Juvenile nodular aponeurotic fibroma in the area of the knee joint.

A case of calcifying aponeurotic fibroma of the knee occurred in a 14-year-old boy. Review of the literature reveals that calcifying aponeurotic fibroma is an uncommon benign tumor with a tendency for local recurrence. It is manifested by a progressively growing mass involving fat, fascia, and aponeurosis. Chondroid tissue and calcification within the areas of proliferated spindle cells are the characteristic features of this lesion and differentiate it from the other forms of fibromatosis.

Administration of myelin basic protein-coupled spleen cells prevents experimental allergic encephalitis.

Intravenous administration of mouse myelin basic protein covalently coupled with chromic chloride to syngeneic spleen cells (MBP-SC) prevents the subsequent induction of experimental allergic encephalitis (EAE). Whereas 1 in 28 mice receiving MBP-SC developed EAE after immunization with mouse spinal cord homogenate (MSCH) and adjuvants, 16 out of 25 mice receiving ovalbumin-coupled spleen cells (OA-SC) had EAE following encephalitogenic challenge. The effect of administration of antigen-coupled spleen cells on in vitro proliferation responses is shown.

High doses of diazepam improve neuroleptic-resistant chronic schizophrenic patients.

According to the two currently most popular biological hypotheses, schizophrenic symptoms result from a hyperactivity in dopaminergic neurotransmission or from a hypoactivity in GABAergic neurotransmission. Since diazepam is known to reduce dopamine release and to potentiate GABA, the possible beneficial effects of diazepam were tested in ten hospitalized chronic schizophrenic patients who were resistant to standard neuroleptic treatment. High doses of diazepam, up to 200 mg/day initially, but smaller maintenance doses (less than 55 mg/day diazepam in eight of the ten patients) were added to the previous neuroleptic medication of these patients. The diazepam dose was adjusted daily to avoid oversedation. The effects of diazepam treatment on the mental status were assessed weekly for 12 weeks by the Brief Psychiatric Rating Scale (BPRS), the physician's Clinical Global Impressions Scale (CGI), and the Psychotic Inpatient Profile Scale (PIP). For additional documentation, videotapes of mental status interviews were obtained at baseline and during diazepam treatment. These videotapes were rated blind by an independent psychiatrist. The addition of diazepam produced a marked improvement in three, a moderate improvement in four, a mild improvement in one and no change in two of the ten patients. Four of the ten patients were so much improved that they were discharged from the hospital. No side effects were noted, except for one patient who became confused and disoriented on 160mg diazepam/day.(ABSTRACT TRUNCATED AT 250 WORDS)

Lack of association between [3H]imipramine binding sites and uptake of serotonin in control, depressed and schizophrenic patients.

Uptake of serotonin and [3H]imipramine binding were studied in parallel in the platelets of control, depressed and schizophrenic patients. In the depressed patient group, uptake of serotonin was consistently reduced while [3H]imipramine binding was only decreased in a number of these patients. In the schizophrenic group, uptake of serotonin was reduced to 62% of control with no changes in [3H]imipramine binding being observed. These data demonstrate a clear dissociation between uptake of serotonin and [3H]imipramine binding sites in human platelets. The possible functional role of these [3H]imipramine binding sites remains to be determined.

Restricted environmental stimulation therapy (REST) as a treatment for autistic children.

This study explored the usefulness of 48 hours of Restricted Environmental Stimulation Therapy (REST) as a treatment for autistic children. In order to provide quantified objective measures for evaluating the effects of this treatment, a battery of psychological tests was developed which would be useful and practical for the assessment of these children in regular diagnostic settings. Several positive changes in learning, social and play behavior, and cognitive functioning were noted.

Ultrasound mammography for the augmented breast.

Ultrasound examinations were performed on 25 women with augmented breasts using whole-breast water-path scanning. In these patients, ultrasound imaged carcinoma, benign lesions, and fibrous capsule formation; these were confirmed by surgery. In addition, it was noted that certain prostheses appeared to be displaced posteriorly onto the chest wall and some prostheses had internal echoes. To evaluate these findings, in vitro B-scanning was performed on three different commonly used mammary prostheses: the silicone gel, the saline inflatable, and the double-membrane gel-inflatable type. Ultrasound study of the augmented breast allowed evaluation of breast abnormalities in the parenchyma overlying the prostheses and also permitted evaluation of the complications unique to the implantation procedure: abscess formation, displacement of the implants, fibrous capsule formation, and wrinkles or cracks within the implant.

Prevalence of tardive dyskinesia in affective disorder patients.

Of 83 consecutive patients in an affective disorders clinic who were assessed with the Abnormal Involuntary Movement Scale, 35 (41%) were found to have tardive dyskinesia (TD). TD patients were found to be older than affective disorder patients without TD.

Effect of domperidone on apomorphine-induced growth hormone secretion in normal men.

Domperidone, a peripheral dopamine (DA) receptor blocker which poorly crosses the blood-brain barrier and which is inactive towards dopamine-sensitive adenylate cyclase, in a dose (100 micrograms/kg) sufficient to increase serum prolactin levels at least 5-fold, decreased the growth hormone (GH) response to the DA receptor agonist, apomorphine HCI (Apo) (0.5 gm s.c.) in each of six normal men examined. The mean GH increment at 30, 45, 60 and 75 min following Apo injection, the mean individual peak increment and the mean individual GH secretion (ng min) was significantly decreased by domperidone pretreatment (p less than 0.005 -p less than 0.002). These results indicate that in man Apo stimulates GH secretion by an effect on DA receptors which are not linked to adenylate cyclase and which are situated at a locus in the hypothalamic-pituitary axis that lies outside the blood-brain barrier.

Diazepam in high doses is effective in schizophrenia.

1. Diazepam in high doses, up to 400 mg per day, was administered to paranoid schizophrenic patients in a double-blind placebo-controlled study. 2. The effects of treatment were assessed by the following: the Brief Psychiatric Rating Scale (BPRS); the Clinical Global Impressions Scale (CGI); the Schizophrenia Subscale of the Present State Examination (SS-PSE); the Simpson-Angus Rating Scale (SARS) for extrapyramidal symptoms; the Nurses' Observation Scale for Inpatient Evaluation (NOSIE); and a 90 item Self-Assessment Questionnaire (SCL-90). 3. Within a few hours to a few days from the onset of diazepam treatment both positive (such as auditory hallucinations and persecutory delusions) and negative (such as emotional withdrawal and blunted affect) schizophrenic symptoms were dramatically eliminated in 5 out of the 6 patients.
