[Anterolateral approach to giant cell tumor in T4 vertebral body].

A case of giant cell tumor involving the Th4 vertebral body is reported. A 27-year-old man, who had a 2 month history of back pain and paraparesis, was admitted to our department in August, 1986. On the first admission, he manifested severe paraparesis and sensory disturbance below the Th5 dermatome level. Neuroradiological examination demonstrated an epidural tumor involving the Th4 vertebral body and compressing the spinal cord. Emergency laminectomy for decompression and biopsy was performed. The surgical specimen was a giant-cell tumor. 6 weeks after the first operation, total removal of the tumor and anterior fusion of the Th 3-5 with iliac bone graft was performed through the anterolateral transthoracic approach. Postoperative course was uneventful. A curative resection and immediate stabilization was thus able to be undertaken through the anterolateral transthoracic approach, when the epidural tumor was confined to the thoracic vertebral body.

[A case of traumatic syringomyelia which disappeared after anterior decompression: a suggestion for surgical treatment of syringomyelia].

A 37-year-old man had received cervical spinal-cord injury 20 years previously. Recently, he complained of progressive spastic gait and was shown to have fracture dislocation of the C5 vertebral body which compressed the spinal cord posteriorly. He also had a syrinx from the C5 to C2 vertebral level. Postoperatively, this syrinx disappeared after anterior decompression of the C4 and C5 vertebral bodies. Such a case as this has never been reported, and this case may give a suggestion for surgical treatment of syringomyelia, because it is very natural that the most reasonable treatment of any disease is removal of the cause.

[Complete occlusion of right middle cerebral artery by radiation therapy after removal of pituitary adenoma: case report].

Radiation angiopathy is a well-known complication accompanying irradiation. But the majority of effected vessels are small vessels. Cases of intracranial major arteries being effected are not so numerous. We report the case of a 47-year-old female patient. The removal of pituitary adenoma was carried out on her in 1982. After that, 4500rad irradiation was performed. It was about six years after irradiation that the first angiography was made. Right MCA occlusion, multiple stenosis of both ACAs and Moya-Moya like vessels were discovered. On admission light paresis on her left lower extremity was recognized and hyperlipidemia was noted. It was interesting that the multiple stenosis was found in an area outside the irradiation field. MCA-STA anastomosis was performed and the prognosis was good. The authors described this case and reviewed the pertinent literature.

[The quantitative analysis of electroencephalography in primary hypothyroidism].

The EEG topography was recorded in the patients with primary hypothyroidism, and the relationship between equivalent potential and blood thyroid hormonal levels were investigated. To determine the relationship between the % equivalent potential fraction (%EPF) and the levels of blood free T3, free T4, and TSH in each patient, regression lines were drawn in respective EEG spectral bands. The %EPF of each patient showed a tendency to have a negative correlation with the values of blood free T3 and free T4 in slow frequency spectral bands such as delta, theta, and alpha 1 waves, whereas a positive correlation in fast frequency bands such as alpha 2, beta 1, and beta 2 waves. In particular %EPF had a significant relationship with blood free T3 levels in alpha 1 and beta 2 spectral bands. These results suggest that the thyroid hormones may have influence on the mechanisms of EEG rhythm formation in primary pypothyroidism.

[Right ventricular myxoma complicated by paroxysmal atrial tachycardia: a surgical case report].

A rare case of right ventricular myxoma complicated by paroxysmal atrial tachycardia (PAT) is presented. A 13-year-old boy was referred to our hospital because of the recurrent episodes of PAT. Every available medical treatment was tried to reverse the PAT, during which an abnormal mass in the right ventricle was found by echocardiography. Subsequently magnetic resonance imaging computed tomography demonstrated that the tumor developed from the anterior wall of right ventricle with a pedicle. A 10 g of reddish myxomatous mass originating from the base of anterior papillary muscle was resected including the stalk during total cardiopulmonary bypass. The transient paroxysmal atrial tachycardia developed shortly after operation, however, tachycardic episode has never recurred after that. It is noteworthy that intracardiac evaluation is necessary when there is longstanding and inveterate tachyarrhythmia in otherwise healthy children.

