[Why must we manufacture vinyl chloride and polyvinyl chloride?].

The manufacture of PVC is necessary for supplying the demands of modern society, for maintaining the equilibrium in the chemical industry such as the manufacture of caustic soda solution-chlorine by means of rocksalt electrolysis, and for the safe disposal of chlorine for environmental reasons. The manufacture of vinyl chloride and dichloroethane is illustrated. The conventional processing of PVC into consumer goods, after treatment of rigid and plasticized PVC compounds or granulate, is demonstrated.

[Non-resecting surgery for gastroduodenal ulcer. I. Pathophysiological principles (author's transl)].

Selective proximal vagotomy with pyloroplasty is a method which permits ulcers to be operated on with less morphological and pathophysiological disturbances than is possible with any other method. After ingestion, a reactive yet reduced acid secretion is still possible through the gastrin stimulus. The retained vagal antral release of gastrin seems to be without importance to the denervated delomorphous cells. Side effects of this type of vagotomy are small. The disturbances of motility and evacuation which also occur here can easily be corrected by an additional pyloroplasty suitable in shape and function.

[Non-resecting surgery for gastroduodenal ulcer. II. Indication and technique (author's transl)].

The indication for the non-resecting combination method arises if the presence of genuine ulcer disease is confirmed by complete diagnostic measures (history, roentgenologic examination, endoscopy, secretion analysis and possibly psychic testing). An extended indication exists for erosive gastritis with hypersecretion, for hiatal hernia with reflux esophagitis (Berman's syndrome), for cardiospasm and prophylaxis of hemorrhage. The technique yields permanent curative results if a complete selective proximal vagotomy is combined with a pyloroplasty suitable in form and function. This is also true for duodenal ulcer. In 22% of cases of gastric ulcer, selective vagotomy with antrectomy is necessary.

[Non-resecting surgery for gastroduodenal ulcer. III. Clinical results (author's transl)].

During the last 12 years, 1489 vagotomies were performed at the Chirurgische Poliklinik of Munich University; 1339 of them were selective proximal vagotomies (SPV). The main indication was duodenal ulcer (n = 915) and gastric ulcer (n = 188), including emergency operations for extensive bleeding. Further indications were: sliding hiatal hernia, erosive gastritis and achalasia of the cardia. The SPA was combined in all cases with a pyloroplasty based on form and function. The results are shown in detail related to mortality (elective 0.5%), recurrency (1.6%) and functional results (good 88.2%, fair 7.2%, poor 4.6%). The combined operation of SPV with pyloroplasty is, in our opinion, an operative procedure which allows non resectioning surgical treatment of GDU without selection, i.e. based on form and function.

[Puncture techniques in emergency medicine (author's transl)].

In order to be able to carry out effective emergency medicine outside the hospital, knowledge of some of the important puncture techniques is essential. Sometimes injection and infusion is required under difficult conditions, and sometimes decompression punctures are the vitally decisive interventions. Among these are central venous access via the subclavian vein, intracardial injection, decompression puncture in cardiac tamponade, relief of tension pneumothorax, cannulation of the trachea and relief of mediastinal emphysema. These interventions are outlined according to indication, technique and complications.

[Ten years of emergency medical service in Munich. Experiences and results].

The emergency ambulances of the emergency medical service in Munich have been called out on 89,748 occasions. From the point of view of the specialties involved, internist emergencies were the most frequent with more than half the calls. One fifth of the emergencies were street accidents, occupational and domestic accidents. Genuine saving of life was possible in 1.3% of the cases. Vital functions were disturbed in 10% of all emergencies. With increasing popularity, the emergency ambulance is called to every urgent medical case, because it appears for every patient with unsurpassed reliability and speed. Herein lies the dangers of misuse and decreasing efficiency of an organization originally conceived only for victims of street accidents.

[Recurrent ulcer following vagotomy: completion of vagotomy or resection (author's transl)].

An adequate therapy must be directed towards the cause of the recurrent ulcer. This is incomplete vagotomy in most cases, more seldom a hypergastrinemia with stasis in the antrum after insufficient drainage, in hyperparathyroidism, in hyperplasia of the antral G cells or in gastrinoma. After confirmation of the diagnosis by endoscopy, a causal diagnosis must therefore be made which includes secretion analysis and determination of the gastrin profile (feeding test, glucagon provocation test, secretion or calcium infusion). Criteria for evaluation and clinical conclusiveness are shown in examples. The indication scheme, whether revagotomy alone, resection alone or the combination of the two corrective operations should be performed is determined according to these criteria. So far, 41 patients have been operated on with good results in accordance with this graduated indication.

