Three-dimensional structure of rabbit liver [Cd7]metallothionein-2a in aqueous solution determined by nuclear magnetic resonance.

In previous work the metal-polypeptide co-ordinative bonds in the major protein species of a reconstituted [113Cd7]metallothionein-2 preparation from rabbit liver in aqueous solution were determined, the secondary polypeptide structure was found to contain several half-turns and 3(10)-helical segments, and a preliminary characterization of the overall polypeptide backbone fold in the beta-domain containing the three-metal cluster, and the alpha-domain containing the four-metal cluster, was obtained. Using a new, more extensive set of nuclear magnetic resonance data these earlier structures were improved by new structure calculations. The new experimental data consist of distance constraints from measurements of nuclear Overhauser effects, and dihedral angle constraints derived from both coupling constants and nuclear Overhauser effects. The structure calculations were performed with the program DISMAN. Since no information on the orientation of the two domains relative to each other could be obtained, the structure calculations were performed separately for the alpha-domain and the beta-domain. The average of the pairwise root-mean-square distances among the 20 structures with the least residual violations of input constraints was 2.9 A for the beta-domain and 1.4 A for the alpha-domain (1 A = 0.1 nm). The overall chirality of the polypeptide fold is right-handed for the beta-domain and left-handed for the alpha-domain. For each of the seven metal ions the local chirality of the co-ordination of the four cysteinyl Sy atoms is clearly defined. The improved structures of both domains show the previously noted differences relative to the recently published crystal structure of metallothionein-2a from rat liver.

Effects of 5-azacytidine and methyl-group deficiency on NAD(P)H: quinone oxidoreductase and glutathione S-transferase in liver.

Treatment with 5-azacytidine or dietary methyl-group deficiency effected DNA hypomethylation in mouse liver. With these treatments, NAD(P)H: quinone oxidoreductase (EC 1.6.99.2) and some glutathione S-transferase (EC 2.5.1.18) activities were over-expressed, lactate dehydrogenase (EC 1.1.1.27) activity was unaffected and the level of cytochrome P-450 was decreased. The 5-azacytidine induction of NAD(P)H: quinone oxidoreductase was significantly suppressed by puromycin, suggesting that increased enzyme activity results from an elevated level of enzyme-protein synthesis. Regulation at the transcriptional level was revealed by a substantial increase in mRNA of NAD(P)H: quinone oxidoreductase, as shown by Northern-blot analysis. The enzyme pattern observed with 5-azacytidine and with the (carcinogenic) dietary methyl-group deficiency resembles that found in hepatic nodules.

[Osteosis cutis multiplex, an organoid nevus?].

We report on a 71-year-old woman suffering from histologically proved osteosis cutis multiplex (Arzt's disease). The differential diagnosis includes multiple milia, closed comedones and hidradenomas. We discuss various etiopathologic concepts and a nevogenic origin in particular.

[Etretinate therapy in solitary keratoacanthomas].

In an open clinical trial, 5 patients suffering from solitary centrofacial keratoacanthoma were treated with etretinate. The dosage was 1,5 mg/kg body-weight. All patients showed complete healing after 4-5 weeks of therapy. There were no recurrences within 3 months after therapy.

The secondary structure of the toxin ATX Ia from Anemonia sulcata in aqueous solution determined on the basis of complete sequence-specific 1H-NMR assignments.

The toxin preparations ATX I, ATX Ia and ATX Ib from Anemonia sulcata were investigated by proton nuclear magnetic resonance (NMR). High-resolution phase-sensitive two-dimensional NMR experiments were used to monitor the separation by high-performance liquid chromatography of the two isoproteins ATX Ia and ATX Ib. For ATX Ia complete sequence-specific resonance assignments were obtained and the secondary structure was determined. To obtain the NMR assignments we used a variant of the sequential assignment technique which relied extensively on cross-peak fine-structure analysis in phase-sensitive spectra, using spectrum simulations based on density matrix calculations with the program SPHINX. These procedures, which resulted in extensive amino acid spin system identifications prior to the sequential assignments, should be generally applicable for small proteins with relatively narrow 1H-NMR lines. The secondary structure of ATX I includes a beta sheet consisting of four strands. No evidence was found for the presence of regular helical segments. The four beta strands are connected by two extended loops and a tight turn, for which further characterization has to await the complete determination of the three-dimensional structure.