[Metastatic Grawitz's tumor to the cauda equina: case report].

A case of Grawitz's tumor metastasizing to the cauda equina is presented. A 51-year-old male was hospitalized due to severe low back pain radiating to the left lower extremity. Neurological examination showed only hyporeflexia of the left patella reflex and positive Lasegue's sign. MRI showed intradural mass at the L4 level. Preoperatively, we diagnosed a cauda equina tumor. A laminectomy of both L3 and L4 was performed, and total removal of the cauda equina tumor was performed. Microscopically, the tumor cells were large, the appearance of the cytoplasm ranging from optically clear with sharply outlined boundaries, to deeply granular, with many transitional forms. These histological findings were typical findings of Grawitz's tumor, and were the same as those of this patient's renal tumor. Finally, we diagnosed Grawitz's tumor metastasizing to the cauda equina. Metastatic cauda equina tumor from outside the central nervous system is very rare and only 7 cases have been reported. This case is the first one of Grawitz's tumor spreading to the cauda equina.

[Multiple myeloma superimposed on adult T cell lymphoma].

A 77 year-old male was admitted to the hospital because of lumbago and M-proteinemia. IgA (kappa) monoclonal protein (8,100 mg/dl) was demonstrated in serum, and Bence Jones protein (kappa) in urine samples. The bone marrow examination showed an increased number of pathological plasma cells (34. 5%). Multiple osteolytic lesions were evident on X-ray films. A diagnosis of multiple myeloma (MM) was made. He had exudative erythematous skin lesions on his back. His serum was positive for antibody to ATLA. A biopsy specimen from the skin lesions showed Pautrier's micro-abscess which were filled with Leu 3a positive T lymphocytes. 159 base pairs of human T cell leukemia virus I (HTLV-I)/pX position was identified from a cutaneous sample utilizing the polymerase chain reaction method. Thus, a diagnosis of MM superimposed on adult T cell lymphoma was made. An extensive search failed to find any cases complicated with these two diseases except a report by Tagawa et al. concerning a patient with ATL who developed IgA (kappa) MM during a five year follow up. Therefore, this is the first reported case of MM superimposed on ATL.

[A case of bilateral coronary artery--pulmonary artery fistulas associated with mitral valve stenosis].

A case was reported of bilateral coronary artery-pulmonary artery fistulas associated with mitral valve stenosis. A thirty seven year old female was admitted with the complaint of exertional dyspnea and palpitation, which had lasted for the 3 years previous to her admission to our hospital. Electrocardiogram showed sinus bradycardia and no ST-T changes, but exercise ECG showed significant ST depression after 6 min of exercise. The DDR (9 mm/sec) and mitral valve area (0.61 cm2) were shown by UCG examination to have decreased, and the pressure at half time (360 msec) was shown by Doppler UCG to be prolonged. On cardiac catheterization, coronary arteriography showed fistula from RCA to PA, and fistula from LCA to PA, but no occlusive lesions were demonstrated. P/S blood flow ratio was 1.09, and O2 saturation was stepped up 3% in PA. She was operated on and given open mitral commissurotomy and closure of the fistula opening, via the PA. After surgical repair, no abnormality was found by exercise ECG, and no fistulas were shown on coronary arteriography.

[Pathological studies of multiple submucosal gastric gland lesions in thirty-eight cases].

We have examined 38 stomachs with the adenocarcinoma and multiple submucosal gastric gland (SG) lesions. Almost the SG lesions consisted of the foveolar epithelium, which stained negative with alcian blue whereas the ordinary glands stained positive with con-A (III), or with both types of epithelium, and they had no atypicality. Only 14 out of 2,188 SG lesions (0.63%) showed atypical proliferation. Three hundred and forty-nine out of 1,535 SG that were examined by lysozyme staining were strongly lysozyme positive. Almost all the propria mucosa in the areas of the minute SG showed erosive lesions with an atrophy of ordinary glands, glandular erosion, and a regenerative epithelium. Eleven stomachs (11/38 cases, 29.0%) had multiple adenocarcinomas.

