Histaminergic H1 and H2 receptors do not mediate cortisol release in response to naloxone in normal men.

In order to evaluate the role of histamine as a possible mediator of the ACTH-cortisol response to naloxone, a specific opioid receptor antagonist, 12 normal men were treated with naloxone before and after the administration of dexchlorpheniramine and cimetidine, respectively H1 and H2 histamine receptor antagonists. Cortisol levels in the plasma were measured before and after drug injections. Naloxone significantly stimulated the secretion of cortisol in all subjects; the administration of dexclorpheniramine or cimetidine failed to modify this response. These data confirm the stimulatory effect of naloxone on cortisol secretion, but do not support the hypothesis that a histaminergic pathway mediates this response.

Remission continuation therapy for adult acute lymphoblastic leukemia. Preliminary results with a multidrug treatment protocol.

Twenty-four adolescent and adult patients with newly diagnosed acute lymphoblastic leukemia (ALL) were treated with the multidrug therapeutic program called IGG-74(9). Seventeen patients (71%) obtained a complete remission (CR) after induction therapy. They subsequently underwent a consolidation course with cytosine arabinoside and a seven drug maintenance regimen, lasting three years, with cyclophosphamide, 6 mercaptopurine, methotrexate, BCNU, adriamycin, vincristine and prednisone. The remission continuation treatment was well tolerated and produced an actuarial three-year disease-free survival rate of 57% and an overall median survival of 19 months. The efficacy of multidrug programs for the prevention of relapse in adults with ALL is discussed.

Myoadenylate deaminase deficiency in a 5-year-old boy with intermittent muscle pain.

A 5-year-old boy with occasional pain of brief duration at the right or the left leg during exercise since the age of 4 years is reported. There was no weakness nor any other abnormality at the neurological examination. The serum creatine phosphokinase activity was elevated three times out of four. The electromyogram showed myopathic abnormalities in the biceps, it was normal in quadriceps and anterior tibial muscles. A quadriceps muscle biopsy was performed. There were no histological nor histochemical abnormalities at the routine techniques. However, a completely negative reaction with myoadenylate deaminase ( MADA ) stain was found. The MADA enzymatic activity in the muscle was very low. It is likely that the clinical syndrome is related to MADA deficiency.

Naloxone does not alter the effect of gamma aminobutyric acid derivative, baclofen, on GH release in man.

To evaluate the interaction between opioid peptides and GABAergic system in regulating GH secretion we administered 5 mg of baclofen, a GABA derivative, to eight normal male subjects. The results were compared to those obtained in the same subjects treated with naloxone (10 mg/2 h) plus baclofen. GH levels increased significantly above basal value either after baclofen and naloxone plus baclofen without any significant difference between GH responses during the two tests. It is suggested that the two substances do not act at the level of the same receptor site. The evaluation of a possible interaction between opioid peptides and GABAergic system on GH release requires further investigation.

Binding of mannosylated ferritin to chicken bone marrow macrophages.

Ferritin conjugated to diazo p-aminophenylmannoside and mannan was used for ultrastructural visualization of binding and endocytosis via the mannose receptor. Conjugates were bound by live macrophages but not by glutaraldehyde-fixed cells. Binding was inhibited by 0.1 mM mannan and 0.2 M alpha-methylmannoside, and strongly reduced, but not abolished, after trypsin degradation of surface receptors. Binding sites were rapidly assembled on coated pits, which often showed a polar distribution on the cell surface and entered the cell following the direction of microtubuli. Some coated vesicles occasionally kept in touch with the extracellular space. 10-min exposure to the conjugate resulted in changes in the surface morphology, such as a loss of infoldings and increased membrane tension. Ferritin accumulated in large smooth vesicles with coated membrane regions. It was mostly detached from the membrane and agglutinated into big clumps, together with fibrillar material and small vesicles which may derive from the vesicle membrane.

Eosinophilic blast crisis in a case of chronic myeloid leukaemia.

A case of typical chronic myeloid leukaemia (CML) is described, of which the terminal blast crisis was characterized by an impressive proliferation of atypical eosinophils and by the simultaneous complete deficiency of neutrophilic myeloperoxidase. Since eosinophilic cells preserved a strong peroxidase positivity, a high number of immature non-granular eosinophilic precursors could be recognized among the leukaemic blast cells, thus supporting the diagnosis of eosinophilic blast crisis of CML.

Italian study of cerebral reversible ischemic attacks. II. The hematocrit and hemoglobin distribution and its relationship between clinical pattern and angiographic abnormalities.

A clinical and angiographic study was conducted in 360 patients with previous reversible ischemic attacks (RIAs) in order to verify the influence of hematocrit, hemoglobin and red cell count within the normal range in the clinical picture of cerebral infarction and its relationship with angiographic atherosclerosis. No significantly different distribution of the three parameters considered has been found according to the presence and the degree of atherosclerosis. On the contrary, our data have shown a significant correlation between levels of hematocrit, hemoglobin and red cell count and some clinical aspects of the RIA when angiographic atherosclerosis is present.

Detection of circulating immune complexes in acute non-lymphatic leukaemia: is it reliable?

The in vivo and in vitro phagocytic ability of leukaemic cells and the serum levels of circulating immune complexes (CIC) have been evaluated in 11 patients affected by acute non-lymphatic leukaemia (ANLL). High levels of serum CIC were detected in 17% of the cases showing phagocytic ability and in 80% of the cases lacking phagocytic ability (p less than 0.05). In 3 patients serum CIC determinations were negative while immunoglobulins with complement-fixing ability were detected in the cytoplasm of leukaemic cells indicating in vivo phagocytosis of CIC. These results suggest that leukaemic cells may sometimes interfere through their phagocytic ability on the detection of CIC in the serum. Therefore, the clinical and prognostic value of serum CIC in ANLL seems questionable.

Enlargement of the right descending pulmonary artery in pulmonary embolism.

Pulmonary embolism often is undetected during life, even though adequate diagnostic procedures are available, probably because the condition is not suspected. Chest radiographs were reviewed for changes in the right descending pulmonary artery in 73 patients with confirmed pulmonary embolism and in 85 in whom the original suspicion subsequently was not confirmed. In embolic patients, the vessel, measured at four different levels starting from the superior venous angle, was larger during acute embolization than some weeks later. In particular, the two proximal diameters of descending pulmonary artery were significantly enlarged (p less than 0.01). This enlargement of the superior part was responsible for the shape modification of the vessel, which appeared to taper off sharply. Some weeks later, the artery took back its regular cone shape, tapering gently. In about one-fourth of the patients with pulmonary embolism, enlargement and shape modification were so marked that the artery showed a special "sausage" appearance. This feature did not occur in patients with unconfirmed suspicion of pulmonary embolism. Recognition of enlargement of the descending pulmonary artery may increase suspicion of pulmonary embolism; in particular, detection of "sausage" appearance of the vessel should identify patients with high probability of disease.

[Double-contrast enema in the diagnosis of colonic diseases. Contribution on 3.500 examinations].

The Authors expose the technical aspects of the double contrast enema in the colonic diseases. They describe the patient preparation, the use of hypotonic agents and the radiographic technique. They discuss also the accuracy of the method and its diagnostic value. On the bases a four year program 3.500 patients were examined; the ichonography enclosed demonstrated the efficaciousness of this method, particularly in the diagnostic of small parietal lesions.
