[Graduate teaching aspects to be considered in the planning of undergraduate dental courses].

The general part of the present paper deals with questions concerning the demanding conceptual design of teaching from the aspect of creative studying. The students' qualification for responsible scientific work and creative studying is considered to be an essential question of the conceptual design of the demanding process of learning and acquiring the scientific bases. Thus, it will be possible to improve effectively the attitude towards studying and the engagement in creative achievements (in a narrow sense or in explicit forms of organization). In the empirical part of the present paper, the results from the interviewing of 240 dental students on relevant conceptual factors of teaching are discussed. An analysis of situation led to valuable insights and conclusions as to more effective teaching in the framework of stomatological study.

Competency-Based Education↗

[Problems and aspects of the solution of the health education of schoolchildren in the lower grades in questions of oral hygiene].

It is dealt with the significance of the education about oral hygiene which must be started at preschool age, the repercussions of its neglect being illustrated by authoritative studies (e.g., those of Künzel). The results obtained by questioning 107 pupils (aged between 8 and 10 years) and 33 dentists revealed the unsatisfactory state of information and education about oral hygiene. Furthermore, searching interviews of class-teachers showed that the existent opportunities of taking influence are not used or that their use is not effective enough. Ways of realization are presented in the form of suggestions referring not only to curricular and extra-curricular possibilities but also to aspects of the planning and management as well as of the instruction and continuing education of dentists and teachers.

[pH measurements following delivery from breech presentation (author's transl)].

In 1977 the adaptation capability of 22 out of 89 breech presentation new-borns was studied by way of punctual pH-measurements within the first 30 minutes after birth. Thus, it was possible to gain an insight into the extra-uterine adaptation, especially with respect to the mode of delivery. Further points of comparison were parity, maturity and a possible correlation with the Apgar Score. The study included a comparison with identical measurements of 20 spontaneous head position new-borns. According to the results under certain preconditions a vaginal delivery in breech presentation is also today by all means to be justified.

Efficacy of propranolol versus placebo in long-term treatment in patients with mitral valve prolapse.

In 60 patients with mitral valve prolapse syndrome a randomized controlled interindividual double-blind study of propranolol (p) was performed. Patients received p 80 mg (A), 160 mg (B) and placebo (C) orally for 4 weeks. Prior to and after treatment, heart rate (HR) and blood pressure (RR) as well as systolic time intervals (STI) were measured and corrected for heart rate--electromechanical systole (QS2I), left ventricular ejection time (LVETI), and preejection period (PEPI). The ratio PEP/LVET was calculated. Plasma levels were measured by an optimized fluorimetric method. 1. STI lay in the upper part of the normal range, indicating that some patients had a hyperkinetic cardiac function. 2. P had no influence on QS2I and LVETI in A and B. PEPI was prolonged (A: +13.2 ms, B: +14.2 ms) and PEP/LVET was increased (A: +0.040, B: +0.050). 3. As indicated by the changes in HR, RR, PEPI, and PEP/LVET p showed in B compared to A only minor additional effects. 4. Plasma levels of p were in B three times higher than in A (A: 89.2 +/- 10.0 nmol/l B: 246.7 +/- 30.5 nmol/l). With a dose of 80 mg propranolol a point close to the plateau of maximum efficacy was reached, where a higher dose resulted only in small additional negative inotropic effects.

Human cutaneous topography a new photographic technique: observations on normal skin.

We describe herein a convenient and inexpensive technique for photographic examination of the surface of the skin. With this method, the fine lines of the skin appear as furrows or folds. The furrows demarcate units of epidermal structure which we have designated polyangular units. These, in turn, are arranged in a mosaic pattern. This technique was used to define the topography of normal young adults. Surface features fall into four major patterns: 1) dermatoglyphic, 2) hair bearing, 3) frictional, and 4) flexural.

[Echocardiographic investigation of the left ventricular outflow tract in healthy children (author's transl)].

The width of the left ventricular outflow tract was measured in the echocardiograms at the beginning and the end of the systole in 80 healthy children ranging from 3 to 16 years. The values were referred to the body surface area and the heart volume which was estimated by mean of X-ray. The regression lines as well as the confidence levels of 95 per cent are marked. It becomes obvious that the dispersion of the data related to the heart volume is less than that related to the body surface area. The width of the left ventricular outflow tract with reference to the heart volume and the body surface area resp. shows in relation to the heart volume and the body surface area resp. an exponential behaviour, i.e. in the growing heart the outflow tract decreases in relation to the two parameters. The narrowing of the outflow tract from start to end of the systole is about one third.

[Chemotherapy of inoperable endothoracic tumours (results of the Karrer and aco polycytostatic therapy) (author's transl)].

The success of chemotherapy of inoperable endothoracic cancer largely depends on interdisciplinary cooperation. 30 years of research have succeeded in developing some promising combinations of cytostatic agents. Administered sequentially they are better tolerated and this has made outpatient treatment possible. 87 persons had polycytostatic therapy according to the Karrer/Sighart formula. It was followed by a difinite improvement in the general condition of the patients, but without any substantial lengthening of the average survival time. The ACO scheme (Seeber/Schmidt) has been used since April 1977 in 42 persons. Preliminary results indicate a definite prologation of survival time, especially in cases of small-cell bronchogenic carcinoma.

[Convalent binding of various tetra- and tricyclic heterocycles with isothiocyanate structure to human serum albumin and bovine gamma globulin. Part 24: Immunosuppressive agent - antigen conjugates (author's transl)].

In view of their potential activity as antigen-specific immunosuppressive agents, the authors produced conjugates by treating the tri- and tetracyclic compounds 1-8 (with isothiocyanate structure) with human serum albumin (HSA) and bovine gamma globulin (BGG). In certain cases the conjugates of BGG were obtained only under conditions leading to protein degeneration. The corresponding products were devoid of antigen-specific immunosuppressive activity and must be considered to be merely repository forms of the respective low-molecular compounds. In contrast, the conjugates of HSA could be obtained under conditions under which no protein denaturation was detectable. These conjugates may be used for the planned investigation. After reaction of the isothiocyanates with the proteins, time dependent changes were observed in the ultraviolet spectra of some conjugates, the possible causes of which are discussed.

A 1H nuclear-magnetic-resonance study of the solution conformation of the isoinhibitor K from Helix pomatia.

The isoinhibitor K from Helix pomatia was investigated by high-resolution 1H nuclear magnetic resonance at 360 MHZ. Detailed studies of the labile protons and the resonances of the aromatic residues indicated extensive homologies between the spatial structures of the snail inhibitor, the basic pancreatic trypsin inhibitor (Kunitz) from bovine tissue and the cos colostrum trypsin inhibitor. Comparison of these three homologous inhibitors indicated that the overall flexibility of the globular protein conformation is reduced and its stability with respect to thermal denaturation raised when the content of amino acids with charged side chains is increased. It is suggested that this relation between amino acid composition and stability of the globular solution structure might be valid also for other classes of homologous proteins.