[MR imaging of cervical disc disease: value GRASS imaging].

Since magnetic resonance imaging (MRI) technology has been greatly improved, MRI for cervical disc disease has become widely used in many facilities. Among non-invasive procedures, MRI is regarded as one of the most useful ones. Conventional myelography, CT myelography, and MRI were performed on 10 patients with cervical disc disease. The authors discussed the correlation between conventional myelography, CT myelography, and MRI as regards their ability to determine the localization and the laterality of disc protrusion and osteophyte. In our MRI study, we use both short-echo (SE) images and GRASS images. The parameters of our GRASS included 5mm-thick sections. TR = 200, TE = 20, and flip angles of 10 degrees. This pulse sequence generates images with high signal cerebrospinal fluid (CSF), resulting in a high-contrast CSF-spinal cord, osteophyte, and disc protrusion. As yet, although it easily shows the localization and laterality of the disc and/or osteophyte, image quality of the GRASS is not yet sufficient to allow us to evaluate detailed deformity of the spinal cord and nerve root. The authors stress the usefulness of this GRASS image for the evaluation of suspected cervical disc disease.

[Diagnosis of hemodynamic compromise in patients with chronic cerebral ischemia; the detection of impaired vasodilatory capacity with 133Xe SPECT and acetazolamide (Diamox) test].

To evaluate the efficacy of 133Xe SPECT and Diamox test, for selecting patients with hemodynamic compromise, tests using 133Xe inhalation method and single photon emission computed tomography (SPECT) with acetazolamide (Diamox) were performed in twenty-three patients with chronic cerebral ischemia, before and after extracranial-intracranial bypass surgery or carotid-endarterectomy. All patients complained of TIA, RIND, or minor completed stroke, and cerebral angiography of all patients demonstrated severe stenosis or occlusion in the ipsilateral internal carotid artery or middle cerebral artery. Cerebral blood flow (CBF) was also measured 15 minutes after intravenous administration of 10-12 mg/kg Diamox, which is known as a cerebral vasodilatory agent (Diamox test). Our results revealed that all patients could be divided into four types according to the findings of their resting rCBF and Diamox reactivity. The patients who had normal resting rCBF and normal Diamox reactivity (type 1) were considered to have well-developed collateral circulation and normal cerebral perfusion pressure (CPP) in spite of severe occlusive lesions in the carotid system. Moderate vasodilatation due to reduced CPP was considered to occur in patients who had normal resting rCBF and decreased Diamox reactivity (type 2). The resting rCBF remained unchanged, but Diamox reactivity improved to the normal range after surgery in the patients of type 2 (p less than 0.01), which indicated the improvement of CPP and the resolution of the autoregulatory vasodilatation. Maximum vasodilatation or dysautoregulation was considered to occur due to the inadequate collateral flow and the severely reduced CPP in patients whose findings revealed decrease in the resting rCBF and impaired Diamox reactivity (type 3).(ABSTRACT TRUNCATED AT 250 WORDS)

[Clinical and neuroradiological features of syringomyelia associated with Chiari malformation].

The clinical presentation and radiological features were analyzed in 30 cases of syringomyelia associated with Chiari malformation. None of the patients had spinal dysraphism. The age on admission ranged from 6 to 59 years with a mean of 27 years. Syringomyelia was diagnosed by CT myelography and or MRI from 1982 to 1988. The initial symptoms were skeletal abnormality (43%) such as scoliosis (12 cases) or pes cavus (one case), unilateral pain or numbness (40%) and unilateral motor weakness (17%). Frequently seen signs on admission were sensory deficit (100%), scoliosis (57%), muscle weakness (57%), muscle atrophy (37%) and lower cranial nerve palsy (40%). The neurological findings were asymmetrical in all patients. The characteristic neurological findings in the cases presenting under 20 years of age were unilateral sensory and motor deficits (61%) with decreased or absent deep tendon reflex on the same side. The localization of the syrinx in axial section varied according to the level even in the same case. In 15 cases with unilateral sensory disturbance or unilateral sensory and motor deficit, the syrinx was located in the region corresponding to the posterolateral portion on the same side as that of sensory disturbance in the cervical or thoracic level. On the other hand, in 15 cases with bilateral sensory and motor deficit, the syrinx was located in the central portion and extended into the posterolateral portion of the more affected side.(ABSTRACT TRUNCATED AT 250 WORDS)

[The MR evaluation of normal children and disorders of neuronal migration and myelination].

