[The effect of arteriosclerosis on the wall elasticity of the human common carotid artery].

The arterial distensibility and the modulus of volume elasticity of more than 100 isolated human carotid arteries was measured and correlated with arteriosclerosis and aging. The loss of arterial distensibility progresses steadily with aging. Arteries with severe arteriosclerosis and arteries with minimal arteriosclerosis show almost similar distensibility. On the other side the differences between distensibility of arteries with moderate arteriosclerosis and arteries with minimal as well as severe arteriosclerosis, especially in the younger and middle ages, are significant.

Diminished synaptosomal dopamine (DA) release and DA autoreceptor supersensitivity in schizophrenia.

Post-mortem brain regions of schizophrenics were investigated in comparison to matched controls regarding synaptosomal K(+)-stimulated [3H]DA release and its modulation by DA autoreceptors. Brain specimens were cryopreserved in liquid nitrogen until release experiments; synaptosomes from brain pieces preincubated with dimethylsulfoxide have unchanged ability for [3H]DA release and its autoreceptor-mediated inhibition after cryo-preservation. Release data were individually corrected for post-mortem delay back to the zero-time state based on [3H]DA release alterations after in situ preservation of rat heads. In schizophrenia, the K(+)-stimulated [3H]DA release from superfused synaptosomes of the nucleus (n.) accumbens and n. caudatus was diminished significantly. Furthermore, functional supersensitivity of the modulatory DA autoreceptors could be demonstrated in these regions. The alterations demonstrated seem not to be due primarily to medication since in neuroleptic-free patients similar changes were shown.

Inhibition of acetylcholinesterase activity in human brain tissue and erythrocytes by galanthamine, physostigmine and tacrine.

Galanthamine, physostigmine and 9-amino-1,2,3,4-tetrahydroacridine (tacrine) were evaluated as inhibitors of human acetylcholinesterase activity from samples of postmortem human brain, fresh brain cortex biopsies and human erythrocytes. Acetylcholinesterase activity was most effectively inhibited in all tissues by physostigmine, followed by tacrine and galanthamine. The respective inhibitor concentrations exerting a half maximal effect (IC50) on acetylcholinesterase in postmortem human brain frontal cortex were 14 nmol/l, 1.0 mumol/l and 3.2 mumol/l versus 15 nmol/l, 1.1 mumol/l and 2.8 mumol/l in the hippocampus region. In addition, the inhibition of acetylcholinesterase by galanthamine was similar in postmortem brain and brain cortical biopsies from patients submitted to brain-tumour removal, indicating that postmortem changes up to 28 h after death probably did not influence the measurement of acetylcholinesterase inhibition. While physostigmine and tacrine acted equally on acetylcholinesterase from different sources, galanthamine was 10-fold less potent in inhibiting the enzyme activity from human brain that from human erythrocytes. Comparison with issues from mice revealed that galanthamine was selectively more potent in suppressing acetylcholinesterase in human erythrocytes. The results are discussed in the light of pharmacokinetic data, and conclusions are drawn for further clinical studies.

[Importance of immunohistochemistry for neuro-oncology. II. Localization of factor VIII-associated proteins and glial fibrillary acidic protein (GFAP) in angiomatous and sarcomatous tumors].

Immunhistochemical methods utilizing specific antibodies against Factor VIII-related antigen and glial fibrillary acidic protein were employed in studies of 48 intracranial and intraspinal tumors. Factor VIII-related antigen occurred only in endothelial cells of the vascular wall and is therefore not of importance for the differential diagnosis of CNS tumors. Isolated Factor VIII positive cells in the stroma of hemangioblastomas turned out to be mast cells which may also normally contain this substance. The GFAP positive cells in hemangioblastomas are believed to all be of astrocytic lineage. Many of the multinuclear giant cells present in monstrocellular sarcomas contained GFAP but were Factor VIII negative. Genuine fibroxanthoma of the meninges can apparently exist next to pleomorphic xanthoastrocytomas. As demonstrated by one of our cases, the demonstration of GFAP alone can successfully distinguish between them.

[Myelomalacia caused by embolism of the substantia gelatinosa in the vertebral column and spinal cord arteries].

A 56 year old woman developed a transverse lumbar paraplegia within two hours following a jump from an 80 centimeter high window. The patient died 13 days later from a pulmonary embolus. At autopsy an intervertebral disc tissue embolus was found in the arteries to the 12th thoracic vertebral body and the spinal cord. The embolus originated in a Schmorl nodule in the 12th thoracic vertebral body. The findings and the clinical course suggest that dislodged nucleus pulposus tissue was expressed into the vascular system over some time as a result of tears in small vessels. The implications for treatment of patients with similar spinal cord symptoms are considered.

[Significance of immunohistochemistry in neuro-oncology. I. Demonstration of glial fibrillary acid protein (GFAP) in extracranial metastases from primary brain tumors].

