[Granuloma anulare perforans: 2 clinically different forms].

Two different forms of granuloma annulare perforans are described. One form, which particularly affects young people, is characterized by small nodular lesions with central umbilication and a monomorphic clinical picture. These lesions can persist for many years, but spontaneous remission often occurs. The second form affects older patients. The clinical picture is polymorphic, consisting of dull red papules, umbilicated papules, pustular lesions and small atrophic scars. This form is resistant to a variety of therapies and does not heal spontaneously.

Association of ulcerative colitis, sclerosing cholangitis and cholangiocarcinoma in a patient with IgA deficiency.

A case of an unusual association of ulcerative colitis, sclerosing cholangitis, cholangiocarcinoma and IgA deficiency in a 22-year-old man is reported. The patient was admitted to the hospital with jaundice. Ulcerative colitis had been diagnosed 4 years before. The association of sclerosing cholangitis and cholangiocarcinoma was demonstrated. In addition IgA deficiency was discovered. Possible pathogenetic implications are discussed.

Echocardiographic and hemodynamic assessment of right heart impairment in chronic obstructive lung disease.

42 patients with chronic obstructive lung disease underwent right heart hemodynamics (Swan-Ganz catheter) and M mode echocardiography. Echocardiographic study showed that right ventricular index (RVI) and right ventricular anterior wall thickness (RVAWT) were increased in most patients, while septal thickness and motion were nearly always normal. The finding of impaired RVI and normal RVAWT in several patients, while no one showed normal RVI and increased RVAWT, suggests that arterial pulmonary hypertension results in initial dilatation and only later in hypertrophy of the right ventricle. Echocardiographic parameters were well correlated with hemodynamic and pulmonary function parameters. The good correlations between echocardiographic and hemodynamic data suggest that echocardiography may be a useful technique in the noninvasive assessment of the effects of pulmonary hypertension on the right heart in COLD.

Right heart impairment in sarcoidosis: haemodynamic and echocardiographic study.

62 patients with histologically confirmed sarcoidosis (7 with Stage 0, 9 with Stage I, 20 with Stage II, 26 with Stage III) underwent right heart haemodynamics (Swan Ganz) and M-mode echocardiography. Haemodynamic study showed mild to moderate pulmonary hypertension in many patients with Stage II and III sarcoidosis, and 5 patients with Stage 0 healed sarcoidosis, showed borderline values of right sided pressures. Echocardiographic study showed increased right ventricular index (RVI) and right ventricular anterior wall thickness (RVAWT) in most patients. The good correlations between echocardiographic and haemodynamic data suggest that echocardiography may be a useful technique for the non-invasive assessment of the effects of pulmonary hypertension on the right heart in sarcoidosis.

[Echocardiographic and polygraphic study of acute/effects of Ibopamine on cardiac performance].

The effects of Ibopamine, a new orally active dopaminergic agent, on cardiac performance were studied both in a group of healthy volunteers and in a group of patients with congestive heart failure (CHF), using non-invasive methods, such as echocardiography and polygraphy. Randomly, and in double-blind fashion, the healthy volunteers received single doses of Ibopamine 50, 100 and 150 mg orally every 3 days. The CHF patients, also randomly and in double-blind fashion, were given single doses of either placebo or Ibopamine 100 or 150 mg every 2 days. Echocardiographic and polygraphic data were recorded in baseline conditions and 3, 6 and 8 hours after the drug administration. The heart rate (HR), systolic blood pressure (SBP) and diastolic blood pressure (DBP) were measured at the same time. The following parameters were considered: echocardiography: left ventricular end-diastolic diameter (LVEDD), left ventricular end-systolic diameter (LVESD), interventricular septum excursion (IVSE), left ventricular posterior wall excursion (LVPWE), per cent fractional fiber shortening (%FS); polygraphy: electromechanical systole (Q-A2), total ejection time (ET), pre-ejection period (PEP), ET/PEP ratio. In CHF patients, body weight and 24-h diuresis were also measured daily. Neither of the two groups showed significant changes in HR, SBP and DBP with any drug dose. A shortening of ET (-2.2%, p less than 0.01) and Q-A2 (-2.8%, p less than 0.001) was found in the group of normal subjects with a dose of 50 mg.(ABSTRACT TRUNCATED AT 250 WORDS)

Crystalline A-dna: the X-ray analysis of the fragment d(G-G-T-A-T-A-C-C).