We often experience the case that is difficult to differentiate normal from abnormal myelinating process by CT scan, because the diagnosis of myelinating maturation of the infant during developmental process is confusing. We can understand maturation of myelination adding to the advancement of diagnostic resolution by MR imager. So we obtain more informations about the abnormality of myelination and development of the brain by MR imaging than CT scan. This study demonstrates the ability of MR imaging to show progression of myelination in 10 infants and children. We also reveals the patients with disorders of neuronal migration or myelination during developing process. MR scanning could be obtained by sedation using such as triclofos or chloral hydrate for infant and younger children. It was necessary to use diazepam or pentazocine intravenously for elder children with mental retardation or restlessness. MR scans were performed with Siemens MAGNETOM 1.5 Tesla. T 1-weighted spin echo image (TR 600/TE 15) and T 2-weighted image (TR 3000/TE 90) were obtained for all cases. The ten normal infants and children (7 boys, 3 girls) were aged from 1 month to 4 years. Maturation of white matter generally proceeded from posterior limb of internal capsule to middle cerebellar peduncle, cerebellar white matter, corpus callosum, anterior limb of internal capsule, occipital white matter, centrum semiovale, finally frontal white matter. Myelination was matured during the first two years by T 2-weighted images. Changes caused by brain myelination were seen earlier on T 1-weighted images (7 months after birth) than on T 2-weighted images (one year and 9 months after birth).(ABSTRACT TRUNCATED AT 250 WORDS)

[Diagnosis of hemodynamic compromise in patients with chronic cerebral ischemia; measurement of cerebral blood volume (CBV) with 99mTc-RBC SPECT].

To evaluate the efficacy of tests for selecting patients with hemodynamic compromise, measurement of cerebral blood volume (CBV) with 99mTc-RBC single photon emission computed tomography (SPECT) was performed in thirteen patients with occlusive cerebrovascular disease, and was compared with results obtained by 133Xe SPECT and acetazolamide (Diamox) test. All patients in our study suffered TIA, RIND, or minor complete stroke. Cerebral angiography demonstrated severe stenosis or occlusion in the ipsilateral internal carotid artery or middle cerebral artery, although plain CT scan or MRI revealed no or, if any, only localized infarcted lesions. Regional cerebral blood volume (rCBV) was measured with 99mTc-RBC SPECT and regional cerebral blood flow (rCBF) was measured with 133Xe SPECT before and after intravenous injection of 10 - 12 mg/kg acetazolamide (Diamox). Our results suggest that the ipsilateral rCBV/rCBF (mean transit time) is a more sensitive index of the cerebral perfusion reserve than the use of only rCBV or rCBF of the ipsilateral hemisphere. Also, the ipsilateral rCBV/rCBF is significantly correlated (r = -0.72) with the Diamox reactivity of rCBF, which is considered to represent the cerebral vasodilatory capacity in patients with chronic cerebral ischemia. Postoperative SPECT study revealed remarkable improvement of ipsilateral rCBV/rCBF and Diamox reactivity in four patients who underwent EC/IC bypass surgery to improve the hemodynamic compromise. In conclusion, our results suggest that the measurement of rCBV/rCBF with 133Xe SPECT and 99mTc-RBC SPECT is useful for detecting the hemodynamic compromise in patients with occlusive cerebrovascular disease.

[A case of primary coronary artery dissection].

