[Androgen therapy of incurable breast neoplasms. Controlled clinical study: nandrolone-testololactone-drostanolone].

In 91 patients with advanced breast cancer testololactone, drostanolone, and nandrolone were compared in a controlled clinical trial. Remissions were registered after 4 weeks and after another 12 week period; during this second interval the patients received an additional treatment with cyclophosphamide. There was no difference in the effectivity between the three drugs. Remission rate was in average after 4 weeks 24% and after 16 weeks 46%.

[Silence in children with oncologic diseases (author's transl)].

Silence is one of the dominant features in the behavior of fatally ill children and of the adults who care for these children. Silence signals the effectiveness of taboos that are still present in society, hospital personnel as well as in parents and children.

[Surgical interventions on the vertebral bodies of the thoracic spine (author's transl)].

27 operations on the vertebral bodies of the thoracic spine are reported. Indications for operation included markedly progressive infantile and juvenile scoliosis, correction of severe kyphoses in congenital malformation, fractures, Scheuermann's disease and post-laminectomy as well as tuberculous spondylitis and tumors. The two uppermost thoracic vertebrae can be reached by the method of Southwick and Robinson, the two lowest vertebrae from an extraperitoreal-subdiaphragm approach. Thoracotomy was chosen for the remaining thoracic vertebrae. If the staff and material are available, operations on the thoracic vertebrae can be regarded as relatively safe and, in many cases, are the only possibility for obtaining a satisfactory or good result. In spite of this, strict indications must be observed. Severe complications (death, paraplegia etc.) did not occur in any of the patients.

Comparison between glucose and a combination of glucose, fructose, and xylitol as carbohydrates for total parenteral nutrition of surgical intensive care patients.

The effects of four day periods of infusions of 600 gm/24 hours glucose and 600 gm/24 hours of a combination of glucose, fructose, and xylitol were compared. This study was performed during total parenteral nutrition of twelve postoperative patients with major complications. The mean plasma glucose level was significantly lower during the infusion of the combination of sugars (154.2+/-19.5 mg/100 ml versus 193.9+/-15.0 mg/100 ml[p is less than 0.005). Furthermore, the required dosage of exogenous insulin was significantly lower (18.9+/-12.3 units/day versus 43.7+/-19.7 units/day [p is less than 0.01). Mean renal carbohydrate losses were 0.85 per cent during glucose infusion and 1.7 per cent during infusion of the combination. The influence of both infusion regimes on values for pH, base excess, lactate, pyruvate, free fatty acids, insulin, sodium, potassium, chloride, magnesium, phosphorus, bilirubin, alkaline phosphatase, SGOT, and SGPT 0.85 has been investigated. No clinical side effects were observed. It is concluded that the administration of the investigated combination of glucose, fructose, and xylitol is justified in patients in whom hyperglycemia during infusion of glucose alone is difficult to control with insulin.

The basle PEEP weaner. A versatile device for respiratory assistance.

A device to modify the physiological impact of the abrupt transition of IPPV and PEEP to spontaneous breathing is described. It consists of a coaxial breathing system with a spring-loaded reservoir bag and underwater pressure limit. Electric alarms are incorporated.

[Antibiotic prevention using cephalothin].

A single intravenous dose of Cephalotine, given at the start of operations, significantly reduces the incidence of postoperative wound infections. The concentration of Cephalotine was maintained throughout operation. The occurrence of other infections was also reduced. The results of this study imply that operations on patients with higher infectious risk should be accompanied by antibiotic prophylaxis.

[Epidemiology of gastric cancer. 2nd communication (author's transl)].

Of 257 patients with gastric cancer and of 766 persons with different stages of chronic gastritis or normal gastric mucosa, 35 items from epidemiologic studies were used for this analysis. The comparison of epidemiology of chronic gastritis with epidemiology of stomach cancer by univariate and multivariate variance analysis shows numerous common variables (social group; digestive diseases). Furthermore gastric cancer is characterized by variables which give hints to cancerogenous noxes. Gastric cancer patients are distinguished from persons with atrophic gastritis by the following variables: cancerogenes; social class; genetic factors, and digestive diseases. It is suggested that these factors are relevant for stomach cancerogenesis in atrophic gastritis.

[The technic of tracheotomy: experience with 250 tracheotomies].

The results of 250 tracheostomies, primarily or secondarily performed at the intensive care unit of the Surgical Department of the Basle University, are presented. The standardized technique of tracheotomy is described, details for the choice and management of large prestretched double-cuff tracheostomy tubes are given, a new method of low-pressure cuff-blocking is outlined. The follow-up studies --autopsy, clinical examination and tracheoscopy--demonstrate a very low complication rate, due to the technique of tracheostomy, the preferred tracheostomy tubes and the method of alternate low-pressure cuff-blocking.
