Impetigo herpetiformis and RhE isoimmunization: a case report.

Impetigo herpetiformis is a rare acute eruption associated with severe systemic disturbances, typically occurring in pregnant women. We describe the case of a 33-year-old woman who presented with impetigo herpetiformis during her third pregnancy, the appearance of which coincided with an increase in the titer of E antigen antibodies.

In vitro activity of rokitamycin, a new macrolide, against Borrelia burgdorferi.

The activity of rokitamycin, a new macrolide with a 16-member ring, was tested against Borrelia burgdorferi in vitro. The antibiotic had a lower MIC at which 50% of the isolates are inhibited than erythromycin, the parent 14-member macrolide, but the same MIC at which 50% of the isolates are inhibited as the other recent 14- and 15-member macrolides, like clarithromycin and azithromycin. The MBC was equal to the MIC at which 50% of the isolates are inhibited, so rokitamycin can be considered bactericidal against B. burgdorferi. The sensitivity of the Borrelia strains tested was not correlated with the particular species Burgdorferi sensu stricto, B. garinii, and B. afzelii or with the number of subcultures of the isolates.

[Threatened paradoxical embolism: its direct visualization by two-dimensional echocardiography].

We describe the case of a 67-year-old patient, who had a stroke with subsequent right hemiparesis two years before coming under our observation. Afterwards he had no more pathological manifestation until he had sudden dyspnoea and marked cyanosis, following a prolonged immobilization due to intervention for slipped disc. Symptomatology regressed rapidly, and few hours later, the patient came under our observation in relatively fair conditions. Echocardiography evidenced moderate enlargement of right heart cavities; by subcostal view we visualized the presence of thrombotic material crossing the foramen ovale. Color Doppler showed moderate tricuspid regurgitation. Pulmonary artery systolic pressure was estimated about 55 mm Hg. The patient was immediately anticoagulated firstly by heparin, and secondly by warfarin, maintaining good clinical conditions. After a treatment of two more weeks he could be discharged with prescription of indefinite anticoagulation. Before discharge an echo examination showed the thrombus was no longer present. Pulmonary systolic pressure was estimated about 40 mm Hg. This is one of the rare cases of direct visualization of impending paradoxical embolus documented in the cardiologic literature.

Use of polymerase chain reaction and specific monoclonal antibodies as rapid method to recognize Borrelia burgdorferi sensu stricto, B. garinii and B. afzelii among Italian isolates of B. burgdorferi.

We previously classified locally isolated strains of Borrelia burgdorferi by a restriction fragment length polymorphism analysis of total DNA, by DNA/DNA Southern Blot hybridization and by a hybridization with rRNA 16 + 23S from Escherichia coli [Cinco et al. (1993) Microbiologica 16:323-332] into three genetic groups which, according to the reference strains used, should correspond to the three species so far described as B. burgdorferi sensu stricto, B. garinii and B. afzelii. To find a simpler method for strain identification, in this study we analyzed the Italian strains and some strains identification, in this study we analyzed the Italian strains and some strains originating from other European countries, employing the species-specific 16S rRNA primers in the polymerase chain reaction technique (PCR) and some phenotypic markers like the B. afzelii-specific monoclonal antibodies and the battery of OspA-specific monoclonal antibodies which were reported to give a reactivity pattern correlated to the species [Wilske et al. (1993) J Clin Microbiol 31:340-350]. The PCR results confirmed those obtained previously by identifying the three groups as B. burgdorferi sensu stricto, B. garinii and B. afzelii; the reactivity patterns obtained with the monoclonal antibodies (mAb) also corresponded to those described as typical of the three species. We standardized the PCR technique to amplify a sample of crude template DNA obtained from a culture of 10(5) spirochetes.

Lyme borreliosis.

Lyme borreliosis is a cutaneous-systemic infection that is generally transmitted by a hard-bodied tick and caused by some species of Borrelia, Borrelia burgdorferi (sensu stricto), B. garini, and B. afzelii. Lyme borreliosis is a widespread disease, present in all continents. It can be divided into an early disease, corresponding to the primary stage, and a late disease, including the secondary and tertiary stages. The typical cutaneous lesions are erythema (chronicum) migrans, lymphadenosis benigna cutis, and acrodermatitis chronica atrophicans. The course of Lyme borreliosis depends on the timeliness of both diagnosis and antibiotic treatment.

The nails in psoriatic arthritis.

Nail involvement is often present in patients suffering from psoriatic arthritis. Up to the present, no data concerning the rate of onychopathy associated with psoriatic arthropathy have been reported. 52 patients with psoriatic arthropathy have been studied with particular attention to the possible presence of and the typical features of onychopathy. Nail changes were noted in 86.5% of patients affected by arthropathic psoriasis. The commonest toenail alteration was subungual hyperkeratosis, while the most frequent fingernail change was pitting.

Psoriasis and endothelins.