An A-DNA type double helical conformation was observed in the single crystal X-ray structure of the octamer d(G-G-T-A-T-A-C-C), 1, and its 5-bromouracil-containing analogue, 2. The structure of the isomorphous crystals (space group P61) was solved by a search technique based on packing criteria and R-factor calculations, with use of only low order data. At the present stage of refinement the R factors are 31% for 1 and 28% for 2 at a resolution of 2.25 A (0.225 nm). The molecules interact through their minor grooves by hydrogen bonding and base to sugar van der Waals contacts. The stable A conformation observed in the crystal may have some structural relevance to promoter regions where the T-A-T-A sequence is frequently found.

[Immunity to diphtheria in advanced cancer patients].

In expectation of a possible anticancer treatment with diphtheria toxin, the antidiphtheria immunity has been tested on 65 advanced cancerous patients. The test has been carried out by Schick's test and by blood antibodies' dosage effected by a process "in vivo" on the depilated skin of the New Zealand rabbit. The results show that about the 30% of these patients has no antidiphtheria immunity. The remaining 70% has antidiphtheria immunity, even if in half cases the antibody level is lower than the limit conventionally considered minimum. In case of a treatment with diphtheria toxin on cancerous patients: we suggest to correlate the doses to the antibody level.

[Diphtheria toxin: ability to agglutinate human leukemia cells].

Human leukaemic cells were agglutinized by diphtheria toxin during an incubation of two hours. the agglutination behaved as a phenomenon of prozone. The cells grouped only in the zone with fitting proportion between toxin and cells, while no aggregation took place where lower or higher doses of toxin were used. The toxin didn't induce the agglutination of granulocytes taken from patients with acute infections diseases or taken from healthy donors. Normal lymphocytes too were not glutinated by toxin. The leucocytic agglutinability caused by toxin may be correlated to changes on the membrane induced by malagnity.

[Experiment comparative clinical study with three liver trophical substances on some alcoholic not cirrhotic liver disease (author's transl)].

By using and comparing three liver trophical substances, the authors have studied their individual effect on some alcoholic not cirrhotic liver diseases. They conclude at last that the removal of the alcoholic noxa and the action of the substances individually used cause an acceleration of recovery and a regression of the alterations of hepatic insufficiency, due to an excessive administration of alcoholic drinks. It's difficult, however, to draw some useful practical indications particularly on the choice of this or that medicament used.

[Further clinical observations on the treatment of noncirrhotic alcoholic liver disease].

After an experiment comparative clinical study, the authors could point out that biohumoral, semeiological and histopathological alterations in alcoholic hepatopaties, can retrograde only by the suspending the pathogenic alcoholic noxa, and that the improvement can be accelerated by a contemporary administration of a. cycloxilic, thiazolidine carboxylic of argynin, EPL: all antisteatosis substances helping the oxidation of alcohol and fat acids, potentiating the rigeneration of the hepatocyte and resolving the hepatosecretory function alterated.

Alcohol and marrow granulocyte reserve response to etiocholanolone.

Measurement of the size of marrow granulocyte reserve (MGR) was performed in ten alcoholic patients with clinically uncomplicated Laennec's cirrhosis and in ten healthy subjects by etiocholanolone injection before and during oral 95% ethyl alcohol administration (30 to 60 ml in orange juice every 6 hours for 48 hours). No significant differences in bone marrow response to etiocholanolone were found between normal subjects studied before and during alcohol administration. The group of cirrhotic patients failed to demonstrate a reduction of MGR when compared to control subjects and alcohol administration did not result in an abnormal response to etiocholanolone. It is suggested that the MGR is normal in most alcoholic patients with clinically uncomplicated Laennec's cirrhosis and is not influenced by acute alcohol ingestion. These results are in contrast to other previous reports in which bacterial endotoxin was used to estimate granulocyte reserve.
