Immunocytochemical study of S-100 protein in human pituitary adenomas.

The S-100 protein was localized by immunocytochemistry in 70 pituitary tumors including 30 prolactin, 16 growth hormone, two corticotropin and 22 non-functioning adenomas. Positive immunostaining was observed in only one case (prolactin adenoma). It is concluded that in functioning and non-functioning pituitary tumors there is no particular involvement of S-100 protein-containing cells, at least under the conditions of this study.

The immunocytochemical heterogeneity of silent pituitary adenomas.

An immunocytochemical study was performed by the indirect peroxidase method on the pituitary tumour of 37 patients with clinical and biological signs of silent adenoma. Antisera were used against human PRL, human GH, ACTH1-24, human ACTH17-39, alpha-melanocyte stimulating hormone (alpha-MSH), human beta-endorphin, alpha-subunit of hCG (hCG-alpha), and beta-subunits of human LH (LH-beta), human FSH (FSH-beta) and human TSH (TSH-beta). Immunostaining in at least 5% of the tumour cell population, with one or more antisera, was present in 13 cases; hCG-alpha immunostaining was the one most frequently observed. Combined immunostaining was found in 7 cases. Exclusive immunostaining was present in 6 cases: 4 with hCG-alpha, 1 with ACTH1-24 and 1 with TSH-beta. It is concluded that a significant number of silent pituitary adenomas show a certain secretory pattern of pituitary hormones or subunits of glycoprotein hormones as revealed by the immunocytochemistry.

[H-NMR spectroscopy of plasma cannot be used for cancer screening].

Water-suppressed proton nuclear magnetic resonance spectra were generated from plasma taken from 25 apparently healthy individuals, 103 patients with malignancies and 32 patients with benign thyroid tumors. Narrow linewidths from the lipoprotein methylene and methyl resonances were observed in only 61% of the patients with malignant tumors and in contrast to the results of Fossel et al., there was a considerable overlap of values between patients with and without malignant tumors. We studied the area of the CH2 and CH3 signals which was significantly increased in 83% of the patients with overt malignant disease. However this was also found in 20% of healthy subjects and in 47% of those with benign thyroid tumor. Lipoproteins (VLDL and chylomicrons) can reproduce these modifications. The present data suggest that considerable caution should be taken in interpreting water-suppressed proton NMR spectra for cancer detection.

Chronic hyperprolactinemia and plasma lipids in women.

Plasma total cholesterol (TC), triglycerides (TG), high-density lipoprotein-cholesterol (HDL-C), and low-density lipoprotein-cholesterol (LDL-C) were studied in 15 hyperprolactinemic women who had a prolactin (PRL) adenoma, in comparison to 15 age-matched control women. In the hyperprolactinemic group, plasma lipids were also correlated to age, excess body weight (EBW), plasma PRL, and estradiol-17 beta (E2). Plasma TC, TG, and LDL-C were similar in both hyperprolactinemic and control women, while plasma HDL-C was significantly lower (P less than 0.01) in the hyperprolactinemic group. The correlation study showed a significant negative correlation between HDL-C and EBW (r = -0.64; P less than 0.02) and a slightly significant positive correlation between TG and PRL (r = 0.54; P less than 0.05). The direct effect of PRL on plasma lipids is difficult to establish since many factors influencing lipid metabolism are altered during hyperprolactinemia.

Necrobiotic xanthogranuloma with myeloma. A case report.

Necrobiotic xanthogranuloma is a newly recognized cutaneous manifestation associated with paraproteinemia. Necrobiotic xanthogranuloma is distinctive clinically by yellowish-red nodular lesions, often ulcerated, with predilection for the periorbital area and ocular involvement. Histologically, it is characterized by the combination of inflammatory xanthogranulomas together with areas of necrobiosis. These skin lesions are associated with a dysglobulinemia and preceded the development of an IgG lambda myeloma in a previous patient. Hyperlipidemia may be found in some patients. A case of necrobiotic xanthogranuloma is reported, occurring in a 46-year-old man and associated with an IgG kappa myeloma.