Idiopathic pulmonary hemosiderosis: ultrastructural studies and responses to azathioprine.

Two boys are presented who fulfilled criteria for a diagnosis of idiopathic pulmonary hemosiderosis. A lung biopsy specimen from the first patient showed alveolar-capillary basement membrane abnormalities, together with abnormalities of capillary endothelial cells and hemosiderin-laden macrophages. A lung biopsy specimen from the second patient showed mainly capillary endothelial abnnormalities and interestitial fibrosis. Both patients had a noticeable improvement in symptoms and relative stabilization of their roentgenographic and pulmonary function abnormalities following azathioprine therapy.

The effect of transformation-defective avian oncornavirus mutants on tumor antigen expression.

The recent isolation of conditional (temperature sensitive) and nonconditional transformation-defective mutants of avian sarcoma virus strains has facilitated the investigation of the effect of virus transformation on the cell's phenotype, e.g., with respect to morphology, growth pattern, or cell surface antigenicity. Special emphasis was laid on elucidating the correlation between transformed phenotype and tumor antigen expression. All of the tested nontransforming deletion mutants and the majority of the temperature-sensitive mutants were unable to induce tumor antigens in phenotypically untransformed cells. However, 3 temperature-sensitive mutants were found which were able to support the expression of tumor specific surface antigens even at restrictive temperature, when cells otherwise exhibited a normal phenotype. The theoretical and practical implications of this association between normal phenotype and tumor antigen expression are discussed.

Localization of mannan at the surface of yeast protoplasts by scanning electron microscopy.

The beta(1-3)glucanase of Basidiomycete QM 806 was used to prepare Saccharomyces cerevisiae and Candida utilis protoplasts. Plasma membranes isolated from S. cerevisiae contained a small amount of mannose and traces of glucose and ribose. Randomly distributed alpha-mannan was detected by scanning electron microscopy at the surface of prefixed protoplasts using colloidal gold labelled with Concanavalin A as a marker. C. utilis protoplasts were also marked with anti-mannan antibodies. Again the distribution of mannan was random. This experiment indicated also that plasma membrane mannan has the same immunochemical determinants as cell wall mannan. It is hypothesized that mannan is mainly located in the outer layer of plasma membranes.

Biological and electron microscopic studies on the phenotypic mixing of the thermolabile mutant of vesicular stomatitis virus, tl-17, with avian RNA tumor viruses.

A thermolabile as well as thermosensitive mutant of vesicular stomatitis virus, VSV-tl-17, could be thermostabilized by phenotypic mixing with avian RNA tumor viruses (ATV). Biological, immunological and morphological studies revealed that this effect is due to a replacement of the thermolabile projections of the VSV by the avian tumor viral projections. The arrangement of projections of VSV and ATV on phenotypically mixed viria was studied by electron microscopy. A-type particles were detected in chick embryo cells which were doubly infected by ATV and VSV. Their intracytoplasmic appearance seemed to be the result of a disturbed maturation of ATV due to the relative deficiency of envelope proteins which were depleted by VSV maturation in the course of phenotypic mixing.

California arbovirus (La Crosse) infections. III. Epidemiology of California encephalitis in Minnesota.

Mosquitoes (eggs, larvae, and adults), small woodland animals, and residents of an area where California encephalitis is endemic were studied to elucidate the host-vector cycle of La Crosse virus. Elementary schoolchildren from surrounding communities and gray squirrels from another area were tested to compare the prevelence of serum antibodies to La Crosse virus in areas where the disease is endemic with the prevalence of these antibodies in areas where the disease is not endemic. From 1971 to 1974, eight isolations of La Crosse virus were made. Three of the isolates were from adult mosquitoes, one from Aedes triseriatus eggs, and four from A. triseriatus larvae. The isolation of virus from field-collected eggs and larvae confirms previous studies from Wisconsin that suggest that La Crosse virus overwinters in eggs of the mosquite A. triseriatus. In an area where California encephalitis is endemic, 10 of 19 small woodland animals (53%), which are the natural hosts of A. triseriatus, had hemagglutination-inhibiting and neutralizing antibodies to La Crosse virus. In contrast, none of 10 squirrels from an area where the disease is not endemic had such antibodies. Fourteen of 79 residents of this area (17.7%) had both types of antibody. Eleven of the 14 seropositive residents lived in one small sector of the community studied, an indication that foci of La Crosse virus activity may be very localized. Elementary schoolchildren from surrounding communitites had a significantly lower prevalence of hemagglutination-inhibiting antibody to La Crosse virus than did the residents of the area where California encephalitis was endemic.