BACKGROUND: Psoriasis is characterized by an abnormal proliferation and increased turnover of keratinocytes, the presence of acute and chronic inflammatory cells and microangiopathic changes. Endothelins are a family of peptides which have been investigated especially for their effects on the cardiovascular system. Recent studies have demonstrated their involvement also in human skin. AIM OF THE STUDY: We evaluated the Endothelin-1 and 2 plasma levels in psoriatic patients, as endothelin-1 can be produced in vitro by keratinocytes and can stimulate the proliferation of fibroblasts as well as modify the skin microcirculation dynamics. PATIENTS AND METHODS: We studied 30 patients: 10 affected with psoriasis (PASI from 5 to 10), 10 affected with cardiovascular diseases and 10 healthy controls. The Endothelin-1 and 2 plasma levels were evaluated by radio-immunoassay procedure. RESULTS: A significant increase in Endothelin-1 and 2 plasma levels was observed in the psoriatic patients, in comparison with the controls. CONCLUSIONS: Our data seem to suggest a possible relationship between psoriasis and increased plasma level of endothelin-1 and 2, though the possible role played in the pathogenesis of psoriasis needs further studies.

DNA repair investigations in nine Italian patients affected by trichothiodystrophy.

Trichothiodystrophy (TTD) is a rare autosomal recessive disorder characterized by brittle hair, mental and growth retardation, peculiar face, ichthyosis, and in 20% of the reported cases photosensitivity. Cellular photosensitivity due to the same genetic defect present in xeroderma pigmentosum group D (XP-D) has been described in several patients. Nine patients with clinical symptoms diagnostic for TTD have been identified in Italy to date. We report the results of DNA repair investigations performed in cultured fibroblasts from these patients and 8 TTD parents. Survival, DNA repair synthesis and RNA synthesis following UV irradiation were all normal in the 8 TTD heterozygous cell strains. Among the 9 TTD-affected individuals, normal cellular UV sensitivity was observed in the 2 patients without signs of clinical photosensitivity. In contrast, the other 7 TTD cell strains showed a notable reduction in UV-induced DNA repair synthesis (UDS) levels, ranging between 40% and 5-15% of normal values. Complementation analysis indicated that in the repair-deficient TTD cell strains the genetic defect is the same as that present in XP-D cells. The biochemical heterogeneity of the XP-D defect in TTD patients characterized by different degrees of defective UDS results in different patterns of response to the killing effect of UV light in non-proliferating cells.

Isolation of Borrelia burgdorferi from a Lyme seronegative patient in northern Italy: expression of OspB immunodominant proteins on the isolated strain.

This paper describes the antigenic characteristics of a strain of Borrelia burgdorferi isolated from a patient seronegative for Lyme borreliosis, in the early stage of the illness. The strain was not recognized by a late serum sample from the patient; the isolate reacted in immunoblotting with some of the monoclonal antibodies directed against the immunodominant antigens of Borrelia burgdorferi. In addition to the OspA antigen this strain carries also the epitopes of OspB proteins, unlike the majority of European Borrelia burgdorferi strains.

Articular involvement in European patients with Lyme disease. A report of 32 Italian patients.

Lyme disease (LD) is a well recognized multisystem disorder, caused by the spirochaete Borrelia burgdorferi. It involves the skin, nervous system and heart. Arthritis is described in 50-60% of American patients but seems to be less common in Europe. We studied the pattern of articular involvement in a group of 67 Italian patients with LD. Thirty-two (48%) of 67 had rheumatological manifestations. The prevalence of arthritis in our patients was 16/67 (24%), the remaining 16 experiencing only arthralgia. The pattern of articular involvement did not differ from the literature and was most often mono- or oligoarthritis with polyarticular arthralgia. We noted an early onset of articular symptoms (range 1-3 months). Incidence of early and late manifestations of LD were in keeping with previous reports. In addition, interpreting serological tests for antibodies against B. burgdorferi and the real prevalence of arthritis in LD is complicated by the possible existence of seronegative LD and by the effect of early antibiotic treatment.

Psoriatic microangiopathy modifications induced by PUVA and etretinate therapy. A nail-fold capillary microscopic study.

Histological and capillaroscopy alterations of capillary circulation in psoriasis are well known: the most common of them are the increase of "minor dystrophies", the higher haematic flux in the psoriatic plaques and the abnormal visibility of the venous subpapillary plexus (in arthropathic less than 65-year-old patients). The predisposition of psoriatic patients to a vascular damage has often been stressed in literature. In the present study, we have evaluated the effects of systemic etretinate or PUVA therapy on capillary functions. We examined the nail-folds of 30 patients (ranging 15-64 years) affected by diffuse psoriasis for less than three years. This study has been performed by using a Leitz capillary microscopy. We evaluated the percentage of capillary minor dystrophies (type I-IV) and the flow rate before and after 1 month of treatment. These parameters were assessed by means of a 1-4 rating scale. Our results underline that a significant percentage of 1st type dystrophies is a prognostic rather than a diagnostic parameter in the capillaroscopic evaluation of psoriasis.
