[The 24-hour-effect of bisoprolol on blood pressure at rest and during stress].

In a multi-center study, 188 patients with mild to moderate essential hypertension were treated with the beta-1 selective beta blocker bisoprolol. Treatment was started with 5 mg bisoprolol once a day; if the aim of therapy, namely normalising (less than or equal to 90 mmHg) the diastolic blood pressure at rest (DBP-R) 24 hours after drug intake was not achieved, the dose was increased every two weeks to a maximum of 20 mg. With the individual dose established in this way, the patients were admitted to long-term treatment that was scheduled to last one year. Prior to and during bisoprolol therapy, bicycle ergometry studies were carried out up to a maximum of 100 watts. After only two weeks on 5 mg bisoprolol, both resting and exercise blood pressures were clearly reduced. By means of success-related dose increments, normalisation of the DBP-R was achieved in 79% of the patients by the end of the 6th week. The percentage of patients manifesting load-positive blood pressure reactions during exercise was reduced from 63 to 18%. On conclusion of one year of treatment, 33%, 56% and 11% of the patients were receiving 5 mg, 10 mg and 20 mg bisoprolol, respectively. On the basis of subjective and objective parameters, including laboratory examinations, bisoprolol was seen to be well tolerated even over the long term.

Vasoactive intestinal polypeptide (VIP)- and neuropeptide Y (NPY)-containing nerve fibres in the penile cavernous tissue of green monkeys (Cercopithecus aethiops).

The cavernous body of green monkeys contains many unmyelinated and few myelinated axons. The unmyelinated axons form terminals in the adventitia of the arteries, between trabecular muscle cells, in the interstitium, and close to endothelium cells of the sinuses. All terminals displayed predominantly" small clear vesicles" and very few "large granular vesicles"; "small granular vesicles" were not seen. However, in rabbit penises, terminals with many large granular vesicles are prominent. Immunohistochemistry (PAP technique) showed a dense network of VIP- and NPY-reactive fibres around the arteries and around trabecular muscles. The density of nerve fibres was particularly high around the subendothelial cushions of the helicine arteries. Double staining for NPY and VIP revealed that both peptides were colocalized. Immunocytochemistry (preembedding PAP technique) showed VIP- and NPY-reactivity in terminals with small clear vesicles; the reaction product was bound to the cytoplasmic face of different membrane types. Although the intracellular localization of the reaction product is probably due to artefactual displacement during preparation, the uniformity of the terminals questions the view that large and small granular vesicles in all species characterize peptidergic and noradrenergic terminals, respectively. The essential findings can be summarized as (1) a high degree of uniformity of nerve terminals, (2) colocalization of VIP and NPY, (3) heavy innervation of the subendothelial cushions of the helicine arteries, and (4) possible innervation of endothelial cells.

Effect of bisoprolol on cardiac performance in coronary heart disease.

The effects of 5 and 10 mg bisoprolol once daily for 7 days on exercise ECG, myocardial perfusion and left ventricular function in 25 patients with stable coronary heart disease have been assessed in a double-blind, randomized, parallel group trial design. ST-segment depression during exercise was reduced by 56% by 5 mg bisoprolol and by 64% after 10 mg; the difference between the dose levels was significant. Heart rate, systolic and diastolic blood pressure and the rate-pressure product were reduced to similar extent both at rest and during exercise by both doses. Left ventricular thallium-201 scintigrams indicated a significant reduction in myocardial perfusion defects after 10 mg bisoprolol compared to baseline; however, the difference between the two active treatments was not significant. Left atrial and left ventricular diameters obtained by one-dimensional echocardiography, and the calculated shortening fraction, remained unchanged after bisoprolol, and so gave no evidence of a negative inotropic action. It is concluded that 5 mg bisoprolol was effective in once-a-day treatment of angina pectoris due to coronary heart disease, and a further improvement can be expected on increasing the dose to 10 mg.

The vascular anatomy of the cavernous body of green monkeys.

The cavernous body of monkeys was studied by light and electron microscopy. The intima of the deep artery contained longitudinal muscle cells, but cushions or valve-like structures were not found. Some branches of the deep artery directly connected to subtunical veins. Only the helicine arteries which drained into the cavernous sinuses displayed subendothelial cushions. The "epitheloid cells" forming the cushions unequivocally were smooth muscle cells. It is suggested that the helicine arteries open during tumescence, thereby diverting blood from the shunt vessels into the dilating sinuses, and that the rising intracavernosal pressure eventually occludes the shunt vessels. The endothelium of the sinuses contained many intermediate filaments, but there was no morphological evidence for the hypothesis that the cells are contractile.

Electrical activity of corpus cavernosum during flaccidity and erection of the human penis: a new diagnostic method?

