[Value assessment of the MIRA (multichannel infant reflex audiometry) hearing evaluation with and without biofeedback. 3. Diagnosis from curves or behavioral observation?].

Basing on the results of behavioural audiometry, MIRA (Multichannel Infant Reflex Audiometry) with and without biofeedback as well as in several cases also from the ABR, the authors demonstrate the advantages of the combination of curve analysis with behavioural observation compared with isolated looking at physiological curves or at the behaviour. The results were obtained from 100 babies in their first year of life accepted in sequence for testing their hearing; nearly one half of them had considerable hearing risks, most of them having been subjected to several different hearing tests.

Indications for combined high frequency ventilation in clinical use.

A retrospective study of 20 surgical intensive care unit patients is reported. They were ventilated with continuous positive pressure ventilation (CPPV) and then switched to combined high frequency ventilation (CHFV). To find why there were variable responses to CHFV, the data of 20 patients were retrospectively evaluated with respect to respiratory index (RI), compliance (C) and extravascular lung water (EVLW). The results suggest that the indication line for CHFV was a C value greater than 45 ml/cmH2O and an EVLW value greater than 15 ml/kg BW.

Development and validation of a radioimmunoassay for serum melatonin.

A radioimmunoassay using N-[3-(4-hydroxy-3-[4-hydroxy-3- [125I]iodophenylpropionyl)]-5-methoxytryptamine as tracer for determination of melatonin in the serum of different species is described. Melatonin antisera were raised in rabbits by immunization with a bovine serum albumin conjugate of N-[3-(2-aminoethyl)-5-methoxy indole] hemisuccinamide. A single high affinity, specific antiserum was obtained. In contrast to previous studies, the tracer was synthesised in one step in the absence of water, giving an excellent yield of highly pure product. No chromatographic purification step was needed. Polyethylene glycol in combination with goat antirabbit immunoglobulins was used to separate bound and unbound tracer. Sera were delipidized with Lipoclean prior the extraction of melatonin with diethyl ether. This sample preparation allows the determination of melatonin in the presence of widely varying amounts of lipids in human, rat and hamster serum. Using this extraction procedure, the sensitivity of the radioimmunoassay was approximately 1 ng/l of serum. Dilutions of sera and of synthetic melatonin gave the same parallel response in the radioimmunoassay. High performance liquid chromatography analysis of a serum extract showed only one immunoreactive peak co-eluting with synthetic melatonin. Characteristic diurnal rhythms of melatonin were observed in all species. All assay components including standards and serum controls are stable for at least 1 year at 4 degrees C, thus facilitating the determination of melatonin in a routine laboratory.

[Stimulation of nonspecific immune defense and antibody synthesis with zymosan].

A local sterile inflammation was induced in mice of different inbred strains by means of an i.p. injection of zymosan, a water-insoluble polysaccharide of the cell wall of Saccharomyces cerevisiae. This resulted in a strong enhancement of the unspecific resistance against infections with a LD 90 of gram-negative bacteria (Escherichia coli, Pseudomonas aeruginosa, Klebsiella pneumoniae and Bordetella bronchiseptica). Furthermore, the treatment with zymosan before or after an immunization with sheep erythrocytes (SE) resulted in a significantly increased specific IgM and IgG primary response in the spleen. However, it was particularly striking that the number of plaque-forming cells against SE was very drastically enhanced in non-immunized mice, in particular cases even up to more than 70 times over the background. Consequently, zymosan possesses immunomodulating effects both on the unspecific resistance and on immune reactions. These results are discussed in relation to the Lyt+-B-cells and an acute phase reaction of B-cells.

[Synchronous occurrence of neoplasias of the thyroid gland and hyperthyroid nodular struma in an endemic area].

The simultaneous occurrence of hyperthyroidism and differentiated carcinoma of the thyroid gland is more frequent than previously assumed. In our endemic goiter region 16.5% of patients with thyroid carcinoma have hyperthyroidism and 1.8% of patients with hyperthyroidism have a thyroid carcinoma. The tumors are predominantly located in autonomous adenomas but are also found in cold nodules. Not only scintigraphically cold nodules but also warm and hot nodules must be suspected of malignancy. Consistent preoperative cytodiagnosis of all goiter nodules, including hot nodules, is therefore recommended. Surgical treatment of toxic multi-nodular goiters should be considered more often.

[Multiple pregnancy following in vitro fertilization and embryo transfer].

In a 32 year old patient 14 eggs were harvested and inseminated with donor semen. Six embryos were transferred resulting in a triplet-pregnancy. In spite of a septic disease three preterm but healthy children were born by caesarean section 199 days after ET (male 1620.0; female 1180.0; male 1560.0). Up to nowadays there is not any handicap. We don't know a general but safe method for avoiding multiple pregnancies.

Comparison of cineradiographic and photodetection techniques for assessing velopharyngeal function during speech.

This study investigated the extent to which high-speed lateral-view cineradiography and a photodetector system described by Dalston (1982) provided comparable information concerning velopharyngeal activity. We observed the production of isolated utterances and running speech for 2 subjects in three contexts. A comparison was made between the times at which the velopharyngeal port was observed by each method to begin opening, reach maximal opening, begin closing, and initially attain closure. The photodetector system was found to be sensitive to changes in velar position observed by cineradiography. The correlation between the magnitude of photodetector output and the magnitude of velar displacement from the posterior pharyngeal wall was .89 for Subject 1 and .78 for Subject 2. The lack of a perfect relation between the output of the photodetector device and movements of the velum observed by cineradiography was not unexpected given the two-dimensional limitation of lateral cineradiography and the complexity of events associated with changing port size. The significance of these findings is discussed with respect to the potential research and clinical uses of this photodetector system.