The effects of a transtelephonic surveillance and prehospital emergency intervention system on the 1-year course following acute myocardial infarction.

We evaluated the effect on morbidity and mortality of a post-myocardial infarction management and intervention system. One thousand four patients were prospectively randomized to an intervention group that included routine and emergency transtelephonic follow-up and ECG monitoring or to control; all subjects were followed for 1 year. For symptoms suggestive of myocardial ischemia, intervention patients telephoned the project emergency office, were instructed by a project nurse to self-administer intramuscular lidocaine with an auto-injector, and were then transported to the nearest emergency facility. Cardiac death or arrest was decreased 29% in intervention patients (p = 0.066), while all-cause mortality was decreased by 24% (p less than 0.11). Routine transtelephonic ECG monitoring detected ventricular ectopy in 48% of intervention patients, with almost 50% of these findings classified as complex forms. Ventricular ectopy detected during routine calls within 60 days of the acute myocardial infarction conferred a threefold increase in mortality (p = 0.001). In addition, control patients were 2.4 times more likely to be clinically depressed (p less than 0.03) and returned to work less quickly (p less than 0.03) when compared to intervention patients. Lidocaine injections were associated with an absence of ventricular ectopy on arrival at the Emergency Room in 64% and with a low incidence of lidocaine-associated side effects. There was only one case of unauthorized use of the self-injector. These findings demonstrate that an outpatient post-myocardial infarction transtelephonic surveillance program can be safely and effectively conducted, can detect complex ventricular arrhythmias of prognostic significance, can improve the quality of life, and may reduce 1-year cardiac mortality.

Diaphragm insertion increases human vaginal oxygen tension.

The normal insertion of a contraceptive diaphragm into the vagina of five young women induced a dramatic increase in the vaginal oxygen tension (131 +/- 13 mm Hg, mean +/- SD, n = 5), which slowly fell at an initial rate of 0.9 +/- 0.2 mm Hg/min. Withdrawal of the diaphragm immediately restored vaginal oxygen tension to the normal hypoxic state (11 +/- 12 mm Hg). The change in gaseous equilibrium created by the trapped air of the diaphragm could have significant actions on the microbial activity and balance of the vagina.

[Comparison of urodynamic findings before and after loop operations for recurrent incontinence].

30 patients with a recurrent stress incontinence were undergoing a dura-sling operation. The mean period of incontinence complaints up to the second surgery was 2.6 years. In the follow-up investigation the urodynamic parameters showed a significant improvement of the UCP, the urethral functional length and the DepQ. One year after surgery 86% of the women showed both a clinical and a urodynamic continence. A short term of urination disorders has to be considered in a part of the cases.

[One-step synthesis of 2-aminothieno [2,3-d]thiazen-4-ones in some cases 5,6-anellated from ethyl 2-benzoylthioureidothiophen-3-carboxylates and evaluation of their anti-allergy activity].

The cyclization of ethyl 2-benzoylthioureidothiophen-3-carboxylates under basic conditions is known to give 2-thioxothieno[2,3-d]pyrimidin-4(1H, 3H)-ones. On the other hand, we have found that ethyl 2-benzoylthioureidothiophen-3-carboxylates 10-16 on treatment with concentrated sulphuric acid or polyphosphoric acid/ethanol undergo cyclization to give the new 2-aminothieno[2,3-d][1,3]thiazin-4-ones 17-23, in some cases 5,6-anellated. Reaction of 10-16, which are readily available from ethyl 2-aminothiophen-3-carboxylates 3-9 and benzoyl isothiocyante affords the title compounds 17-23 in good yields. Mass spectral fragmentation of 17-23 is discussed. A series of 2-aminothieno[2,3-d][1,3]thiazin-4-ones, which contain a free or substituted amino group was evaluated in the rat active cutaneous anaphylaxis test for anti-allergy activity. One compound, 23 had weak activity in the range of theophylline. Only high concentrations of this compound inhibited weakly the histamine release from rat peritoneal mast cells activated by protamine sulfate.

