Benign gastric ulcer: a reliable radiologic diagnosis?

A review of 7,600 double-contrast barium-meal studies performed between 1975 and 1981 revealed 221 gastric ulcers reported as benign. The follow-up for 199 of these was adequate and detected no case of carcinoma. The widespread practice of immediate endoscopy and biopsy for all cases of radiologically benign gastric ulcer represents a misuse of expensive medical resources. When a gastric ulcer is demonstrated clearly as benign on double-contrast examination, there is no need for endoscopic confirmation of the diagnosis.

Getting the most from word processing.

Users of word processing in healthcare organizations do not get the productivity they should with today's word processing technology. The technology performs well, better each year, but healthcare users haven't organized their operations to maximize productivity. Out-of-date ideas about office work, lack of organized direction in the use of word processing technology and a continuing resistance to change all contribute to the loss of potential productivity increases from word processing in healthcare organizations.

Opportunistic gastritis in graft-versus-host disease.

Two patients underwent bone marrow transplantation and subsequently developed graft-versus-host disease. Barium examination showed a gross nodular pattern in the stomach in one patient, an appearance not previously described. Both patients developed characteristic changes in the small bowel and in one the colon was also affected. The dramatic rise in the patient's cytomegaloviral titers (CMV) and the presence of viral inclusion bodies in gastric cells suggest that the changes in the stomach may be due to a CMV gastritis.

Distribution of gastric ulcers by double contrast barium meal with endoscopic correlation.

The double contrast barium meals and endoscopic examinations of 246 patients with 264 benign gastric ulcers were reviewed with regard to the location of these ulcers. Seventy-four per cent of ulcers were found in the antral portion of the stomach within 7 cm of the pyloric canal. A similar study in the United Kingdom revealed that the majority of ulcers were in the upper body of the stomach. Since true gastric ulcers almost always arise in a pyloric type of mucosa, or in the zone of transition from pyloric to body type mucosa, perhaps this junction zone lies higher in a United Kingdom population than in North Americans. Possibly the higher transition zone and the documented higher frequency of gastric malignancy in the United Kingdom are related.

Cerebrospinal fluid flow abnormalities in patients with neoplastic meningitis. An evaluation using 111indium-DTPA ventriculography.

Cerebrospinal fluid flow dynamics were evaluated by 111Indium-diethylenetriamine pentaacetic acid (111In-DTPA) ventriculography in 27 patients with neoplastic meningitis. Nineteen patients (70 percent) had evidence of cerebrospinal fluid flow disturbances. These occurred as ventricular outlet obstructions, abnormalities of flow in the spinal canal, or flow distrubances over the cortical convexities. Tumor histology, physical examination, cerebrospinal fluid analysis, myelograms, and computerized axial tomographic scans were not sufficient to predict cerebrospinal fluid flow patterns. These data indicate that cerebrospinal fluid flow abnormalities are common in patients with neoplastic meningitis and that 111In-DTPA cerebrospinal fluid flow imaging is useful in characterizing these abnormalities. This technique provides insight into the distribution of intraventricularly administered chemotherapy and may provide explanations for treatment failure and drug-induced neurotoxicity in patients with neoplastic meningitis.

Identification of factors of affective meaning in four selected activities.

The premise that activities have inherent meanings is basic to the theory and practice of occupational therapy. In this study the investigators hypothesized that different activities would elicit different kinds of affective meanings in their participants. The subjects in this study were 59 students beginning their training in occupational therapy. On their first day in an activities class, and before instruction in activity analysis, the subjects were presented with four selected activities. Immediately after performing each activity, the students were requested to rate the activity according to Osgood's 12-scale short-form semantic differential. The data generated were reduced subsequently to Osgood's three factors of affective meaning: "evaluation," power," and "activity" (in this study called action). Results indicated that the four activities elicited significantly different responses on all three factors. Specific results and implications of this study's methodology for future occupational therapy research are discussed.

CSF infections complicating the management of neoplastic meningitis. Clinical features and results of therapy.

This study analyzes the clinical and bacteriologic features of CSF infections that occurred in patients with neoplastic meningitis treated with thiotepa and methotrexate administered through a subcutaneous reservoir and ventricular catheter (SRVC). Thirty-one patients were treated, and CSF infections occurred in four (13%). Staphylococcus epidermidis was the infecting organism in each case and Pseudomonas maltophilia occurred with S epidermidis in one patient. Fever, headache, lethargy, and evidence of CSF extravasation around the SRVC were the common manifestations of infection. The CSF leukocytosis was the only laboratory abnormality noted. All infections were cured with the appropriate antibiotics and removal of the SRVC. Risk of CSF infection did not seem to be related to the use of high doses of dexamethasone, cranial radiation therapy, or the presence or absence of leukopenia. The SRVCs were replaced and treatment of neoplastic meningitis was resumed in three patients; infection did not recur. A CSF infection during management of neoplastic meningitis may be treated effectively and does not preclude adequate therapy of neoplastic meningitis.

Treatment of neoplastic meningitis with intraventricular thiotepa and methotrexate.

Twenty-five patients with involvement of the meninges by disseminated cancers were treated weekly with intraventricular thiotepa (10 mg) and methotrexate (10 mg) and with radiation therapy. Twenty-two of the 25 patients were clinically improved or stable. Malignant cells were cleared from the cerebrospinal fluid in 76% of the patients. Myelosuppression was the dose-limiting toxic effect, and neurotoxicity occurred in three patients. Combination chemotherapy employing thiotepa and methotrexate is a safe and effective treatment for neoplastic meningitis.

OT-CLL: a human T cell chronic lymphocytic leukemia that produces IL 2 in high titer.

This report identifies and describes a human T cell chronic lymphocytic leukemia, OT-CLL, which can be triggered by selected mitogens (either PHA or Con A) to produce IL 2 in high titer. Optimal IL 2 production requires culturing OT-CLL cells at 2 to 5 X 10(6)/ml for 24 hr in the presence of 1 to 2% PHA-M. Under these conditions, the titer of IL 2 generated is greater than 20-fold that obtained from conventional sources, e.g., from mitogen-activated tonsillar lymphocytes. Two lines of experimental evidence suggest that the tumor cell product(s) is IL 2. First, in functional assays, suprenatants derived from cultures of PHA-activated OT-CLL cells trigger the proliferation and long-term growth of IL 2-dependent human TCL cells. Second, a partial biochemical purification of the active moiety(ies) derived from OT-CLL demonstrates marked similarity to conventional human IL 2. Thus, the biologically active material(s) precipitates in 50 to 70% saturated (NH4)2 SO4 solutions; elutes from DEAE-Sepharose in the presence of 0.04-0.08 M NaCl; and has an apparent m.w. of approximately 14,000, as determined by Sephadex G-100 gel filtration. In addition, analysis of OT-CLL cells by indirect immunofluorescence, utilizing a panel of monoclonal antibodies, confirms not only that these tumor cells are of T cell lineage but that they display surface antigens that define the normal human peripheral T cell subset subserving helper or inducer function: OKT3+, OKT4+ , OKT8-.

Radiologically assisted endoscopic papillotomy.

Endoscopic sphincterotomy is now established as a safe technique for the treatment of common bile duct stones or papillary stenosis. The procedure is occasionally unsuccessful because of difficulty in cannulation. Two patients are described in whom the presence of a transbiliary drainage catheter facilitated endoscopic sphincterotomy. With such radiologic assistance, there should be few patients in whom the procedure cannot be accomplished.
