, 17 beta-diol 6.07 +/- 2.33, 2.17 +/- 0.24, 1.93 +/- 0.02 and 1.17 +/- 0.20; 5 alpha-androstane-3 beta, 17 beta-diol 56.66 +/- 20.97, 3.55 +/- 0.19, 2.21 +/- 0.27 and 3.34 +/- 0.32; estradiol-17 beta 5.36 +/- 3.0, 1.08 +/- 0.014, 1.44 +/- 0.038 and 1.47 +/- 0.03, respectively. Incubation of human testicular tissue with [3H]androst-5-ene-3 beta, 17 beta-diol or [3H]dihydrotestosterone showed that both androstane-diols were exclusively formed from dihydrotestosterone. Since high concentrations of 5 alpha-androstane-3 beta, 17 beta-diol are found in testicular tissue it is suggested that this steroid may be an index of seminiferous tubular function.

[Amino acid uptake in brain tumors using positron emission tomography as an indicator for evaluating metabolic activity and malignancy].

Diagnosis and post-therapeutic follow-up of tumour patients necessitates morphological and particularly functional imaging methods. For the latter approach positron emission tomography has proven a valid tool for the measurement of perfusion, of energy consumption parameters such as oxygen extraction, glucose metabolism and amino acid uptake. However, neither perfusion nor energy consumption parameters have yielded unambiguous information on the clinical status of various tumours in respect of their malignancy and their growth status. It is shown in this paper that amino acid uptake seems to be a valid measure for the functional activity of tumour tissue for a broad range of neoplasms. The uptake of 11C-L-Methionine was measured in 33 patients having various brain tumours, and was compared with 6 patients who had an infarction, and with 8 patients suffering from arachnoidal cysts. The amino acid uptake correlated well with the histological grading of the tumours and the clinical status of the patient. The uptake was well differentiated against metabolically inactive lesions. Parallel investigations on the uptake mechanisms of amino acids in an animal model have shown that transport phenomena regulate the uptake rather than protein synthesis rates. However, protein synthesis may nevertheless exercise a control function on the transport process.

[Quantitative determination of LP-X in the differential diagnosis and treatment of direct hyperbilirubinemia in infancy].

Quantitative measurements of serum concentrations of LP-X were performed in 45 newborn and infants. The changes of LP-X concentrations before and after a 2-3 weeks' course of cholestyramine therapy differentiated extrahepatic biliary atresia (n = 6) from other causes of neonatal liver disease with a sensitivity of 100%, a specificity of 78.6 to 84.6% and an efficiency of 81.3 to 86.7%. The efficiency was decreased in children with alcoholic stools (75-80%). Cholestyramine treatment of 3-7 days did not allow to diagnose all children with extrahepatic biliary atresia. However, the test was superior to a combination of single measurements of LP-X and GGT. The changes of LP-X concentrations in serum were influenced by the individual course of the disease but not by the synthetic function of the liver (as indicated by CHE activities) or by parenteral nutrition or bacterial infections. LP-X was a valuable parameter in the management of cholestyramine therapy in infants with liver diseases.

Breast augmentation using the expander mammary prosthesis.

The expandable mammary prosthesis that has been successfully used for breast reconstruction has been modified for breast augmentation procedures. A new microreservoir that can be buried subcutaneously for extended periods has been developed. The unique design of this implant allows its size to be adjusted postoperatively as many times as desired, thus eliminating the possibility of a second operation to replace an implant whose size is unsatisfactory. Accurate, aesthetically satisfactory results are possible because the volume adjustments are made after the swelling has decreased and the implant has settled into position. Almost all patients offered this opportunity requested postoperative volume adjustments. The results of 59 patients are reported. The surgery was performed over a 3-year period, with the average follow-up time being 6 months. The incidence of capsular contracture has been less than 10 percent. Leakage was a problem in the original implant design; however, since the introduction of the new valve, there has been no leakage to date.

The expandable mammary implant.

An expandable saline-gel implant having a detachable injection dome is described. The implant functions initially as a tissue expander. Once the correct size has been obtained, the injection dome is removed, leaving the implant in position.

Immediate breast reconstruction after mastectomy using a permanent tissue expander.

We describe a new breast implant that greatly facilitates immediate breast reconstruction after mastectomy. Through a detachable filling reservoir, the implant can be filled with saline by percutaneous injection; this allows the empty implant to be placed in position and filled only after viability of the flaps is assured. Because filling can be controlled, the fear of skin flap necrosis or wound breakdown is eliminated. Furthermore, the chest wall skin can be expanded, thus avoiding the use of distant flaps.

Mechanical problems with small-diameter enteral feeding tubes.

The use of nasogastric enteral feeding for nutritional support of hospitalized and homebound chronically ill individuals has been expanding during the past ten years. Nasogastric and nasoenteral alimentation is safer and more cost effective than total parenteral nutrition. Mechanical problems related to slowing of the formula and/or clogging of the tube have been reported but have received little attention in the research literature. This exploratory survey was designed to determine the extent of mechanical problems and the procedures used to care for enteral feeding tubes. Fifty-eight percent of 91 hospitals reported a significant occurrence of mechanical problems with small-diameter enteral feeding tubes, and only 31% had written procedures for care of the tubes. Implications for practice are discussed, and recommendations for further research in the area are proposed.

Morphology of normal and malignant germ cells.

By means of semithin section histology and electron microscopy the following male germ cells are described: gonocytes and prospermatogonia in the embryonic and fetal testes; spermatogonia and primary spermatocytes in the testis of the adult; abnormal spermatogonia and spermatocytes, which are not tumour cells. In contrast to these cells, four main tumour cell types which on the basis of morphological criteria can be said to be derived from germ cells, are classified and designated as Tc 1 to Tc 4. The various tissues accompanying the tumour cells are briefly described.

Determination of androgen, progestin and estrogen receptors with two different assays in renal cell carcinoma.

Twenty-one renal cell carcinomas were analyzed for the presence of androgen progestin and estrogen receptors by agar gel electrophoresis and a dextran-coated charcoal assay. An androgen receptor could be demonstrated in 3 tumors, a progestin or estrogen receptor was not detectable. It was therefore concluded that renal cell carcinoma is generally not a hormone-dependent tumor, and a hormonal treatment of metastatic renal cell carcinoma as first-line therapy does not seem to be justified.
