Comparative in vitro investigations on the dialysability of hypnotic and psychotropic drugs by hemodialysis and controlled sequential ultradiffusion.

The efficiency of hemodialysis and controlled sequential ultradiffusion (CSU) for the elimination of toxic drug concentrations was tested by in vitro-investigations. In the 6 benzodiazepin derivatives tested, the clearance is markedly higher at a blood flow of 200 ml/min than at 100 ml/min. Pyrithyldione, glutethimide and phenobarbital are better dialysed than the benzodiazepines with exception of chlordiazepoxide. In comparison with hemodialysis, hemofiltration by means of CSU was less effective because of the small amount of ultrafiltrate obtained.

HLA antigens and affective disorder: a family case report.

A family of 6 affective ill siblings is described. Two suffered from bipolar illnesses, 2 from recurrent unipolar illness, and the remainder showed alcoholism, depression and schizo-affective disorder. HLA typing revealed that all the tested members shared the antigens A3 and B7. Because only ill members were available for testing, there was insufficient information in the family to draw any definite conclusion as to whether these antigens were linked to the illness. However, the observation is of some interest in the light of other recent reports which have suggested that these 2 antigens are associated with affective disorder.

Medical correlates and diabetic retinopathy screening.

A systematic ophthalmological, medical, and biochemical screening of a rural population of diabetics in Western Australia is being carried out. A team of ophthalmologists, physicians, and paramedical personnel establish regional screening centres on a voluntary basis at weekends. Every attempt is made to locate and examine all known diabetics. All patients are given a thorough ophthalmic examination including standard multiple field colour fundus photography. Thorough physical examination includes assessment of cardiovascular and neurological status. Biochemical parameters include random blood sugar, urine analysis, serum creatinine, glycosylated haemoglobin, cholesterol, and high density lipoprotein. A detailed questionnaire and interview is carried out to assess the historical degree of control and the patient's knowledge of the control and management of diabetes. The sample now comprises an unselected population of 400 diabetics living in rural areas and allows comparison of areas which have scarce medical services with centres that are well supplied with medical practitioners. Incidence of retinopathy varies from 25 to 36 per cent of the diabetic population. Positive correlations of retinopathy have been found with duration, age, hypertension, level of control assessed biochemically at the time of examination, presence of ischaemic heart disease, peripheral neuropathy, and renal disease. Smoking also correlates positively. The data provide a baseline by which attempts to alter diabetic knowledge, management, and control may be measured in follow-up studies.

[The effect of chronic renal insufficiency with and without dialysis therapy on the manganese content of various human organs].

On the basis of the organ values of nine decreased, formerly with Al (OH)3 treated patients with renal insufficiency without dialysis and 15 patients with dialysis their Mn-state was investigated in comparison to 60 control persons comparable as to age and sex. The dialysis patients for a considerable part died of the so-called dialysis encephalopathy three to five years after the beginning of the Al-treatment. In all investigated nine organs with the exception of the blood of the dialysed patients (liver, hair of the head, myocardium, brain, rib, lung and testicle) the Mn-content was, in comparison to the control persons, essentially, partly highly significantly reduced. These findings were discussed in various kinds of animals in connection with the skeletal and nervous disturbances conditioned by the Mn-deficiency. It may not be excluded that the Al-hydroxyde used for the phosphate binding protects also another essential trace elements and particularly Mn from resporption and has an antagonistic influence on the Mn-metabolism, respectively. Animal experiments must clarify the complex mentioned.

[The clinical value of unspecific humoral immune parameters. Attempt at an immunogram. II. Quantitative determination of CH50E, CB50E, C3, C4 and lysozyme and measurement of indirect phagocytosis rate in the serum of patients with chronic pyelonephritis].

In 183 patients with chronic pyelonephritis as unspecific humoral immune parameters CH50E, CB50E, C3, C4 and lysozyme as well as the indirect phagocytosis against Staphylococcus aureus SG 511 and E. coli were tested. Clinically a differentiation according to 3 degrees of the renal function and 3 degrees of the activity of the disease was performed. The statistical analysis took into consideration mean and limit values. With the help of contingency tables tendency pictures were developed. C3, C4 and lysozyme proved as suitable for a humoral immunogramme in chronic pyelonephritis. C3 was increased in clear activity, C4 and lysozyme in restricted renal function and expressed activity. In view of the high biologic variation of humoral test parameters advantages and disadvantages of an immunogramme are discussed. But for patients with chronic pyelonephritis finally the estimations of IgM, IgG, IgD, C3, C4 and lysozyme are regarded as clinically useful. A feasable immunologic test set should be controlled, precised and completed.

[The clinical value of unspecific humoral immune parameters. Attempt at an immunogram. 1. Quantitative determination of IgG, IgM, IgA and IgD and demonstration of antibodies against cell and tissue bound antigens in patients with chronic pyelonephritis].

