[Lymphadenosis benigna cutis of the mammary areola: Lyme borreliosis?].

This lesion began two months after a tick bite on the left breast, occurring during a trip in Karst (highland behind the city of Trieste). This area represents an ecological shell for Ixodes ricinus, hard tick often infested with Borrelia burgdorferi, the aetiological agent of Lyme disease. Some days after tick-bite, the patient developed a typical ECM of the left breast; this lesion resolved in two months. Shortly after, an erythematous, infiltrated nodule appeared on the left areola; ist size was about two cm. Histological diagnosis confirmed that the lesion was a LABC; the lymphocytic infiltrate consisted in a large number of B cell (detected with monoclonal antibodies). Indirect immunofluorescence for Borrelia burgdorferi (after absorption with Treponema phagedenis) was positive (titre 1/128-IgG). The lesion was treated with doxycycline, 200 mg/day per os for two weeks. The lesion cleared up three months after the end of therapy. Till today (two year thereafter), the patient didn't present with local or general signs of Lyme borreliosis; furthermore the serological test for Borrelia burgdorferi became negative.

[Thymopentin in the treatment of cutaneous melanoma].

Thirty-three patients were treated with thymopentin (synthetic thymic-hormone) after complete surgical removal of cutaneous malignant melanoma without clinical evidence of metastases and with evidence of immunocellular deficiency, with the aim of obtaining a normalization of the immunological parameters. The cell-mediated immunity was evaluated by utilizing monoclonal antibodies CD3, CD4, CD8, CD21, NK and the Multitest C.M.I. (Institut Merieux-Lyon-France), before treatment and then every three months. A follow-up at 29 months showed an improvement of the immune parameters: respectively 58%, 67% and 42% of the patients undergoing therapy had an increase of 40% of the CD3, CD4 and NK lymphocytes. Seven patients, with pre-existing hypergy following tests of skin-reactions, present a normalization of this parameter during the treatment. In the follow-up three patients had metastases. The results show the improvement of the considered immunological parameters and low percentage of relapses. It may be considered as a preventive measure for immunological control of relapses.

Lyme disease in Italy.

We present the first cases of Lyme disease found in Italy. The diagnosis was based on clinical and laboratory data. The antigen used for indirect immunofluorescence (I.I.F.) was kindly supplied to us by Prof. R. Ackermann (Köln). Reciprocal titer was 64 in five patients, 128 in six, 256 in three and 512 in one. The patients came either from the Eastern Ligurian Coast or the Trieste Karst: these are consequently the first two Italian areas where Lyme disease has been recognized up to the present.

Xeroderma pigmentosum (complementation group D) mutation is present in patients affected by trichothiodystrophy with photosensitivity.

We studied the response to UV irradiation in cells from four patients, from three apparently unrelated families, affected by trichothiodystrophy (TTD). They showed all the symptoms of this rare autosomal recessive disorder (brittle hair with reduced sulfur content, mental and physical retardation, ichthyosis, peculiar face) together with photosensitivity. We found a decreased rate of duplicative DNA synthesis in stimulated lymphocytes, reduced survival in fibroblasts, and very low levels of unscheduled DNA synthesis (UDS) in Go lymphocytes and fibroblasts after UV irradiation. Complementation studies showed that normal values of UDS are restored in heterokaryons obtained by fusion of TTD cells with normal and xeroderma pigmentosum (XP)-complementation group A-cells. In contrast the defect is not complemented by fusion with XP-complementation group D-fibroblasts.

Bilateral follicular basal cell nevus with comedo-like lesions.

A 12-year-old white boy presented with symmetrically appearing papular, nodular, comedo-like, highly itching lesions involving the skin on his ankles. They had first appeared 3 years earlier. Numerous cutaneous biopsies showed tumoral buds of basalioma cells associated with follicular structures, infundibular cysts and also hypertrophy and hyperplasia of sebaceous glands. To the best of our knowledge, there are no reported cases with the same clinical and histopathological findings as in those reported here. We believe the lesions represent a "bilateral follicular basal cell nevus with comedones".

An analysis of T-lymphocyte subpopulations in psoriasis using monoclonal antibodies.

We evaluated subpopulations of T-lymphocytes by using the monoclonal antibodies OKT3, OKT4 and OKT8 in two groups of psoriatic patients, with active and stable psoriasis respectively. Whereas data from the patients with stable psoriasis were similar to those obtained from the control group, the patients in an acute flare condition revealed a relative decrease in lymphocytes reactive with OKT8, and a slight increase in the proportion of lymphocytes reactive with OKT4. The results of this study are analysed.

Morphological microcirculatory aspects in chronic uremia.

A fingertip biopsy was made in 18 patients affected by chronic uremia, in order to study the histangic changes. Partial occlusion of the blocking devices, capillary dilation and regressive changes in sensitive corpuscles, especially in Vater-Pacinian ones, were observed. The incidence of dermal necrosis and of calcium deposits was also high.

Correlations between cutaneous and retinal microaneurysms in diabetes mellitus (preliminary note).

A group of diabetic patients was submitted to a capillaroscopic examination at the level of the peri-ungual vallum and to a fluoroangiographic examination of the retina, for the evaluation of possible correlations between vessel conditions in to various districts. The Authors remarked that the subjects affected by proliferative diabetic retinopathy presented a greater dilatation of the ansae at cutaneous level, with haemorrhagic extravasations in 70% of the cases. On the contrary, on the subjects with nonproliferative diabetic retinopathy numerous aneurysmal "buttons" were noted at cutaneous level, both apically and laterally along the vascular ansa.