[Persistence of antibodies of the IgG class against cytomegaloviruses in blood donors].

High titre plasmas gained from blood donors are the initial material used for producing human immunoglobulin containing cytomegalovirus antibodies (CMV-HIG). For this purpose the sera gained from 467 permanent donors at the District Institute for Blood and Transfusion Service in Berlin were investigated on their CMV antibody content of IgG class by means of an indirect immunofluorescence test (IFT). The infection rate of blood donors amounted to 62% (291/467). CMV-IgG titre greater than or equal to 1:40 was determined in 88 sera (18.8%) and greater than or equal to 1:160 in 14 sera (3%). Two CMV-HIG laboratory samples (charges 3113 and 3117) were produced from these plasmas. Particularly immunoglobulin fraction of charge 3117 (No. 3117 N II) revealed excellent antibody titres (IFT 1:640 [CMV-IgG] or 1:40 [CMV-IgM] respectively, neutralisation test 1:32). Checks made with the donors' CMV-IgG titres after repeated application of plasmapheresis resulted in maximal titres changes of two dilution stages in a period of 15 months. Thus, in producing CMV-HIG a sure, tested pool of donors can be resorted to.

[Therapy of suspected intrauterine fetal retardation].

In a randomized prospective study, performed on 45 pregnancies with clinical and sonographic suspicion for intrauterine fetal growth retardation, examinations were done to evaluate the therapeutic effect of bed rest and of additional daily administered glucose infusions or oral galactose applications respectively on the fetal growth, the hemodynamic, respiratory and endocrine insufficiency of the materno-feto-placental unit as well as selected biochemical parameters of the umbilical blood. There is no positive therapeutic effect, either on the impaired endocrine partial function nor on the reduced respiratory function of the feto-placental unit. It is obvious, that despite the improvement of the nutritional supply of the fetus and the revival of the regular fetal growth, the previous lack of fetal growth can not be altered. This can be demonstrated in all three therapeutic groups due to the fact of a resulting high hypotrophy-rate of the newborns. The results prove that the additional supply of the fetus with glucose or galactose does not remarkably improve the therapeutic effectivity as opposed by strict bed rest alone.

[Intraoperative rapid biopsy of thyroid neoplasms and surgical procedures in atypical adenoma].

For a differential diagnosis of atypical adenoma and highly differentiated follicular carcinoma cryosections are of value only if findings are positive. Between 1977 and 1982 we observed 85 thyroid carcinomas and 47 atypical adenomas. In 5 cases the diagnosis on the basis of a cryosection had failed to detect a carcinoma. In patients with atypical adenoma we performed a subtotal resection; a primary lobectomy was not carried out as it would have meant exposing an unnecessarily high percentage of patients to an increased risk of complications. 3-8 years postoperatively the patients show no pathological findings.

[Relation between corpus luteum function and serum prolactin concentration in early pregnancy].

In 20 patients between the 6.-8. week of pregnancy serum prolactin concentration could be decreased successful by administration of a single dose bromocriptine. Coincidentally the steroid secretion of the corpus luteum has been monitored by measurement the serum concentration of progesterone and estradiol. There are no prolactin related changes in the corpus luteum function under these conditions.

[Relation between corpus luteum function and serum prolactin concentration early in pregnancy].

In 23 patients between the 6.-8. week of pregnancy the serum prolactin concentration excessively increased by bolus injection of 10 mg metoclopramide. Coincidentally the steroid secretion of the corpus luteum has been monitored by measurement the serum concentration of progesterone and estradiol. There are no changes in the corpus luteum function caused by hyperprolactinaemia under these conditions.

[A so-called atypical adenoma of the thyroid in an endemic struma area].

In view of the benign clinical course of the atypical adenoma and the high diagnostic accuracy of our frozen sections we feel that subtotal strumectomy with in toto removal of the adenoma and closely spaced follow-ups constitutes an adequate treatment of this thyroid neoplasm. In our opinion primary lobectomy means unnecessarily exposing a disproportionately high percentage of patients to the higher complication risk of this more radical surgery. From 1977 to 1982 there were 47 cases of atypical adenoma among our patients. The ratio men: women was 1:3.7, the mean age of the patients was 44 years. The patients underwent subtotal strumectomy. In the period under review "atypical adenoma" diagnosed on the basis of frozen sections turned out to be carcinoma in 5 instances. The follow-up period of the 47 patients that underwent surgery ranges from 1 1/2 to 7 years. None of the patients has had tumor regrowth so far.

[The chemical composition of "zymosans" from yeasts].

The chemical composition of "Zymosanes" (complex cell-wall-polysaccharides), prepared by different methods from Baker's yeast, were analyzed by classical and modern techniques, e.g. GLC and TLC. They are very complex natural products consisting of a basic structure of polysaccharides, which proteins and different lipids are bound to. Higher purified zymosanes seem to be free from nucleic acids and do not contain sialic acid.

[Reference values for the inhomogeneity parameter V2/V1].

In 1,169 healthy subjects (821 males and 348 females) aged 15 to 70 years was determined parameter V2/V1 from the flow-volume loop for the estimation of unequal ventilation by means of the electronic spirometer Pneumotest. Reference values and prediction equations for parameter V2/V1 are reported.
