Progesterone and estradiol in saliva after in vitro fertilization and embryo transfer.

The importance of the luteal function to ensure adequate implantation prompted us to assay salivary estradiol (SE2) and salivary progesterone (SP) concentration serially after embryo transfer by simple and rapid radioimmunoassays to allow a noninvasive easily repeatable approach. SE2, which has fallen 1 day after human chorionic gonadotropin injection, increased after reimplantation (day 3), plateaued during approximately 1 week, and returned to basal levels before menstruation in absence of conception or in chemical pregnancy, whereas it increased steadily in successful pregnancy. SP increased in all the cases, formed a plateau, and returned progressively to basal levels before menstruation, whereas in ongoing pregnancy there was a transitory decline in the midluteal phase followed by a rapid and continuous increase, with a significantly higher ratio day 13/day 10 in successful pregnancy.

Transient hypoparathyroidism induced by synthetic human parathyroid hormone-(1-34) treatment.

Daily injections of low doses of a synthetic fragment of human PTH [hPTH-(1-34) have increased iliac trabecular bone volume when used in the treatment of osteoporosis. In approximately 50 patients no major side-effects had occurred. However, during daily sc 100-micrograms injections of the peptide, one patient repeatedly developed parathyroid hypofunction which resolved each time treatment was stopped. Specific immunoglobulin G (IgG) antibodies binding [125I]hPTH-(1-34) were identified in the patient's serum, and positive immunohistochemical reactions were obtained when bovine parathyroid sections were exposed to the patient's IgG. After adsorption with PTH, the patient's IgG, free of anti-PTH antibodies, reacted with renal cell membranes, as demonstrated by indirect immunofluorescence and blocked renal PTH-dependent adenylate-cyclase activation in vitro. These results support the hypothesis that anti-PTH receptor as well as anti-PTH antibodies were generated during hPTH-(1-34) treatment, which led to the development of hypoparathyroidism when their titers were high.

Influence of the oral administration of micronized progesterone on plasma and tissue levels of steroids in human pregnancy.

A single dose of micronized progesterone, orally administered to women at different stages of pregnancy, induced an immediate increase in the plasma levels of progesterone. Progesterone was administered to another group of women just before elective cesarean section. Levels of progesterone, oestradiol-17 beta and oestrone were determined in plasma, placenta and at different sites of the myometrium obtained during the surgical procedure. Results were compared to those observed in a control group of women who did not receive progesterone. Progesterone levels demonstrated a marked increase in plasma and in the whole myometrium 150 min after administration; the levels then decreased rapidly to control values within one hour. The concentrations of progesterone in the placenta did not show any change. Evident modifications of the oestrogen levels appeared in the myometrium and in the placenta after the administration of progesterone whereas no change occurred in the plasma. No difference was observed in estradiol-17 beta in the myometrium whereas an increase was seen in the placenta. Estrone levels decreased in the myometrium and in the placenta. This study demonstrates a prompt delivery of an orally administered natural progesterone to the myometrial target tissue and subsequent modifications in the estrogen pattern. This provides a theoretical mechanism for the clinical use of progesterone in the prevention of premature labor.

Oral administration of micronized natural progesterone in late human pregnancy. Effects on progesterone and estrogen concentrations in the plasma, placenta, and myometrium.

A single dose of micronized oral progesterone was administered to 15 pregnant women immediately prior to elective cesarean section. Levels of progesterone, 17 beta-estradiol, and estrone were measured in the plasma, in the placenta, and at different sites in myometrium obtained during the surgical procedure. Results were compared to those observed in a control group of women who did not receive progesterone. Progesterone levels demonstrated a marked increase in plasma and in the whole myometrium 150 minutes after administration. The levels then decreased rapidly to control values in 1 hour. The concentrations of progesterone in the placenta did not show any changes. No difference appeared in 17 beta-estradiol levels in the plasma or the myometrium, whereas an increase was observed in the placenta. Estrone levels did not change in the plasma, but they decreased in the myometrium and in the placenta.

Radioimmunoassay of plasma and salivary progesterone during the menstrual cycle.

