Topical treatment of periocular basal cell epithelioma with solcoderm.

25 patients with 29 histologically confirmed periocular basal cell epitheliomas were treated by topical application of Solcoderm, a solution of organic and inorganic acids and ions. The cure rate of the 29 lesions, which were followed for a mean period of 32 months, was 80%. The overall cure rate following second Solcoderm treatment of the 6 recurrent lesions was 96.7%. The cosmetic and functional results were excellent and there were no serious complications and side effects.

Eye injuries and ocular protection in the Lebanon War, 1982.

There was a high incidence of ocular trauma (6.8% of all casualties) in the Lebanon War, 1982, which is similar to the rates in previous Arab-Israeli wars. Combat in the Lebanon War consisted of both armored warfare and infantry operations, with much of the fighting taking place in built-up areas. Consequently, there were two main types of ocular injuries: perforating eye injuries caused by the small high-velocity missiles that characterize armored combat, and a wide range of injuries to the eye caused by ricochets during combat in built-up areas. The latter injuries varied in severity from complete destruction of the eye to minor corneal erosions. Not a single eye was injured in soldiers who had ballistic protective goggles properly placed over the eyes at the time of injury. Most of the ocular trauma could have been prevented had the goggles been in universal use. A considerable degree of eye protection is also afforded by ordinary ophthalmic eyeglasses.

Severe combat eye injuries in the Lebanon War, 1982.

The findings, treatment and follow-up of 21 severe, blunt and perforating ocular injuries that occurred during combat in the Lebanon War, 1982, are presented. The diagnostic and therapeutic approaches to complex and severe ocular injuries are discussed. The patients were followed up for 1 year and the long-term effects of the injuries on their eyesight are described.

Corneal abscesses in silicone and soft contact lens wearers.

Fourteen cases of corneal abscesses in wearers of contact lenses were seen in our department during the last 3 years. The hospitalization period varied from a few days to seven weeks. In one case, it was necessary to perform penetrating keratoplasty in order to restore good vision. Cultures taken from the corneal abscesses were positive in six cases. Pseudomonas was found in five cases and Enterobacter in one case. During the three years of the survey, no hard contact lens wearers with corneal abscesses were hospitalized. Wearers of silicone contact lenses comprised 21.4% of the cases with corneal abscess. This is a high percentage considering the fact that these lenses are very rare among contact lens wearers in Israel.

Pars plana vitrectomy for vitreous hemorrhage and retinal vein occlusion.

We studied 16 cases of retinal vein occlusion and vitreous hemorrhage treated by pars plana vitrectomy. The visual acuity in all nine eyes suffering from branch vein occlusion was improved postoperatively, reaching an acuity of 6/6 to 6/12 in 33% of the patients. In the seven eyes suffering from central vein occlusion, however, the postoperative visual acuity results were poor. The incidence of postoperative complications was higher also in the central vein occlusion group.

Anterior uveitis as a first sign of primary orbital lymphosarcoma.

A 47-year-old woman had a painless anterior uveitis in her right eye. Later, it appeared to be secondary to a developing extraocular orbital tumor, which also caused marked proptosis. Histopathologic examination of the tumor revealed lymphocytic lymphosarcoma. The tumor regressed rapidly after irradiation and cytological treatment.

Combined pars plana vitrectomy and retinal surgery in massive periretinal proliferation.

Thirty-two patients with retinal detachment and massive periretinal proliferation underwent combined vitreous and retinal surgery. The cases were divided into two groups: 18 patients primarily operated on and 14 patients with a history of previous unsuccessful retinal surgery. Successful reattachment was accomplished in 13 eyes of the first group (70%), with a mean follow-up period of 8.5 +/- 1.9 months. In the second group, reattachment was accomplished in seven eyes (50%) followed for 11.4 +/- 2.3 months. Visual improvement was achieved in 61% and 29% of eyes in the first and second groups, respectively. These results indicate that eyes with retinal detachment and massive periretinal proliferation following retinal surgery have a much worse prognosis than patients who were not previously operated on.

The penetration of gentamicin into the vitreous humor in man.

Twenty-two patients received gentamicin intramuscularly or subconjunctivally 35-220 min prior to undergoing ocular surgery. There were no detectable gentamicin levels in the vitreous humor of patients who received the drug systemically. Of the patients who received gentamicin subconjunctivally, three quarters had no detectable gentamicin levels, only three of these patients had therapeutic concentrations in their vitreous humor. These results confirm kinetic drug studies performed in animals in which low or absent vitreal gentamicin levels were observed following systemic and subconjunctival administrations. It is suggested that intravitreal injection of aminoglycosides is required in the treatment of bacterial endophthalmitis.

Effect of subconjunctival injection of hyaluronidase on outflow resistance in normal and in open-angle glaucomatous patients.

The effect of subconjunctivally injected hyaluronidase on ocular hydrodynamics was tested in 30 living human normal and 30 open-angle glaucoma eyes. It was found that after 1,500 TU of hyaluronidase injected subconjunctivally, the outflow resistance was significantly lowered: 34% in normal eyes, on the average; and by about 57% in the glaucomatous eyes. The possibility of either a higher amount of hyaluronic acid in the glaucomatous eyes or a normal amount of hyaluronic acid in the presence of compromised trabeculum is discussed. Clinical applications and further research are advocated.

Operative difficulties in removal of retained intraocular foreign bodies by vitreous surgery.

The presentation deals with operative difficulties during vitreous surgery for the removal of long-retained intraocular foreign bodies (IOFBs). The cases described and the problems discussed are: (1) IOFBs embedded deeply in dense fibrous tissue which cannot be exposed and removed, (2) IOFBs located in a 'dead space' at the anterior pars plana and ciliary body regions, (3) readherence of a residual stump causing traction retinal detachment, and (4) abnormal hyperviscosity of the vitreous in perforation injuries with long-standing retained IOFBs, without vitreous hemorrhage. The presentation shows that an envelope of dense fibrous tissue does not prevent the spread of metallic ions. It emphasizes that an early surgery, before fibrous organization develops, is imperative.

Effect of aceclidine(+) isomer and pilocarpine on the intraocular pressure decrease and the miosis in glaucomatous eyes. Effect on accommodation in normal eyes of young subjects.

Previous studies demonstrated that the (+) isomer of aceclidine has a stronger cholinergic effect than pilocarpine or the racemic mixture of aceclidine. The effect of aceclidine(+) and pilocarpine on the intraocular pressure and the sphincter pupillae in patients suffering from open angle glaucoma, and on the accommodation in normal young subjects, was studied. The results demonstrated a similar effect of both drugs. The discrepancy between the cholinergic potential of the drugs and their effect in the eye is discussed.

Pars plana surgical approach for various anterior segment problems.

Various anterior segment problems can be approached through the pars plana and treated with instrumentation that has been developed for vitreous surgery. Two cases of aphakic bullous keratopathy due to vitreous touch and three cases of subluxated lenses were successfully treated. Of two cases of advanced, persistent hyperplastic primary vitreous, one showed a successful result.

Falciform fold of retina.

A boy of 2 had evidence of congenital retinal non-attachment. The mother had a veil like fold in one eye. There was no evidence of any systemic disorder. The inheritance might be X-linked recessive. Mild manifestations of the disease in the carrier mother may be more frequent than has been assumed and should be looked for.