8 cases were studied to determine whether immunohistochemical investigation with anti-GFAP could contribute to confirming a primary brain tumor origin for an extracranial metastasis. The materials studied consisted of 3 glioblastomas, 3 anaplastic astrocytomas, and 2 medulloblastomas, along with their extracranial metastases. GFAP could be immunohistochemically demonstrated in all 6 primary glial tumors as well as in the metastases of the 3 astrocytomas and of 2 glioblastomas. The medulloblastomas and their metastases were immunohistochemically GFAP-negative. GFAP is thus a marker for extracranial metastases of astrocytomas and glioblastomas. A negative result however does not exclude the possibility that a metastasis is of glial origin as shown by the GFAP-negative metastasis of the one glioblastoma.

[Significance of basic concepts in depth psychology for psychosomatics].

The concept of "the unconscious" is considered the central concept of depth psychology. While the unconscious in Freud's conception consists essentially of repressed material and through conversion can result in psychosomatic illnesses, C. G. Jung saw in the "collective unconscious", which he discovered, the source of all psychic and spiritual development. Mediation between the collective unconscious and the conscious is effected by means of the "archetypes", whose function can be compared with the instincts. Archetypes are manifested through symbols, whose assimilation by consciousness is a prerequisite for psychic and physical health. Consideration of the archetype of the physician suggests that the grounding of medical science in depth psychology would modify not only our understanding of illnesses, but also the physician's understanding of himself.

[Age dependence of metastases. A study of more than 5000 cases of death from cancer].

The frequency of haematogenous and lymphogenous metastases from 5,708 cancers of various types, confirmed on autopsies, decreased nearly continuously beyond the age of 60. Cases without metastases are found three times more in senility than in younger people. The decrease of tumor metastasis in elderly people is nearly the same for all kinds of cancers. In contrast to the haematogenous and lymphogenous metastases, a weak reduction of peritoneal metastasis indicates that the sclerosing of capillaries is an essential factor for the reduction of remote metastases in elderly people.

[Pleural mesothelioma, asbestosis and silicosis following long-term exposure to talc dust].

A case report with pleural mesothelioma, asbestosis and silicosis after long time exposure to talc dust has been given. Pathological findings, professional exposure, diagnostic aspects, and medical assessment of disablement are described. The relationship between the diseases of lungs and the professional exposure to talc dust as well as the possibility of cause-distinction by co-working of pathologist and industrial hygienist are discussed.

[The incidence of metastatic carcinomas in meningiomas. A report of 4 cases].

88 meningiomas were found among 8,371 autopsies. 29 meningiomas were associated with extraneural primary malignancies. In 4 cases a carcinoma metastasized into a meningioma. This unexpected high frequency of carcinomas metastasizing into meningiomas could be an accidental event. The preferential occurrence of meningiomas in females and the disposition of breast cancer to metastasize to the vertebrae and meninges might be responsible for the high fraction of breast cancers among the malignant tumors with metastases in meningiomas.

[Distribution of lung cancer metastases. II. Effect of tumor histology].

The various histological types of 1,283 lung cancers show remarkable differences in sex distribution, in mean age at the time of death, in the localization of tumors and duration of cancer disease as well as in their metastatic behaviour. The proportion of the various histological types of lung cancer in ten organs with blood borne metastases has been analysed. Metastases of small cell carcinomas most frequently occur in pancreas, liver, thyroid gland, bone and adrenal, whereas squamous cell carcinomas are found in kidney and heart. Anepidermic squamous cell carcinomas are most frequently seen in brain and large cell carcinomas in the spleen. As already mentioned in the preceding paper, combinations of metastatic affected organs can be seen with special frequencies in all histological types of lung cancer. However, small cell carcinomas provide the most evident findings.

[Karyometric studies on a case of recurrent and metastasizing meningioma].

An unusual case of a recurrent and metastasizing meningioma in a 59-year-old female patient is reported. 11 years before death the woman had undergone craniotomy. The necropsy revealed a recurrent intracranial tumor and a solitary metastasis in the adrenal gland. Karyometric investigations were carried out on Feulgen-stained microscopic slides using an image processing system described previously. The frequency distributions and correlation diagrams of parameters for nuclear size and shape did not show any significant difference between the intracranial tumor and adrenal metastasis. There is no karyometric or visual evidence for dedifferentiation in the metastasis. The findings emphasize the role of craniotomy in extracranial dissemination of meningiomas.

[Distribution of lung cancer metastases. I. Combination and frequency of organ metastases].

The distribution of metastatic tumors of 935 lung cancers with blood borne metastases has been analysed. The frequency of affected organs decreased in the following sequence: liver, bone, adrenal, brain, lung, kidney, thyroid gland, pancreas, spleen and heart. In cases with only one metastatic affected organ the frequency of brain and lung metastases is greater than the frequency of liver, bone and adrenal metastases. Especially in cases with two different affected organs the pair combination of liver and bone metastases as well as of brain and adrenal metastases is evident, however, combined brain and bone metastases and brain and liver metastases rarely occur. The distinct pattern of metastatic distribution in the body supports the evidence of the "soil" hypothesis. Seldom affected organs rarely shows a strong metastatic involvement and go hand in hand with many other affected organs. In autopsies the duration of cancer disease is much longer in cases without metastasis or small metastatic involvement than in cases with many affected organs. Therefore, the generalization of metastatic spread mainly depends on the malignancy of tumors and to a less extent on the duration of cancer disease.
