Left-to-right shunts. (Quantification, mathematical methods and their statistical comparison).

This work describes a non-traumatic technique for quantitative determination of left-to-right shunts. Various mathematical methods to evaluate the radioisotope concentration lung curve are considered. Data were collected with a gamma camera system connected to a computer. The first results were obtained from 18 patients at the Ospedali Riuniti in Parma, Italy. All the mathematical processes are described. The three methods used were based on: 1. the count ratio (C2/C1) between two pulmonary activity concentrations, 2. the gamma function fitting, 3. decreasing exponential. A comparison of the three methods shows that the gamma function method is the most reliable. In this kind of investigation the patient's radiation dose is low enough for it to be repeated so that the course of the disease can be followed.

[Use of new osteotropic isotopes in osteoarticular diseases, with special reference to phlogistic arthropathies].

By using an osteotropic complex (99mTc pyrophosphate) scintigraphic and scintiphotographic images were obtained with a view to studying the bone changes in inflammatory arthropathies. In rheumatoid arthritis there is an increase in radioactivity next to affected joints and this phenomenon which seems to contrast with the osteoporosis of the juxta-articular regions is confirmed, since the reduced density that is observed in the radiograms is a result of the lack of equilibrium between bone formation and bone resorption, but only if these two processes are proceeding actively. The "warm" zones, which are very close to the joints, are just as easy to see in relation to inflamed joints as they are in relation to joints in which the inflammation has ceased. Not all the inflamed joints accumulate the radioactive osteotropic complex. The bone changes can occur in cases of chronic inflammatory arthropathies only if the inflammation has been present for at least several months. The images obtained with the polyphosphate correspond to those that can be obtained with other calcium-mimetic and osteotropic isotopes (Sr etc.).

[Immunoscintigraphy in the diagnosis of choroid melanoma].

This study was aimed at evaluating the role of 99mTc-labelled anti-melanoma antibodies as in vivo tumor markers in the diagnosis of uveal melanomas. Usually, neither conventional semeiological techniques nor more recent diagnostic imaging methods allow a differential diagnosis to be made of benign and malignant intra-ocular tumors. Immunoscintigraphy was employed to examine 9 patients with monolateral reduction of sight, and photopsy. All patients were previously submitted to funduscopy, fluorescein angiography, and US. Four hours after iv injection of 370 MBq of 99mTc F(ab1)2, we obtained 500,000-count images of skull, abdomen, and thorax, with a computerized gamma camera equipped with a general-purpose collimator. A SPET of the head followed the conventional examination. Our first results seem promising enough for further study to be carried out to assess immunoscintigraphy specificity in the diagnosis of uveal melanoma.
