[Uremic plasma as cause of metabolic changes in red blood cells].

Under the influence of the uraemic plasma a number of changes of the metabolism of red blood cells develop. By means of cross experiments with the help of filtrability, of sulf-haemoglobin formation and the 125-iodine-hippuran intake the influence of the uraemic plasma and of the plasma of healthy persons on the red blood cells of children who undergo a chronic haemodialysis as well as of healthy adults (cross experiments) was examined. The red blood cells of the control persons after an incubation at 37 degrees and 60 or 180 minutes showed approximately the same changes of filtrability, formation of sulf-haemoglobin and 125-iodine-hippuran as the red blood cells of the patients with incubation in the autologous plasma. Disturbances of the filtrability and the formation of sulf-haemoglobin could be proved in the uraemic red blood cells still after reincubation in the normal plasma, though clearly reduced. Only in 125-iodine-hippuran a normalisation could be got. Filtrability and formation of sulf-haemoglobin apparently comprise more complex processes which essentially influence the life span of the red blood cells, whereas the 125-iodine-hippuran value characterizes only a part of the metabolism of the red blood cells, the passive transport of anions.

Risks of cervical intraepithelial neoplasia among DES-exposed women.

The increased occurrence of cervical intraepithelial neoplasia (CIN) in women with in utero exposure to diethylstilbestrol (DES) is described. Among 335 self-referred women exposed to DES, 14.9% (50) had a least 1 cytologic smear interpreted as CIN, and 16.2% (54) had at least 1 biopsy interpreted as CIN. Excluding those with grade 1 CIN, 8.7% (29) had at least grade 2 CIN on biopsy and 4.2% (14) had biopsies interpreted as grade 3 CIN. No invasive lesions were detected. Because the problem of differentiating true intraepithelial neoplasia from morphologically indistinguishable but benign lesions in DES-exposed women remains unresolved, the authors believe a conservative approach to treatment is still indicated. This study supports the recommendation that DES-exposed women merit continued thorough gynecologic evaluation, including regular cytologic evaluation and colposcopy, with biopsies as indicated.

[Removal of infected entrapped pacemaker electrodes by continuous traction (author's transl)].

To remove an infected pacemaker system in order to control the infection is often difficult when there is extensive connective-tissue fixation of the electrode. Forced manual extraction may lead to severe complications. For this reason, operative removal, in certain circumstances involving cardiotomy under extracorporeal circulation, has been practised. An alternative is continuous traction in which the probe is attached peripherally and by continuous traction removed from its cardiac attachment. Nine patients have been treated successfully by this method. In eight the electrode was removed within 1-10 days without significant complications. In one instance the electrode-catheter tore within the superior vena cava after 18 days of continuous traction; the remaining catheter, about 12 cm long, was removed with a sling catheter.

[Orally active vasodilators in the management of chronic treatment-resistant cardiac failure (author's transl)].

The acute haemodynamic effects of 40 mg isosorbide dinitrate (10 subjects), 4 mg prazosin (20 subjects) and 50 mg dihydralazine (8 subjects) were compared in 24 patients with the clinical picture of chronic therapy-resistant cardiac failure (NY Heart Association stages III-IV). There was a fall in left-ventricular filling pressure of about 15% and right-atrial mean pressure of 21 and 24%, respectively, with isosorbide dinitrate and prazosin, while there was no change with dihydralazine. Cardiac output rose by 23% with dihydralazine and 20% with prazosin, but remained unchanged with isosorbide dinitrate. These data indicate that a reduction in pulmonary and systemic-venous congestion due to chronic decompensated cardiac failure can be achieved with isosorbide dinitrate and prazosin, while cardiac output can be improved only with prazosin and dihydralazine.

[Longterm results of operative treatment of benign strictures of the extrahepatic biliary tracts (author's transl)].

With reference to choice of procedure and longterm results, 52 cases of benign stricture of the extrahepatic biliary tracts are presented. In 86.2% of the cases followed up, the aim of immediate biliary drainage and the formation of a permanent outflow into the intestine are achieved. But freedom from relapse and complaints was attained in only 62.1%. The danger of new obstructions, cholangitis and subsequent liver damage is not removed so that a definite cure cannot be spoken of. Since in most cases strictures in chronic gallstone diseases and iatrogenic stenoses after the first operation on the biliary tract or stomach are involved, an early operation for confirmed gallstone diseases and accurate operative technique achieve considerable prophylactic character.