We have developed specific and sensitive radioimmunoassays for progesterone in saliva (SP4) easily applicable in routine practice to assess ovarian function. One of them uses [3H]progesterone, the other 3-CMO-[125I]histamine as tracer. The reliability criteria of both techniques have been controlled and the results obtained with the two methods in 50 saliva samples are correlated well. Our clinical study has been chiefly done with [3H]progesterone. In the present study we reported the values obtained daily during the greatest part of the menstrual cycle in 14 normal cases and they are in agreement with previous reports. The use of [125I]tracer, by increasing the sensitivity of the RIA and shortening the duration of the assay, improves the use of SP4 as a practical non-invasive sampling alternative for many clinical situations involving evaluation of the ovarian function.

The "dysharmonic luteal phase" syndrome: endometrial progesterone receptor and estradiol dehydrogenase.

The dysharmonic luteal phase (DLP) syndrome is defined by delayed endometrial maturation despite normal plasma progesterone (P) values. In ten patients with DLP the actual date of the endometrial biopsy, dated retrospectively, was 24.7 +/- 2.3 days, whereas the histologic date was 20.0 +/- 2.6 days. The concentration of cytosolic P receptor in DLP endometrium tended to be lower, whereas the concentration of nuclear receptor was significantly higher in DLP than in seven matched patients with normal luteal phases. Endometrial estradiol-dehydrogenase activities were identical in both groups. The DLP syndrome cannot be explained by a decreased sensitivity of the endometrium to P and is probably merely functional in nature.

Detergent effects of kinetic properties of (Na+ +K+)-ATPase from kidney membranes.

Studies on (Na+ +K+)-ATPase generally employ detergents such as SDS and deoxycholate. Under such conditions, the purified enzyme possesses high specific activity. The (Na+ +K+)-ATPase from kidney membranes was unmasked by deoxycholate and SDS as described by Jłrgensen and its kinetic properties were studied. The results suggest that these detergents induce some irreversible alterations in the kinetic properties of the native enzyme. Another detergent, saponin, unmasked the (Na+ +K+)-ATPase as effectively as did SDS, but it seems to affect the kinetic properties of the native enzyme to a lesser extent.

[Atloido-axoid erosive osteoarthritis. Apropos of 3 cases].

Three cases of unilateral atloido-axis arthropathy are described in women aged 39 to 69 years. The initial diagnosis of arthritis made in view of the severity of the clinical (severe pain and stiffness) and radiological (marked chondrolysis, subchondral erosions), manifestations was contested in the absence of any laboratory abnormality of an inflammatory nature, as well as any bacterial, inflammatory rheumatic or metabolic cause. The final diagnosis made at the time of hospitalisation after 8 to 9 month progression of the disease was osteoarthritis, no subject to any doubt on the basis of the subsequent course. C2-C3 block present in 2 cases was a local factor of joint wear-and-tear. Other severe manifestations of osteoarthritis were seen in 2 patients. By analogy with certain forms of destructive osteoarthritis of the fingers, the term erosive osteoarthritis of the atloido-axis joint is suggested to describe this joint lesion.

DHEA-S-loading test in cases of intrauterine growth retardation: relationship between the pattern of the maternal plasma metabolites and the fetoplacental dysfunction.

Dehydroepiandrosterone sulfate loading tests (DLT) have been performed in 17 cases of clinically suspected intrauterine fetal growth retardation. At birth, 9 babies had adequate birth weights for gestational age (AGA) and 8 infants were small for gestational age (SGA). In both groups, plasma levels of dehydroepiandrosterone sulfate (DHEA-S), unconjugated DHEA, delta4-adrostenedione (delta4-A), testosterone (T), unconjugated estrone (E1) and estradiol (E2) were measured seven times from 15 min to 4 h after the intravenous injection of 50 mg DHEA-S. Several differences appear between the AGA group and the SGA group: (1) in the latter group, the rate of disappearance of the injected DHEA-S is decreased; (2) plasma levels of delta4-A and T are significantly increased and remain elevated for 2-3 h, whereas in the AGA group these values are increased only for a short period of time just after the injection. E1 and E2 patterns are similar in both groups. It has previously been demonstrated that an impaired response to DLT reflects a reduced uteroplacental blood flow. In addition, our results suggests a concomitant impairment of the aromatizing activity which reduces the placental metabolization of neutral steroids to estrogens.

[Tertiary hyperparathyroidism after renal transplantation in man. Treatment by parathyroidectomy and auto-graft. Eight cases (author's transl)].