We investigated 7 normal men and 1 diabetic patient with erectile dysfunction. Electromyography electrodes were placed in the corpus cavernosum of the penis and electrical activity was recorded during flaccidity. With sexual arousal the activity decreased and tumescence was initiated. During tumescence and full erection the electrical activity of the corpus cavernosum almost ceased but in the diabetic patient (neurogenic impotence) an increase was observed. This discoordination might be the cause of the erectile dysfunction. Recording the electrical activity of the corpus cavernosum in patients with suspected neurogenic erectile dysfunction could become clinically valuable, since this is the first test possible to study the function of the autonomic motor system that normally regulates penile function.

Standardized evaluation of erectile dysfunction in 95 consecutive patients.

We investigated 95 patients referred for erectile dysfunction by penile blood pressure measurement, the intracavernous papaverine test and Doppler investigation of the penile arteries. Furthermore, penile cutaneous perception threshold, bulbocavernosus reflex latency and somatosensory cortical evoked potentials of the pudendal nerve were measured. In selected cases cavernosometry, cavernosography and corpus cavernosum electromyography were performed. Doppler investigation of the cavernous arteries after papaverine injection was more reliable than penile blood pressure measurement in the diagnosis of arteriogenic erectile dysfunction. Decreased sensibility of the penis may be the sole factor responsible for inability to sustain an erection. Erectile dysfunction may be provoked by impaired function of the pudendal nerve. Penile cutaneous perception threshold measurement and corpus cavernosum electromyography are mandatory in the evaluation of neurogenic etiology. Cavernosometry and cavernosography are reliable methods in the determination of abnormal drainage from the corpus cavernosum.

[A case of Raeder syndrome in a patient with known multiple sclerosis].

In 1918 the Norwegian neurologist Raeder was the first to describe a syndrome consisting of unilateral headache, facial pain and a Horner's syndrome which is incomplete; this syndrome is accompanied by multiple parasellar cranial nerve involvement. This syndrome was named "Raeder's syndrome" or paratrigeminal syndrome. A survey of the literature concerning "Raeder's syndrome" is given; there a two varieties of Raeder's syndrome, the migrainous or reflex type and the symptomatic type with multiple parasellar cranial nerve involvement. The first variety has a close relationship to cluster headache and to the so-called pericarotid syndrome. A case of an own patient suffering from multiple sclerosis and a migrainous type of Raeder's syndrome is described, assuming that his basic disease MS is independent of his Raeder's syndrome.

[Clinical and radiologic findings before and following sling operation for recurrent incontinence].

In recent years, accurate preoperative diagnosis has been increasingly emphasized as an important therapeutic aspect of urinary incontinence in women. One of the diagnostic modalities employed is the lateral urethrocystogram. 25 patients with recurrent stress induced urinary incontinence were subjected to a dura-sling operation. The median duration of incontinence-related problems until secondary surgery (loop surgery) was 2.6 years. Radiological investigations before and after loop surgery showed, that this operation produces good morphological results, if the preoperative diagnosis is accurate. In 84% of the patients the continence obtained was verified by clinical evidence and radiologic findings.

Sequence-specific 1H-NMR assignments and identification of two small antiparallel beta-sheets in the solution structure of recombinant human transforming growth factor alpha.

Transforming growth factor alpha (TGF alpha) is a small mitogenic protein with about 35% sequence identity with epidermal growth factor (EGF). TGF alpha-like proteins have been proposed to play a role in oncogenesis and wound healing. This report describes sequence-specific 1H-NMR resonance assignments for recombinant human TGF alpha (hTGF alpha). These assignments provide the basis for interpreting NMR data which demonstrate that the solution structure of hTGF alpha includes an antiparallel beta-sheet involving residues Gly-19 to Leu-24 and Lys-29 to Cys-34 and a second, smaller, antiparallel beta-sheet involving residues Tyr-38 and Val-39 and His-45 and Ala-46. These data, together with constraints imposed by the disulfide bonds, are combined to construct a molecular model of the polypeptide chain fold for residues Cys-8 to Ala-46. The resulting structure is similar to that of mouse and human EGF. Human TGF alpha and mouse EGF, however, differ with respect to their structural dynamics, since amide proton/deuteron exchange is much faster for hTGF alpha than for mouse EGF at pH 3.5.

Facial expression of schizophrenic patients and their interaction partners.

In his earlier works, one of the present investigators attempted to show that psychologically disturbed persons may be characterized by their tendency to induce their social partners to reveal relatively stable and constant leitmotivs, inclinations to react, emotions and fantasies; hence, it should be possible to describe psychological illnesses by means of the patient's specific interaction strategies (Krause 1988; Krause and Lütolf 1988). We assume that this process of induction is neither deliberate nor self-reflective. Inductive modes of behavior are viewed as part of an unconscious process of problem solving--a process, however, which is unsuccessful, since it is no longer appropriate to the reality of the adult patient. The present paper is part of a more comprehensive research project, the object of which is to account for these kinds of interactive strategies.