Salmonellosis in mice: studies on oral immunization with live avirulent vaccines.

The avirulent Salmonella typhimurium galE strain G30, following oral feeding to mice, developed a state of immunity to a secondary oral challenge with virulent Salmonella typhimurium. This immunity was concommitant with the development of intestinal and serum antibodies and delayed-type hyepersensitivity (DTH) to Salmonella antigens. In contrast, repeated oral doses of a hybrid E. coli vaccine, which expresses the O antigens of Salmonella typhimurium, provided a lesser degree of immunity and, although able to stimulate the formation of serum antibodies, it elicited only a low level of intestinal antibodies and no measurable DTH.

Nuclear events in B-cells of young and senescent rat islets in organ culture.

In vitro, B-cells maintain their capacity to multiply. However, the number of mitotic figures observed in cultured rat Langerhans islets is far larger for young donnor animals than for old ones. The same observation is true for the number of nuclei incorporating tritiated thymidine in vitro. These data are in agreement with the principal observation resulting from the study of senescence on different in vitro cellular models: the decrease of the replication potential with increasing age. The stimulation of DNA synthesis by glucose, however, does not seem to be altered with aging.

Post-menopausal bleeding: a study of more than 1000 cases.

This paper reports the cause, incidence of malignancies, certain clinical features, and the time lag between the bleeding and the establishment of its cause, in the 1019 cases of post-menopausal bleeding investigated by the Universitäts-Frauenklinik in Tübingen, West Germany, between 1969 and 1972. Although many of the patients had experienced just one post-menopausal bleeding before seeking medical help, and most reported that their bleeding(s) had been "light" and of short duration, a malignancy was found in 48% of the cases. The most common were cervical cancer (228 cases) and endometrial cancer (215). 31% of the cervical cancers and 12% of the endometrial cancers were in an advanced stage of development. A clear link emerged between increased age and the incidence of malignancies, but the paper shows the importance of any type of post-menopausal bleeding, however slight, being thoroughly investigated.

Comparison of Naja n. naja and Naja h. haje cobra-venom factors: correlation between binding affinity for the fifth component of complement and mediation of its cleavage.

Two cobra-venom factors, one from Naja n. naja (CVFn), the other from Naja h. haje venom (CVFh), have been purified and compared, functionally and structurally. Both factors interacted with human factors B and DS to form a potent C3 convertase, CVFBb. However, while the convertase formed with CVFn did also efficiently cleave C5, CVFhBb had very little C5-cleaving potency only, in particular when human C5 was used as substrate. Studies with agarose-linked CVF preparations indicated that CVFh has only low binding affinity for C5gp and C5hu whereas CVFn binds to both C5 species with much higher affinity. Since C5-binding (to CVF or to C3b) is a prerequisite for its cleavage by C3/C5 convertases, the difference in binding potency explains the different C5-cleaving activity of the two CVF preparations. When a ligand for C5, surface-fixed C3b, is present, CVFhBb is also capable of cleaving C5. The difference in activities of CVFn and CVFh is reflected in their different potency to interfere with immune haemolysis and in causing indirect lysis by their complexes with activated factor B. By gel chromatography of the CVF preparations in C5-containing medium, a stoichiometric complex CVFn-C5 (1 + 1) could be demonstrated. An analogous complex of C5 was neither found with CVFh, nor with C3hu or soluble C3bhu. Structural differences between CVFn and CVFh were revealed by immunodiffusion analysis and by polyacrylamide-gel electrophoresis in presence of SDS. The data available so far provide, however, no clear information about the structure of the C5 binding site.

Characterization of pili associated with Escherichia coli O18ac.

A strain of Escherichia coli O18ac isolated from the stool sample of a patient with diarrhea was found to agglutinate human erythrocytes. From the results presented it is suggested that this hemagglutination is mediated by pili. Isolated pilus preparations agglutinated human erythrocytes, whereas pilus-negative mutants did not. The serological and chemical analyses indicate that the pili associated with E. coli O18ac are distinct from other types found with E. coli.
