[Which hormone therapy should be used in advanced breast carcinoma in males?].

A 76-year-old man with advanced carcinoma of the breast who had not been orchiectomized underwent sequential hormonal treatment. The tumour progressed during monotherapy with dexamethasone. Objective regression of the tumour followed sequential administration of tamoxifen (twice) and the two aromatase inhibitors, aminoglutethimide and testololactone. In addition, his condition remained stable on two progestogens in high doses, medroxyprogesterone acetate and megestrol acetate.

[Congenital protein C deficiency and thromboembolic diseases].

Besides its cofactor protein S and antithrombin III, protein C is one of the most important inhibitors of plasma coagulation. In seven members of a family as well as in three further unrelated patients, a congenital protein C deficiency with thromboembolic diseases including a coumarin necrosis was observed in two cases. Deficiency of protein C predisposes to the occurrence of thromboembolism, but the severity of the underlying heterozygotic genetic defect can also vary within the family. Long-term oral anticoagulation with phenprocoumon is the therapy of choice. Genetic counselling should always be carried out.

Molecular weight analysis of Entamoeba histolytica antigens recognized by IgG and IgM antibodies in the sera of patients with amoebiasis.

The sera of four patients with amoebic liver abscess and two patients with diarrhea caused by Entamoeba histolytica were tested on "Western blots" prepared from E. histolytica strains A3, SFL-3 and HK-9 separated by SDS-PAGE. IgG antibodies in the sera from patients with liver abscess were found to react with several "major" (i.e. strongly staining) band of approximate molecular weights 150-250 kD, 100 kD, 80 kD, 60 kD, 35 kD, 30 kD, 20 kD, and 5-15 kD and the 150-250 kD group of bands. A comparison of the patterns of reactivity obtained with these sera and strains A3, SFL-3 and HK-9 would indicate that some of these bands, especially the 5-15 kD groups of bands might carry antigenic determinants specific for individual strains of amoebae. This 5-15 kD group of bands is completely pelleted at 30.000 g and could therefore be associated with the cytoplasmic membrane. The 150-250 kD group of bands consists of smaller subunits of 40-65 kD molecular weight linked by disulfide bridges; the respective proteins are sensitive to digestion with elastase but not with trypsin, and seem to carry antigenic determinants containing sugar residues as evidenced by their sensitivity to treatment with NaJO4.

[Arteriosclerotic changes in the carotid arteries in patients with hypersensitive carotid sinus reflex].

Thirty-seven patients (15 female, 22 male; aged 46-89 years, mean age 68.8 years) with III or IV grade carotid sinus reflex carotid arteries were investigated by sonography (B-scan and CW-Doppler). It was demonstrated that stenoses or arteriosclerotic plaques, localized in the region of the carotid bulb or bifurcation, were present on both sides in 27 patients and on one side only in four patients. Five patients did not show these changes and four of them had thickening of common carotid artery endothelium. Thus, vascular changes occurred in 35 patients. Occlusions and stenoses greater than 50% were visible in five patients. Primarily bilateral changes were shown as well as unilateral and bilateral pathologic reflex response; main localization was the posterior wall of the carotid bulb. Thus, arteriosclerotic changes seem to be important in the development of pathologic carotid sinus reflex. Cerebral complications (left-sided PRIND) occurred in one patient who did not have high degree carotid stenosis. In comparison to other authors who investigated only with CW-Doppler a much higher rate of arteriosclerotic wall changes and a greater occurrence of bilateral changes could be demonstrated by application of the highly sensitive B-scan sonography.

High-resolution real-time ultrasound of the carotid bifurcation in patients with hyperactive carotid sinus syndrome.

The carotid bifurcations and the common carotid arteries of 36 patients with the diagnosis hyperactive carotid sinus syndrome (HCSS) were investigated by continuous wave (CW) Doppler ultrasonography and high-resolution real-time B-scan. Using these non-invasive tests, the functional impact of luminal stenosis and the morphological changes resulting from arteriosclerotic deformities could be established. Significant differences were found in comparison with a reference group of 199 patients with a high risk of arteriosclerosis. In the HCSS group, 5 patients had a stenosis of more than 50% at the origin of the internal carotid artery on both sides, or on one side in combination with large plaques or a complete occlusion on the contralateral side. Seventy-five per cent of patients in the HCSS group, as compared to only 23.5% of the control group, had effective arteriosclerotic changes in the carotid bifurcation on both sides; 4 patients had such changes only unilaterally. Marked additional bilateral arteriosclerotic depositions were detected in the common carotid arteries of 17 patients (47.2%). In 5 patients no arteriosclerotic lesions were detectable in the carotid bifurcations, but marked changes were found in both common carotid arteries. These data indicate that bilateral arteriosclerotic changes in the carotid bifurcations and/or the common carotid arteries represent an important pathophysiological factor for the development of an HCSS.

[Creatine kinase and creatine kinase isoenzymes in the serum of newborn infants. 1. Effect of the mode of delivery on total CK and CK-B].

The total CK-, CK-B- and CK-M-activity was determined in 48 newborns from the first to the third day of life by use of the "CK-Monotest" (Boehringer Mannheim). Opposite to a very small decrease of the total CK-activity a significant decrease of the CK-B is evident in the same time. At the first day the detection of CK-B was impossible in only 1 of 27 patients, but at the third day a low CK-B level was detectable in only 5 of 11 test persons. The activities of total CK and the CK-B were determined based upon these investigations in 245 newborns at the first day of life by use of the "CK-Monotest NAC activated" (Boehringer Mannheim). There was no significant difference of total CK- and CK-B-activity dependent on the mode of delivery (spontaneous vertex presentation, forceps delivery, caesarean section, breech presentation.) The proportion of the CK-B varied between 8 and 11% of the total creatine kinase in dependence on the mode of delivery.