In the serum of patients with chronic pyelonephritis IgG, IgM, IgA, IgD and autoantibodies against organ- and species-specific renal antigens were determined. Clinical questions concerned relations to the activity of the disease and the renal function. The Ig-mean-values lying within the biological norm could be precised by taking into consideration individual biological boundary titres and relative ratio values. Pictures of tendency led to the estimation that increases of IgG and IgD appear in severely restricted renal function in normal or decreased IgM. Increases may be a criterion of activity in diseases with normal or slightly restricted renal function. IgA determinations remained without any evidence.-- Autoantibodies were found in 32,5%, increased titres were a sign of deteriorated renal function. The apparently given possibility of a humoral immunogramme for the chronic pyelonephritis is discussed, in which case a combination with other test parameters seems to be necessary.

Reassessment of histone gene expression during cell cycle in human cells by using homologous H4 histone cDNA.

The representation of H4 histone mRNA sequences in RNAs isolated from G1 and S phase HeLa cells was assessed by use of a homologous H4 histone cDNA. S phase cells were obtained by double thymidine block, and G1 cells were obtained by double thymidine block or mitotic selective detachment. Nuclear and cytoplasmic RNAs from S phase cells hybridized with H4 histone cDNA as did nuclear and cytoplasmic RNAs from G1 cells synchronized by double thymidine block. In contrast, significant levels of hybridization were not observed between H4 histone cDNA and nuclear, polysomal, or postpolysomal cytoplasmic RNAs of G1 cells synchronized by mitotic selective detachment. Double thymidine block yields a G1 cell population containing 20-25% S phase cells whereas the G1 population obtained by mitotic detachment contains less than 0.1% S phase cells. The ability of H4 histone cDNA to hybridize with the RNAs from G1 cells obtained after release from double thymidine block can therefore be explained by the presence of S phase cells in such a G1 population--an artifact of the synchronization procedure. We interpret these results to be consistent with the presence of H4 histone mRNA sequences during the S but not G1 phase of the cell cycle in continuously dividing HeLa S3 cells.

Specificity of DNA-associated nuclear antigens in HeLa cells and distribution during the cell cycle.

Chromosomal nonhistone protein:DNA complexes prepared from synchronized HeLa cells were used to immunize white rabbits. The antisera reacted specifically in complement fixation tests with chromatins isolated from HeLa cells and not with those from a number of other human and animal tissues. Specificity to this cell type was also demonstrated by immunocytochemical reaction. Both immunochemical tests revealed that the specific antigens are continuously present throughout the cell cycle. The immunological activity was dependent upon chromosomal nonhistone protein(s) being bound to DNA. Our findings are consistent with these chromatin antigens being stable nuclear components [complexes of chromosomal non-histone protein(s) with DNA] characteristic of cellular differentiation.

Radiolysis of chromatin extracted from cultured mammalian cells: production of alkali-labile strand damage in DNA.

Chromatin has been isolated from cultured Chinese-hamster lung fibroblasts as an expanded aqueous gel. The DNA in isolated chromatin has been examined by sedimentation on alkaline sucrose gradients. The average molecular weight of the DNA has been determined to be 50 million. gamma-irradiation of isolated chromatin degrades the DNA to lower molecular weight. The yield of single-strand breaks in the DNA is 0.02 single-strand breaks per krad-10(6) dalton, calculated from a dose-range of &--400 krad and covering a DNA molecular weight range of 2 X 10(7)-1.4 X 10(5). There is a considerable difference in the efficiency of the formation of single-strand breaks in DNA irradiated as isolated chromatin compared with chromatin irradiated in whole cells before isolation. For isolated chromatin, values of 6 dV per break have been calculated compared with about 80 eV per break for chromatin irradiated in whole cells, which suggest a large contribution from indirect action by aqueous radicals in isolated chromatin.

[The vinyl-chloride disease].

Workers engaged in the PVC-production, who were long-term exposed to the gaseous base vinyl chloride exhibit a complex pattern of damages. Changes of the skin, vascular and bone system, as well as thrombocytopenia, liver fibrosis with portal hypertension, disturbances of the porphyrin metabolism and alterations of the central nervous system were observed. Moreover vinyl chloride is oncogenic and may cause angiosarcoma of the liver. By directed occupational medicine supervision, improvement of the production conditions and introduction of new processing techniques health risks at the workplace could be eliminated to a far extend.

[Studies on the pharmacokinetics of Sulfaclomid, Mebacid, Nevigramon and Metindol in patients with reduced kidney function].

In patients with restriction of the renal function the pharmacokinetics of Sulfaclomid, Mebacid, Nevigramon and Metindol was investigated. According to the different elimination mechanisms of the drugs investigated the kinetics was infleunced in different way by the restriction of the renal elimination of these drugs: The half-life periods for the elimination of Metindol, Nevigramon and Mebacid remained unchanged in patients with renal diseases; the half-value period for the elimination of Sulfaclomid was prolonged.

[Clinical aspects and treatment of pericardial tamponade in uremic pericarditis].

On the basis of four cases with chronic glomerulonephritis in the terminal stage of renal insufficiency with uraemic pericarditis and tamponade of the pericardium, which in one patient developed before and in three patients under therapy of haemodialysis the author adopts a definite attitude to the problems of development, treatment and prognosis of uraemic pericarditis. For the treatment of the tamponade of the pericardium a suction drainage of the pericardium is recommended. Apart from this a particularly intensive haemodialytic treatment is necessary. After overcome uraemic pericarditis must be reckoned with a constriction of the pericardium as late complication which should early be treated operatively.
