[Hageman factor deficiency (factor XII)--hemorrhage or thrombosis?].

Four generations of a kin with congenital Factor XII deficiency were examined for coagulation and fibrinolysis, with the homozygous female carrier of features with a Factor XII below 1% also revealing certain indications of a disturbed fibrinolysis. The other members of the family had to be evaluated as heterozygous ones, showing values of Factor XII between 40 and 60%. The findings are discussed by referring to data from literature.

[Use of heparin in combination with dihydroergotamine for the postoperative prevention of thrombosis in general surgery].

The administration of a combination of heparin and dihydroergotamine for the prevention of postoperative thromboembolism is based on the findings concerning the venous thrombogenesis that heparin low doses (2 X 5,000 E/d, subcutaneously) inhibits the coagulation processes and the venous backflow of blood is improved by dihydroergotamine (2 X 0.5 mg/d, subcutaneously). In numerous studies this combination has proved to be the most effective method in general surgical interventions to prevent both postoperative deep vein thromboses and pulmonary embolism. Therefore, this prophylaxis is recommended for the use in clinical practice on condition that contraindications are taken into consideration. Further trends are reduction of the heparin dose to 2 X 2,500 E/d and buccal application of dihydroergotamine.

[Clinical and chemical factors in kidney calculus formation in childhood. An analysis of 40 patients].

During 5 years, between 1978 and 1982, 40 patients (22 male, 18 female) aged 1 to 17 years presented with kidney stones at the Erlangen University Children's Hospital. Stone analysis showed calcium oxalate in 61%, calcium phosphate respectively magnesiumammonium phosphate in 33% and cystine in 6%. Calcium phosphate/ magnesiumammonium phosphate stone bearers were most frequently found in the first 5 years of age and showed more often urinary tract malformations, staghorn renal calculi, urinary tract infections and a higher urinary pH. Calcium oxalate stone bearers were more frequently found in patients of school age and had more often relatives with nephrolithiasis. In the group of calcium oxalate stones hypercalciuria, hyperoxaluria and hyperuricosuria were most frequently observed.

On the LH response to oestrogen and LH-RH in transsexual men.

In male rats, androgen deficiency during a critical period of sexual brain differentiation was shown to give rise to a predominantly female-differentiated brain. Such animals displayed "homosexual behaviour", i.e., they were sexually attracted preferentially to partners of the same sex. In addition, they exhibited a sex-specific evocability of a positive oestrogen feedback effect. A positive oestrogen feedback effect on LH secretion was also induced in homosexual transsexual men, in contrast to hetero- or bisexual transsexual men. Thus in homosexual transsexual men, an intravenous injection of 20 mg Presomen (Premarin) produced a significant decrease of serum LH levels followed by a significant increase above the initial LH values. In hetero- or bisexual transsexual men, by contrast, intravenous oestrogen administration, while producing a significant decrease of serum LH levels, was not followed by an increase above the initial LH values. A positive oestrogen feedback effect on LH secretion was also found in homosexual non-transsexual men, in contrast to heterosexual men. These findings suggest that transsexual as well as non-transsexual homosexual men possess a predominantly female-differentiated brain which may be based, at least in part, on androgen deficiency during sexual differentiation of the central nervous system. Homosexual transsexual men also showed an increased LH and FSH response to LH-RH as compared to hetero- or bisexual transsexual men.

[The prune-belly syndrome].

All features of the Prune-Belly syndrome--the congenital deficiency of the abdominal musculature with associated malformations of the urinary tract--are presented. The report is divided into the following headings: definition, etiology, morbidity, pathomorphology, staging, correlated pathology and therapy. The therapeutic strategy as proposed 1978 by Woodard et al. is emphasized. A report on six own cases is included.

[Sarcoma of the urogenital tract in children].

Motivated by necessity and supported by a review of the literature of the past 8 years and two cases of bladder-prostate sarcoma in our own clinic, the natural history of juvenile urogenital rhabdomyosarcoma is systematically presented, including the changes in therapy in recent years. The paper is organized under the headings etiology, organ distribution, morbidity, morphology, approach to therapy and complications. Pre- and postoperative cytostatic therapy has not only increased the cure rate, but probably also permits, within certain limits and with reservations, organ preserving operations rather than destructive evisceration.

[Continuous extra-amniotic intracavitary application of prostaglandin F2 alpha for medicamentous induction of labour in high-risk cases of pregnancy interruption].

Reported in the paper is continuous extra-amniotic application of Prostaglandin F2 alpha (PG F2 alpha) to 70 juvenile primigravidae for termination of pregnancy. Successful priming was achieved in 54 patients after three and a half hours of induction, following administration of an average induction dose of 3 mg PG F2 alpha. Induced abortion was "incomplete or retained" and, therefore, stopped in 16 cases. An account is given of advantages of the above approach over intermittent intra-uterine application. The technique described in this paper was found to be suitable and commendable for minimizing of side-effects and risk as well as for better and individual controllability. It has also proved to reduce the variety of contra-indications to prostaglandin induction for patients with pre-existing diseases. Continuous intracavitary induction of labour may be considered as a justified alternative to the risks implied in other techniques of interruption.