The concentration of pepsinogen C in human semen and the physiological activation of zymogen in the vagina.

The relationship between male infertility and the pepsinogen C content in semen has been investigated. The activation of the seminal pepsinogen C in the vagina has been studied under physiological conditions. Samples of semen from 48 vasectomized males and from 46 males of infertile couples were analyzed for pepsinogen C by radioimmunoassay. No correlation was found between the level of pepsinogen C and seminal characteristics, including sperm concentration, motility, and morphologic features. The mean concentration of pepsinogen C was 42.2 micrograms/ml; the first, second, and third quartile were 18.4, 29.6, and 57.6 micrograms/ml, respectively. No significant difference in the level of pepsinogen C was observed between semen of normal quality, semen of reduced quality, and semen with aspermia. Activation of pepsinogen C occurred within 3 h when semen was incubated at pH below 5.0 at 37 degrees C. Intravaginal activation was investigated in six experiments in which semen from two males was instilled in three females. In four experiments with two couples, post-coital activation was investigated. Pepsin C activity in vaginal fluid was detected an average of 3 h (range 2-5 h) and 5 h (4-7 h) after instillation or ejaculation, respectively. Vaginal pH had then been below 4.5 for approximately 1 h. Pepsin C activity was present in the vagina for more than 24 h thereafter. It is most likely that seminal pepsin C is without influence on the fertilizing spermatozoon. However, pepsin C may exert a local effect in the vagina by degrading seminal proteins, thus preventing an immunogenic response in females.

[Synthesis of 3-cyano-6-methyl-4-pyridyl-2(1H)-pyridinethiones as well as 2-substituted 6-methyl-4-pyridyl-pyridine-3-carbonitriles and 3-carbonic acid amides, respectively, and testing their cardiovascular activity].

By the base catalyzed reaction of our previously described 3-cyano-6-methyl-4-pyridyl-2(1H)-pyridinethiones 1 and 2, respectively, with branched and unbranched, respectively, alkyl, aralkyl-, alkinyl-, hydroxyalkyl-, ethoxycarbonyl- and carbamoylalkylhalides the new in 2-position substituted 6-methyl-4-pyridyl-pyridine-3-carbonitriles 15-30 were formed. By means of oxidation of the 2-methylthio-substituted pyridine-3-carbonitriles 3 and 4 with potassium periodate and potassium permanganate in diluted acetic acid, respectively, the sulfinyl compounds 5 and 6, respectively, and the sulfonyl compounds 7 and 8, respectively, were prepared. By heating of 3 and 4, respectively, with concentrated sulphuric acid the 2-methylthio-pyridine-3-carboxamides 9 and 10 were obtained. The oxidation of these compounds with potassium periodate and potassium permanganate, respectively, had yield the pyridines 11 and 12, respectively, as well as 13 and 14, respectively.

[Cytochrome P-450 dependent drug metabolism--use of an in vitro test system in multiple chemical exposure].

Investigations of the influence of an actual exposure on the in-vitro-activities and the inducibility of cytochrom P-450 dependent enzymes have not been described in the literature until now. First results gained from mitogen-stimulated lymphocytes of exposed and not-exposed individuals suggest such an influence of the given mixture of noxious agents on the basic deethylational ability of the examined cells acting in the sense of inhibition. Possible causes for this phenomenon are discussed.

Conformation of [Cd7]-metallothionein-2 from rat liver in aqueous solution determined by nuclear magnetic resonance spectroscopy.

The three-dimensional structure of [Cd7]-metallothionein-2 from rat liver was determined in aqueous solution, using nuclear magnetic resonance spectrometry and distance geometry calculations. The experimental data provided proton-proton distance constraints from measurements of nuclear Overhauser effects, constraints on the geometry of the metal-cysteine clusters determined by heteronuclear correlation spectroscopy, and dihedral angle constraints derived from both coupling constants and nuclear Overhauser effects. The structure calculations were performed with the program DISMAN. As in previous studies with rabbit liver metallothionein-2a, the structure calculations were performed separately for the alpha and beta-domains containing the 4 and 3-metal clusters, respectively, since no interdomain constraints were found. For both domains, the global polypeptide fold, the location of polypeptide secondary structure elements, the architecture of the metal-sulfur cluster and the local chirality of the metal co-ordination are very similar to the solution structure of rabbit metallothionein-2a, but show considerable difference relative to the crystal structure of rat metallothionein-2.