Effects of induced hyperventilation on electrodermal response habituation to agoraphobia-relevant stimuli.

The role of hyperventilation in the aetiology of anxiety disorders was investigated in an analogue study. It was hypothesised that induced hyperventilation would alter subsequent subjective and physiological responses to visually presented agoraphobic material. Three groups (N = 16) of students were used and each was divided into two sub-groups which were presented with slides of either potentially agoraphobic or neutral content. During the first phase of the experiment, subjects were exposed to 10 slides whilst their breathing rate was manipulated. Group FB was instructed to hyperventilate by asking the subjects to breathe fast and deeply, Group SB was instructed to breathe at a slow rate and Group NB breathed normally. After a recovery phase, subjects were re-exposed to the same visual stimuli as used in the first phase of the experiment. Measures of skin conductance response amplitude were obtained for each stimulus in the series. In addition, heart rate and respiration were monitored, and subjective measures of somatic and affective states were obtained. Subjects also rated the pleasantness of the stimuli. The hyperventilation instructions resulted in delayed electrodermal habituation relative to the other groups. This finding could not be accounted for in terms of group differences in either physiological or subjective 'arousal'.

[Results of prevention trials by intervention with lipids].

When only statistically comparable studies are taken into account, there are three primary prevention and eight secondary prevention studies. In 9 out these 11 studies the calculated decrease in the incidence of coronary disease are in favour of a beneficial effect of the lipid-lowering treatment. Some studies suggest that femoral atherosclerosis is receding and that coronary atherosclerosis is stable or progresses more slowly, but this always provided the plasma lipids are significantly and durably reduced. Subjects at high cardiovascular risk, therefore, must be treated, but one should now proceed even further, since the decrease of total cholesterol in the general population is paralleled by a decrease of coronary disease. This is in keeping with the results of extensive epidemiological surveys (notably the Framingham survey) which show that the lower the total cholesterol level the brighter the cardiovascular prognosis.

[Isolated secretion of testosterone by an adrenocortical tumor in a menopausal woman].

A case of adrenal tumor with isolated secretion of testosterone in a menopaused woman is reported. All other androgens (Déhydro-épi-androstérone, Sulfate de Déhydro-épi-androstérone, Delta 4 androstène-dione) were normal as well as other adrenal hormones. This tumor produced a mild hirsutism. A treatment with Cyproterone Acetate, then with Dexamethasone did not produce a significant change. A complete review of literature points out the rarity of this observation, since only twelve similar cases have been previously reported.

A study of the structural heterogeneity of low-density lipoproteins in two patients homozygous for familial hypercholesterolaemia, one of phenotype E2/2.

The structural heterogeneity of the low-density lipoproteins (d 1.019-1.063 g ml-1) in two female patients homozygous for familial hypercholesterolaemia, one of phenotype E2/2, has been evaluated using a new ultracentrifugal density gradient procedure. The mass distribution, chemical composition, particle size and heterogeneity, hydrated density and apolipoprotein content of 16 LDL subfractions were determined. By gradient gel electrophoresis, the lighter LDL subfractions (d 1.016-1.037 g ml-1) displayed a single particle species which progressively diminished in size from 24.8 to 22.0 nm with increase in density. By contrast, subfractions of higher density (d greater than 1.037 g ml-1) exhibited two LDL particle species of distinct size; one component decreased in size from 21.8 to 20.4 nm with increase in density, while the second maintained an essentially constant diameter (between 22.5 and 23.5 nm) across these LDL subfractions. Immunoblotting with anti-apo-B100 of LDL subspecies separated by gradient gel electrophoresis showed all particles to contain apo-B100. However, dot-blots and immunoblotting with a monoclonal antibody to lipoprotein (a) (Lp(a)) revealed that the LDL particle subspecies of greatest diameter (22.5-23.5 nm) present in the denser subfractions (d greater than 1.037 g ml-1) also contained the Lp(a) antigen. These findings, taken together with the high plasma Lp(a) levels (greater than 60 mg dl-1) in our patients, raise the possibility that Lp(a) may contribute in a significant manner to the atherogenic process in homozygous familial hypercholesterolaemia.
