Verapamil and the synthesis of prostacyclin in human veins.

The synthesis of the vasodilator prostacyclin (PGI2) depends on the intracellular calcium concentration. For this reason the purpose of this study was to document the effects of verapamil (1), a slow calcium channel blocking agent on venous tissue. Human varicose veins were studied which were removed by a stripping operation. The veins were investigated ex vivo and in vitro. For ex vivo studies 11 patients were given 0.01 g 1 i.v. during the operation. In vitro experiments were carried out by an addition of 22 mumol/l 1 to the incubate. The synthesis of PGI2 was studied according to the inhibition of the ADP induced platelet aggregation without and with addition of substrate (prostaglandin endoperoxide H2). 1 did not induce changes in the synthesis of PGI2, neither after administration to patients nor after addition to the incubation fluid compared to control vessels without addition of 1.

[In vitro sensitivity of Chlamydiae to antibiotics].

The in vitro activity of 11 antibiotics against 16 Chlamydia trachomatis strains isolated from genital tract and 2 Chlamydia psittaci strains isolated from pulmonary tract. The CMI determination (the lowest dilution of the drug which inhibited the inclusion development when untreated control give 10(3) IFC) was assessed by growth in cycloheximide treated McCoy cells-antibiotics were added after incubation for 2 hours at 37 degrees C. The CMI values for Chlamydia trachomatis and Chlamydia psittaci are similar. The activity of antibiotics by comparison between CMI90 and serial levels. The CMI90 (mg/l) were: doxycycline = 0.2, minocycline = 0.2, roxythromycine = 0.12, érythromycine = 0.5, spiramycine = 4, rifampicine = 0.016, ofloxacine = 4, ciprofloxacine = 4, pefloxacine = 8, lomefloxacine = 8, fleroxacine = 8. For Chlamydia trachomatis species all the strains have the same sensibility, no resistance was detected.

[Tensile strength of plate osteosynthesis. Comparative biomechanical study of the original autocompression plate and a functional modification using the ZESPOL principle].

In order to overcome the problems of stress protection and pressure atrophy of bone, which are associated with the traditional plating technique, in Poland a modified type of plating, ZESPOL has been, developed. In this procedure, the plate is not screwed onto the surface of the bone, but is fixed above the bone by special platform screw bolts in a comblike construction. This paper describes an experimental biomechanical investigation, using cadaver tibia, which compares the stability of the ZESPOL principle with that of the traditionally applied autocompression plate (ACP). In order to make the system biomechanically comparable, instead of the original ZESPOL plates, which are different in dimension, an ACP was used that was specially adapted to fit the ZESPOL criteria. Compared to the ACP, the bending stability of the ZESPOL osteosynthesis was found to be significantly less. When the same stress was applied, the total bending capacity of the ZESPOL system was found to be between 9% and 1348% higher, depending on the position of the plate and the direction of the force applied. In addition, the gap of the osteotomy site was between 6% and 923% greater with the ZESPOL system. Deformation of the bone adjacent to the osteotomy site was more closely approximate to intact bone with ACP than with ZESPOL plate. Our evaluations show that the ZESPOL system has less bending stability than ACP, without having the advantage of a more physiological bone-bending curve. However, the stability of the new system can be regarded as being overall sufficient.

Biomechanical Phenomena

[Bilateral primary giant cell tumor (osteoclastoma) of the ribs].

The primary giant-cell tumour is a semimalignant bone tumour which occurs primarily on the epiphysis of the long tubular bones and the methaphysis. The present case report describes the bilateral occurrence of this tumour in the region of the osseous thorax. The paper also deals with diagnostic, therapeutical and prognostic problems.

[Nutritional status in stomach cancer--a criterion for evaluating operability?].

The nutritional status of 41 male patients with carcinoma of the stomach was recorded, right after admission to hospital, and was related to tumour-specific operability. The nutritional condition of most of the patients was moderately impaired. Significant differences between operable and non-operable patients were recorded from the following parameters: upper-arm circumference, haemoglobin concentration, serum albumin, pseudocholinesterase, intradermal testing, E-rosette count. The findings thus obtained provided some limited information on operability.

[Perforated gastric and duodenal ulcer (635 cases) (author's transl)].

Perforation could be observed in 10.14% of all patients with gastroduodenal ulcer admitted to our clinic from 1946 to 1976. 23.16% of these patients were older than 61 years, whereas 15.9% were younger than 30 years. 91.18% had typical symptoms of perforation, symptomless were 6.93%. 1.81% arrived moribund in the hospital. During the last 30 years as a rule the perforation was only sutured, primary resection was being performed only in rare cases. Nowadays, in cases of perforated duodenal ulcer we add vagotomy plus pyloroplasty to the suture. The total mortality rate came up to 22%, with a significant decrease from 1946 to 1975.

Adolescent

[Selective proximal vagotomy (SVP); technical details (author's transl)].

At present the technique of selective proximal vagotomy (SVP) seems to be standardized. Nevertheless, the surgical procedure is practiced by many authors in a different way concerning the proximal and distal borderline of denervation. In our opinion pylorplasty should only be performed in those patients suffering from retention or stenoses.

Duodenal Ulcer

[Absorption of amino acids in patients after partial gastrectomy (B II) and truncal vagotomy with pyloroplasty or antrectomy (author's transl)].

After surgery for gastric or duodenal ulcer the resorption of amino acids is changed due to alteration of morphology and function of stomach, bile system, pancreas and small bowel. We used the stable isotope labelled amino acid 15N-glycine in patients after partial gastrectomy (B II), truncal vagotomy with pylorplasty as well as antrectomy and in healthy persons. It could be shown that in contrast to the control group the absorption of the amino acid in the surgical group took place more rapid and more intensive.

[The effect of truncular subdiaphragmatic vagotomy and pyloroplasty on the nitrogen balance in bionomal rat gastric surgery using the isotope labeled amino acid 15N-glycine].

By use of stable isotope labeled amin acid 15N-glycine the effects of truncular subdiaphragmatic vagotomy and of pyloroplasty, which are comprised in the method of bionomic gastric operations, were studied separately. The assessment of nitrogen equilibrium in rats after only truncular vagotomy, only pyloroplasty, truncular vagotomy combined with pyloroplasty, and without surgery serving as controls yielded results as following: After truncular vagotomy the 15N equilibrium was worse than in the control animals or after pyloroplasty only. The cause of that is an enhanced nitrogen loss in urine. After truncular vagotomy with pyloroplasty, though, 15N excretion in feces was significantly higher than after only pyloroplasty. It may be concluded from these results that truncular vagotomy is primarily responsible for the disturbances in protein metabolism. Pyloroplasty in combination with truncular subdiaphragmatic vagotomy also causes a higher nitrogen excretion in the feces indeed.