In a series of 76 functional renal transplantations we noted 8 cases of hyperparthyroidism occurring in periods ranging from 2 months to 4 years. Hypercalcaemia was the most constant manifestation. PTH was raised in the 7 subjects in whom it was measured. Eight patients underwent total parathyroidectomy followed in 7 of them by auto-graft of a fragment of parathyroid into a forearm muscle according to the technique of Wells. The operation was followed in all cases by normalisation in blood calcium levels after the 1st month. Serum PTH levels also returned to normal apart from in one case where renal function was impaired because of rejection of the renal graft. Comparative estimations of PTH in venous blood from the side of the auto-graft and the opposite side showed that the parathyroid graft was functioning.

Steroid concentrations and delta 5, 3 beta-hydroxysteroid dehydrogenase activity in human placenta. Comparison between elective cesarean section and spontaneous vaginal delivery.

The level of delta 5, 3 beta-hydroxysteroid dehydrogenase (delta 5, 3 beta-HSDH) was measured in the mitochondrial and microsomal fractions of the human term placenta. The capability of the placenta to transform pregnenolone to progesterone was found to be significantly higher after spontaneous vaginal delivery than after elective cesarean section (p < 0.001). There was a significant rise in the placental concentration of estrone occurring at the time of parturition (p < 0.01). The placental concentration of estradiol-17 beta was also greater after than before parturition but the diffference was not statistically significant. Conversely to the estrogen concentration, no difference existed in the progesterone concentration between the two groups of placentas. The significance of these findings is discussed in relation to the parturition process.

Placental sulfatase deficiency: clinical and biochemical study of 16 cases.

Clinical and biochemical data of 16 typical cases of placental sulfatase deficiency have been observed. In vivo loading tests with DHA-S allowed us to make a prenatal diagnosis. In vitro experiments gave confirmation, showing zero or virtually zero placental sulfatase activity towards delta 5P or DHA sulfates Aromatase activities, when tested, were normal or more often less than standard values, the latter showing themselves rather large individual variations. All pregnancies were associated with the delivery of male neonates in good health but 3. The 15 living babies have been developing normally since then. These results, together with those reported in the literature, suggest that placental sulfatase deficiency is under control of an X-linked recessive character, this being supported by the recent observation of such a disorder in two sisters simultaneously pregnant. As to the high frequency problem of cesarian section, pointed out by several authors, we cannot conclude, from our own observations, that the defect has an obvious influence on the good outcome of labor, as 10 out of the 16 women delivered vaginally near term.

Exogenous DHA-S half-life: a good index of intrauterine growth retardation.

E3 and HPL hormone levels in maternal plasma are predictable of adequate birth weight for gestation age (AGA) when their values are higher than the mean for the term of pregnancy, but low values correspond to 'small-for-gestation-age' (SGA) infants in about 50% of the cases only. DHA-S (50 mg i.v.) half-life (DHS-S t 1/2) was calculated by least-squares analysis on the plot of log DHA-S concentration versus time at several intervals. 102 tests were performed in pregnant women between 30 and 41 weeks of amenorrhea (43 control, 59 suspicion of IUGR among them 29 AGA and 20 SGA). The difference between DHA-S t 1/2 is statistically highly significant (p < 0.001) between SGA and the other groups. Evaluation of birth weight was correct in 95 cases (76 normal babies < 4.29 h, and 19 SGA greater than or equal to 4.29 h, showed false-positive results in 6 cases, and false-negative in 1 case. This test has now been included in our routine practice for further evaluation of intrauterine growth retardation.

[Change test with DHEA-S in vivo and in vitro. Hypotrophy and utero-placental blood flow (author's transl)].

Using the method of Cedard and al., we perfused 18 placentas of women in which DHEA-S loading test has been performed during pregnancy. For each placenta, we measured the increase of estrogens in the perfusate (E1 + E2) 15 minutes after the injection of 10 mg of DHEA-S, and we calculated the half-life of the precursor in the system in well-standardized conditions. The comparison between: 1. Placentas of small for date babies and normal ones. 2. The increase of estrogens and the half-life of DHEA-S measured during pregnancy on the one hand and the same data obtained from the perfusion on the other, leads us to conclude that the capacity of the placenta to metabolize DHEA-S into estrogens is not diminished in small-for-date babies. The factor which modifies the response of the DHEA-S loading test in poor intrauterine growth seems to be due to a reduction of utero-placental blood flow.