[Synthesis of N-alpha-(arylsulfonyl)-4-amidino-phenylalanyl-prolines and N-alpha-(arylsulfonylglycyl)-4-amidino-phenylalanyl-prolines and testing of them as inhibitors of serine proteinases. 33. Synthetic inhibitors of serine proteinases].

The headline compounds were prepared by the reaction of N alpha-(arylsulfonyl)- and N alpha-(arylsulfonylglycyl)-4-cyanophenylalanines to the L- and D-proline derivatives. These compounds were transformed in a usual manner in the amidino compounds. The substitution of the pyrrolidine residue by L-proline in this type of inhibitors decreased the activity against serine proteases, especially against thrombin. In the case of the D-proline derivatives the decrease of the inhibitor activity against thrombin was very high.

[Synthesis of 3-substituted 6-methyl-4-pyridyl-2 (1H) pyridones and testing of their cardiovascular action].

Reaction of our previously described and in 4- and 6-, respectively, position with 3- and 4-, respectively, substituted 3-cyan-2(1H)-pyridones 4 and 5 with concentrated sulphuric acid had yielded the 3-carbamoyl-pyridones 8 and 9. Hofmann reaction was followed and the 3-amino substituted pyridones 10 and 11 were formed. As by-products the 5-bromo substituted 3-amino-pyridones 12 and 13 were obtained. 4 and 5 were refluxed in the presence of diluted sulphuric acid to yield the decarboxylated pyridones 14 and 15. Cardiotonic Activity of the compounds 4-9 compared to amrinone (Cordemcura) was investigated.

[Synthetic inhibitors of serine proteinases. 34. The effect of modification of the amidino function of N-alpha-substituted 4-amidinophenylalanine amides on inhibitory activity].

Cyclic amides of N alpha-arylsulfonylated 4-amidinophenylalanine are selective inhibitors of thrombin. The exchange of the amidino function for an aminomethyl residue does not influence the selectivity and potency of their inhibitory activity. In contrast, the modification of the amidino function of the N alpha-arylsulfonylaminoacylated compound N alpha-(2-naphthylsulfonylglycyl)-4-amidinophenylalanine piperidide results in a drastic loss of inhibitory activity. Only the oxamidino derivative possesses considerable high affinity for thrombin. Obviously, in tight binding inhibitors of thrombin structural variation results in any case in a loss of inhibitory activity.

[Diagnosis of the recurrence of epithelial ovarian cancer with the tumor marker CA 125].

Aim of this study was to evaluate the significance of serum CA 125 levels in our ovarian cancer follow-up programme for earlier detection of recurrent or progressive disease. 43 out of 133 patients developed tumour progression, whereby 42 (97.7%) showed positive serum CA 125 levels and in 25 patients (58.2%) the increase in tumour marker preceded the first clinical signs by 4 to 36 weeks. The mean lead time was 14.2 weeks. Hence, determination of serum CA 125 should be mandatory in the follow-up investigation of women with epithelial ovarian carcinomas.

Sequence-specific 1H-NMR assignments in rat-liver metallothionein-2.

As a basis for the determination of the metal-coordination topology and the three-dimensional fold of the polypeptide chain, sequence-specific assignments were obtained for the 1H nuclear magnetic resonance (NMR) spectrum of metallothionein-2 from rat liver. The 1H spin systems of the 20 metal-bound cysteines were identified from comparison of two metal-homogeneous protein preparations obtained by reconstitution of the apometallothionein-2 with 113Cd2+ and 112Cd2+, respectively. The identification of the spin systems for the remaining amino acid residues and sequential assignments were then obtained with two-dimensional 1H-NMR experiments at 500 MHz. The assignments are complete except for two backbone amide protons, which were not observed, and the side-chain hydrogen atoms beyond beta CH2 for all eight lysines. The chemical shifts are presented at pH 7.0 and 25 degrees C.
