[The results of the dynamic observation of patients with reactive Yersinia arthritis].

As many as 50 patients with reactive Yersinia arthritis were followed up. There were 4 men and 46 women. The mean age of the patients was 31.1 yr. The mean follow-up period was 30.8 months. In 72% of the cases, the disease was preceded by diarrhea. During the first examination, mono-oligoarthritis was detected in 78% of the patients. Extra-articular alterations (damage of muscles, ligaments, heart, skin changes, etc) were also recorded. The results of bacteriological analyses were positive in 22 out of the 50 patients. In 16 persons, Yersinia might be cultivated from the different biological media: blood, feces, urine, fauces, and synovial fluid. In 47 persons, arthritis took a chronic course. Persistence of antibodies of the types 03 and 09 seen for many years merits attention. Monoclonal anti-Yersinia antibodies were also detectable repeatedly. In 8 patients, in whom Yersinia were cultivated from a lot of the biological media, the disease ran an especially grave course. The role of circulating specific antibodies in patients with Yersinia arthritis and the trigger role they may play in the development of RA and ankylosing spondylarthritis requires further discussion.

[Yersinia arthritis in children].

Clinical and laboratory evaluation of arthritis in 36 children with diagnosed enteric yersiniosis is presented on seven-years' follow-up data. Twenty-eight patients were found to have an active course of the disease developing in cycles with a favourable outcome and no residual effects. In eight cases arthritis took a protracted (up to 8-10 months) or chronic (over 12 months) course which ended in clinical and laboratory remission; in three of the eight cases the clinical picture was similar to that of rheumatoid arthritis, in two of which the diagnosis was not excluded by the morphological study of the synovial biopsy sample.

[Immunological structure of the populations of the upper and middle Volga River regions in relation to the causative agent of yersiniosis and its determining factors].

The study of the immunological structure of the population in respect of intestinal yersiniosis at territories with different hydrothermal conditions has revealed the presence of a greater immune stratum (23.9% with the average agglutination titer being 1.8 +/- 0.09 log D) in humid areas, such as the region of the Upper Volga, in comparison with dry areas, such as the region of the Middle Volga, where the level of the immune stratum has proved to be twice as low (10.6% with the average agglutination titer being 0.8 +/- 0.1 log D). At both territories no essential differences in the risk of infection between the inhabitants of rural and urban areas, as well as between different professional and age groups have been revealed, with the exception of persons over 51 years of age, among whom the occurrence of seroconversions has proved to be lower.

[Agricultural animals as a source of yersiniosis].

The role of agricultural animals as the sources of infection was studied. 8 Yersinia strains were isolated from 83 samples taken from cattle. Yersinia were isolated from feces, udder washings and milk. In the examination of 63 samples obtained from sheep the infective agent was isolated from feces in 6 cases. 6 Yersinia strains were isolated from 92 fecal samples taken from pigs. Y. enterocolitica were isolated from sick animals. The isolated strains were classified with serovars 03, 058, 08 and 09. In the foci of animal infection thus revealed antibodies to various Yersinia strains were detected in the blood sera of the animal-tending personnel, which indicates the professional character of the spread of this infection among humans. The infection is probably transferred by the contact and alimentary routes.

Agglutination Tests

[Effectiveness of social hygiene and organized preventive measures in dysentery in preschool institutions].

The work presents the data on the development and realization of the complex of social and hygienic, as well as organizational and preventive measures for dysentery control at institutions for children of preschool age in one of the regions of our country. This complex comprises the improvement of "epidemiological" knowledge of the staff, preventive orientation in the work of the epidemiological section of the local Sanitary Epidemiological Station, the spread of sanitary and hygienic knowledge children and in their families, the improvement of material and technical equipment of children's institutions. The complex proved to be highly effective: at institutions for children of preschool age morbidity -ate in dysentery decrease 4--5 times, and its specific falue dropped from 40% to 5.4%, the frequency of positive results given by tests for the presence of intestinal microflora in washings from various objects decreased more than 25 times. Morbidity rate at institutions for children of preschool age decreased 4--5 times as compared with that among children not attending such institutions. The above data suggest the necessity of introducing this experience in other regions of our country.

[Biological properties of strain of Yersinia enterocolitica isolated from humans].

A total of 1300 patients were examined; this number included patients suffering from food poisoning and with suspected dysentery hsopitalized at the isolation hospital; patients with diarrheal diseases under observation of the intestinal unit of the sanitary-epidemiological station, and patients with acute or exacerbation of chronic affection of the bile tracts. A total of 28 Yersinia enterocolitical strains were isolated from these patients, this constituting 2.1% of the total number os examinations. The strains isolated were typical by ciological properties and were referred to the 3rd and 9th serological and the 3rd and 4th biochemical types.
