Comparative evaluation of three dosages of slow-release isosorbide dinitrate (60, 80, 100 mg) in chronic angina of the aged.

In a single-blind, placebo-controlled study the acute and chronic antianginal effects of three slow-release (SR) new formulations of isosorbide dinitrate (ISDN 60, 80, 100 mg) have been comparatively evaluated in a group of aged affected by chronic stable effort-induced angina. Compared to placebo, overall the active dose paritetically improved the effort tolerance up to 24 h after the first assumption. In the time course of the trial (2 and 4 weeks) the resting hemodynamic changes induced by the first dose were partially blunted without affecting the exercise related-parameters. Also if plasma levels of ISDN and of its metabolites did not correlate to the degree of physical improvement, the peak increase in effort tolerance was observed under 100 mg treatment. Mild to moderate transient headache was experienced by 50% of actively treated and by 20% of placebo treated patients and no other serious adverse effects have been noted. One may conclude that ISDN in slow-release formulations of 60-100 mg isan effective, safe and well tolerated medication in the management of angina in the aged.

Clinical and EEG findings in eleven patients affected by mitochondrial encephalomyopathy with MERRF-MELAS overlap.

A study of mitochondrial DNA disease was carried out on 12 members belonging to three generations of a family from northern Sardinia. On the basis of the diagnostic criteria currently used in the classification of mitochondrial diseases a typical MERRF-MELAS overlap phenotype was seen in 11 patients with the mtDNA tRNA(lys) mutation at nucleotide position 8356. Clinical and instrumental investigations (EEG in particular) were made. Patients were divided into two groups: severely and mildly affected cases. The follow-up was reported. The aim of this study was to identify, through EEG, the early signs of the disease. The EEG findings recorded during the clinical evolution allowed us to recognize four degrees of cerebral involvement, and could also suggest the prognosis.

Effect of fipexide on passive avoidance behaviour in rats.

The effect of fipexide, administered at different intervals after the learning trial of a single step-through type passive avoidance situation was studied. The administration of fipexide immediately after the learning trial resulted in a long-lasting facilitation of passive avoidance behaviour. On the contrary, the administration of this compound 1 h prior to the retention test failed to influence passive avoidance behaviour. The results suggest that fipexide facilitates memory consolidation but does not influence retrieval processes.

[Study on the medium normalized velocity of circumferential lengthening of the fiber using ultrasound. I. Evaluation of left-ventricular compliance].

The following parameters obtained by echocardiography have been measured in a group of normal subjects and in a group suffering from hypertrophic subaortic stenosis (IHSS): 1) speed of mitral valve protodiastolic closing (MVV); 2) circumferential lengthening speed of the fibre (MVCL) both standardized and otherwise. There is a good correlation between MVCL and MVV (P = 0.01 and 0.05). Since a low mitral valve closing rate is an indirect index for measuring ventricular compliance, the use of the MVCL can be considered valid for the same purposes. Using this it has in fact been possible to distinguish subjects with IHSS in which lower MVV is an index of reduced compliance, from normal subjects.

[Study on the medium normalized velocity of circumferential lengthening of the fiber using ultrasound. II. Use of the construction of left-ventricle performance curves].

Telediastolic pressure and volume can readily be employed as expressions of sarcomere length when constructing ventricular performance curves for the normal heart. In pathological hearts, however, these parameters are rendered inadequate as a result of altered compliance, which means that diastolic lengthening of the muscle fibre cell must be determined from telediastolic stress and linear compliance, corrected for wall thickness (muscle fibre stretch, strain index). Ventricular performance curves were constructed for normal subjects and patients with hypertrophic subaortic stenosis, using the normalised mean rate of circumferential lengthening of the fibre as the expression of fiber strength. Comparison was made with the mean circumferential shortening rate and the ejection fraction as the expression of ventricular effort. It was found that both normal subjects and stenosis patients lay on the same curve, the latter acting low down through having short sarcomeres.

[Cost of ICU in Italy. Results from an empirical study on a sample of 12 hospitals].

BACKGROUND: The Italian hospital payment system based on DRG doesn t properly include Intensive Care Units (ICU) costs. Since great emphasis has been recently given to rationing health care resources, assessing ICU costs seems to be dramatically relevant. Aim of the study was to assess the average yearly cost and the cost per diem of a sample of Italian multispecialistic ICU wards. METHODS: In September 1995, a questionnaire concerning data on variable and fixed cost was sent to 25 Italian ICU wards, 11 NHS hospital-based (Northern Italy: 5; Central Italy: 4; Southern Italy: 2) and 14 school of medicine-based (Northern Italy: 7; Central Italy: 5; Southern Italy: 2). Variable cost data included: disposable, drugs, blood and blood-derived products, physical tests, chemical and microbiological routines, instrumental diagnostic procedures and physiotherapy. Concerning fixed costs, data on personnel and equipment were requested. In addition, some hospital overheads data (utilities; power; heating; maintenance; cleaning; laundry; accounting; waste disposal; cafeteria) were collected. RESULTS: On the basis of the 12 questionnaires returned (Northern Italy: 9; Central Italy: 3; Southern Italy: 0), the yearly cost of an ICU ward is Liras 4,580,032,000 (range 2,739,277,000-7,704,292,000), whereas the average cost per diem is Liras 1,802,000 (range 1,234,000-3,179,000). Cost of personnel is about 61% of the above mentioned costs. CONCLUSIONS: Despite the lack of questionnaires from Southern Italy and the unavailability of some data concerning both the cost of equipment and the overheads, the remarkable average cost values obtained could support further research.

[Causal link in the assessment of the relationship between neoplastic diseases and occupational and environmental exposure in the judicial field].

IARC evaluations of the carcinogenic risk to humans, although based on epidemiological and experimental studies, derive their value from the consensus among committee members on a decision based on the current status of knowledge, not from the studies themselves, nor from their summary evaluation. A judiciary setting, based on the opinion of an individual expert, or on the evaluation of the most updated state of the knowledge by a group of experts selected in non scientific settings, or even worse on the evaluation of a specific case with ad hoc epidemiological studies, cannot confute such decisions. Therefore, causal links already defined within the scientific community, such as between exposures in the IARC category 1 and specific cancer sites, should be accepted also in legal trials to establish individual responsibility in case of excess exposures for the general and/or the working population resulting from violation or omission of preventive duties, when those diseases follow such excess exposures. If no violation or omission occurred, a civil responsibility has to be defined when either a disease with established link with the given exposure occurs, or a disease for which no such a level of consensus exists in the scientific community, but for which the legal trial has reached a positive decision. In such instances, if omissions or law violations occurred, the legal trial carries the burden of establishing a causal link between such omissions and violations and the damage for the individual worker or citizen.