[Prospective studies on pregnancy following induced and spontaneous abortion of primigravidae and assessment of fertility. IV. report].

Prospects for imminent and spontaneous abortion were studied in three groups of probands. These included secundigravidae following induced abortion, secundigravidae after spontaneous abortion, and primigravidae. --Chronological distribution of abortion was determined for two periods of time, up to the 16th week of pregnancy and between the 17th and 27th weeks of pregnancy. Reference is made to correlations between abortion and cervical insufficiency. Poor prognosis in a group with a record of spontaneous abortions was compared with accumulation of functional bleeding in the same group. --An account is given also of the incidence of defective infants in all three groups, in the context of prematurity.

[Tubal factors as possible causes of sterility or infertility of primigravidae, following therapeutic abortion (author's transl)].

A screening test was applied to 136 primigravidae, between six months and one year following therapeutic abortion, to check tubal patency, using pertubation, according to Semm. --The methodical errors involved were compared and checked by laparoscopy with chromopertubation. --Reference is made to effects of age at the time of abortion, week of pregnancy, and technique of abortion. --Complete tubal occlusion, as a secondary cause of secondary sterility in the wake of therapeutic abortion, was recorded from 2.96 per cent of the above primigravidae. The rate of tubal infertility (tubal abortion or extra-uterine gravidity) was not significantly increased beyond the figures of other patients (0.96 per cent of all primigravidae, following therapeutic abortion). --Derived from the above results are recommendations for techniques of abortion which should be used on primigravidae.

Mucolipidosis I (acid neuraminidase deficiency). Three cases and delineation of the variability of the phenotype.

Isolated deficiency of the lysosomal hydrolase acid neuraminidase results in multisystem storage of sialic acid-rich oligosaccharides. Wide phenotypic diversity occurs within this biochemical defect. We studied three cases of an infantile form of mucolipidosis I in which the phenotype is dominated by severe Hurloid features. These patients excreted excessive amounts of sialic acid-rich oligosaccharides in their urine, and storage of similar compounds was shown in tissues and cultured fibroblasts. Cultured fibroblasts demonstrated an isolated deficiency of acid neuraminidase; beta-galactosidase levels were normal.

Clinical utility of ambulatory EEG monitoring.

Our initial clinical experiences with the "Oxford" ambulatory nontelemetry EEG monitoring shows that it is a useful and economical device for monitoring EEG and EKG activity in patients in a normal environment. Careful selection of the patient with possible seizure disorder and emphasis on temporal lobe recordings may give a higher degree of diagnostic yield in questionable cases of seizure. Further investigations utilizing this modality (ambulatory EEG monitoring) appears to be fully warranted. Further study, carefully controlled with regard to patient selection, age, chief complaint and previous EEG findings is currently underway at the Neurological Institute, Inc.

[Clinical experience from transcervical intra-amnionic hypertonic saline instillation for termination of pregnancy in second trimester].

Transcervical intra-amnionic hypertonic saline instillation was used and tested for its practicability on pregnant women in the second trimester with contra-indications to prostaglandins. The pregnancies were terminated without any remarkable complication, under most stringent observation of aseptic conditions, careful cleaning of the birth canal, and preventive use of broad-range antibiotics. Immediate morbidity accounted for 4.35 per cent. Hence, the technique was not inferior to other approaches, when reference was made to negative outcome of termination. No late morbidity was recorded by clinical follow-up checks. Once the method will be verified by a higher number of patients and further improved, for example, by reducing the time required for induction, it may become a readily practicable and inexpensive variant of termination in the second trimester.

[Prospective studies into pregnancies of primiparae with record of therapeutic termination of previous pregnancies or of spontaneous abortion and assessment of fertility. First communication (author's transl)].

A comparison was made between 535 primiparae with a record of therapeutic termination of previous pregnancies, 305 primiparae with previous abortion, and 501 primiparae without any record of spontaneous or medically induced abortion, all of these being compared for their prepartum, intrapartum, and postpartum phases as well as the conditions of their children delivered in their first intrapartum, and postpartum phases as well as the conditions of their children delivered in their first full-term pregnancy.--Significant accumulation of spontaneous abortions, bleeding in the first and second thirds of pregnancy, as well as postpartum complications were recordable from women with record of induced or spontaneous abortions, in comparison with primigravid primiparae. All probands with record of previous termination of pregnancies exhibited a trend towards premature births, yet, with no significant accumulation of these. Significantly increased use of therapeutic and prophylactic measures, such as cerclage and tocolysis, should be taken into due consideration, in that context.