Primary coronary artery dissection occurring as a spontaneous event and not associated with trauma due to catheter manipulation is rare. We recently experienced (a case of) a 52-year-old man with primary artery dissection. He was admitted to our hospital with severe chest pain as his chief complaint on September 6, 1988. Electrocardiography and laboratory data showed acute inferior myocardial infarction. He was treated with medication and underwent coronary angiograms on October 3, 1988. Right coronary angiogram revealed an intimal flap and false lumen. But right coronary angiograms 6 months after the onset of myocardial infarction revealed progression of stenosis but no intimal flap, and coronary spasm was not evoked after acetylcholine administration. Primary coronary artery dissection has been reported since Pretty's first description of it in 1931. The majority of earlier cases were diagnosed at autopsy, but recently reports of survivors have been increasing due to the progress in and popularization of coronary angiographic technics. This case is the 46th case of primary coronary dissection found by coronary angiography. So it is not extremely rare. In our case the involved artery was the right coronary artery. In survivors, right coronary artery dissection is more frequent than left, because the area supplied by the right coronary artery is smaller than the area supplied by the left one. Although in our case coronary artery stenosis progressed, after a long term resolution of dissection may occur. There are a few cases in which resolution of dissection occurred naturally.(ABSTRACT TRUNCATED AT 250 WORDS)

[Phosphorus-31 MR spectroscopy of brain tumors].

Eleven normal volunteers and 17 subjects with brain tumors were studied by phosphorus-31 Magnetic Resonance (MR) spectroscopy. Measurements were performed by FROGS (Fast Rotating Gradient Spectroscopy) saturating nearly skin or muscle by Volume of Interest (VOI). This measurement took about 30 minutes for each case, including the imaging procedure, shimming and the measuring of spectroscopy. In phosphorus-31 MR spectroscopy, pH of the subjects with brain tumors showed a statistically significant elevation compared with that of the volunteers. In the subjects with benign tumors, only the elevation of pH was significant compared with that in volunteers. There was no other difference. This result suggests that benign tumors have an almost normal metabolic mechanism. Malignant brain tumors showed a decrease of PCr and an increase of Pi and PME. The increase of Pi indicates the increase of energy consumption. The increase of PME is connected with the acceleration of cell proliferation, there is a difference between malignant tumors and benign tumors. Phosphorus-31 spectroscopy has the potential to investigate the metabolism in vivo.

[A case of intramedullary spinal cord metastasis from adenocarcinoma of corpus uteri].

The authors present a case of a woman with intramedullary spinal cord metastasis from adenocarcinoma of the corpus uteri. Such a case was not able to be found in the literature. A 54 year-old woman complained of right upper extremity weakness and headache. CT scan showed abnormality, and removal of a brain tumor was performed. After that, total hysterectomy, retroperitoneal lymphadenectomy and chemotherapy was carried out. Three months later, she suddenly experienced bilateral weakness in her lower extremities. After a few days she was unable to walk. Gd-DTPA enhanced MRI showed an abnormal mass at the upper part of the conus medullaris. Total removal of the intramedullary tumor was performed and adenocarcinoma was diagnosed histopathologically.

Insulin-like growth factor I (IGF-I) production and the presence of IGF-I receptors in rat medullary thyroid carcinoma cell line 6-23 (clone 6).

To clarify whether insulin-like growth factor I (IGF-I) is an autocrine growth factor of rat medullary thyroid carcinoma (MTC) cell line, 6-23 (clone 6), IGF-I binding to MTC cell membranes, IGF-I levels in the conditioned culture medium of MTC cells and the effects of IGF-I on methyl-[3H]thymidine incorporation to MTC cells were examined. Scatchard analysis of saturation binding studies revealed the association constant and the maximal binding capacity were 1.0 x 10(9) M-1 and 199 fmol/mg of membrane protein, respectively. The binding of [125I]IGF-I to MTC cell membranes was inhibited by unlabeled IGF-I, IGF-II and insulin; the relative potencies were IGF-I greater than IGF-II much greater than insulin, suggesting the presence of type I IGF receptors in MTC cells. IGF-I levels in the conditioned culture medium of MTC cells were 120 +/- 3 pM (mean + SE). IGF-I (10(-10) to 10(-8) M) dose-dependently stimulated methyl-[3H]thymidine incorporation to MTC cells. These findings suggest a possible role of IGF-I as an autocrine growth factor for MTC cells.
