Delayed oto-acoustic emissions evoked by bone-conduction stimulation: experimental data on their origin, characteristics and transfer to the external ear in man.

1) BCEOE have been obtained in subjects with normal hearing by 1 kHz tone-bursts; 2) the morphology of BCEOE varies from one subject to another, and is stable over the course of time. BCEOE do not display a linear relation between their amplitude and the intensity of the stimulus, whose spectral composition is the same as theirs; 3) by contrast with ACEOE, whose mean threshold is the same as that of the subjective tonal threshold for the same stimulus presented by the same stimulation modality, BCEOE threshold, on overage, is about 10 dB HTL higher; 4) this difference in threshold is not due to interference on the part of the controlateral ear, via the efferent fibres, since it is also observed in persons with unilateral anacusis. It is probably the outcome of unilateral competitive inhibition between two stimuli presented to the same ear at the same time. By bone-conduction stimulation in fact, when the meatus is occluded by the probe, as in our experimental situations, Corti's organ is reached by a supplementary contingent of mechanical energy generated by the vibration of the bony part of the meatus and transmitted by air conduction; 5) ACEOE cannot be obtained in otosclerotic subjects whereas they appear after surgery. BCEOE are obtained before surgery and increase in amplitude post-operatively; 6) the findings mentioned in point 5) clearly demonstrate that the ossicular chain plays an important, but not an essential role in the transfer of EOE from the inner to the external ear.

[Changes in hemochromocytometric tests in cancer patients].

The paper shows that haemochromocytometric alterations occur at a very early stage and even in symptom-free cancer patients. This was confirmed by the examination of about 100 cases of confirmed cancer as yet not subjected to surgical, antiblastic or radiation treatment.

Correlation between 5-year survival of T1-T2N0 breast cancer and some pathological parameters.

In order to more clearly define the factors influencing prognosis, 113 cases of T1-T2N0 breast cancers, all operated on by means of radical mastectomy, were reviewed. The following pathologic parameters were analyzed in relation to 5 years survival: histological type, nuclear grading, vascular embolization, size and border of the tumour, and sinus histiocytosis, diffuse cortical hyperplasia and germinal center hyperplasia of axillary nodes. We observed a close correlation between the presence of sinus histiocytosis and prognosis, as many authors have reported. Diffuse cortical hyperplasia stands out from our data as the most unfavourable prognostic factor.

Measurement of fetal surfactant production by fluorescence polarization of amniotic fluid in complicated pregnancies.

The results are presented of fluorescence polarization as a method for measurement of surfactant production in 159 specimens of amniotic fluid collected from pregnant women with diabetes, hypertension, Rh immunization, premature rupture of membranes (for more than 48 h prior to delivery) and intrauterine growth retardation (IUGR). The predictability of the development of respiratory distress syndrome has been assessed by this assay. Its specificity, sensitivity and overall accuracy were similar to the lecithin/sphingomyelin (L/S) ratio. The influence of the conditions detailed on fetal lung maturation was determined, lung development being enhanced until near term by Rh immunization, rupture of membranes and hypertension with IUGR.

Autosomal dominant microcephaly without mental retardation.

We describe six Italian families affected by microcephaly with an apparently autosomal mode of inheritance (total number of microcephalic children and adults, 21). All microcephalic subjects were of normal height, with the exception of one. The head volume was measured directly in at least one adult microcephalic member from five of the six families, and lower values were obtained in these subjects than in control subjects. Psychometric tests were performed on seven children and five adults among the microcephalic subjects. Normal values were found for all but one of the subjects. In the selected families microcephaly seems to be inherited in an autosomal dominant manner. Because some families showing autosomal dominant microcephaly have normal intelligence, psychometric evaluation of microcephalic children and their microcephalic relatives is important for genetic counseling.

Effect of flunarizine on pituitary secretion by healthy men and in woman with migraine.

Flunarizine is widely used in the prophylaxis of migraine. It is both a calcium blocker and a histamine antagonist at H1-receptors and either of these effects could alter hormonal secretion. The effect of administration of flunarizine to 8 women with common migraine on pituitary secretion has been studied. The dopamine antagonist domperidone (10 mg) and gonadotropin releasing hormone (100 micrograms) were injected iv before and after one month of flunarizine therapy (10 mg orally at bedtime). The basal prolactin level was significantly increased by the drug, and the peak induced by domperidone stimulation was reduced. Basal TSH concentrations were not affected, but the increase after domperidone was blunted. After 90 days of therapy there were no significant differences from the baseline concentration. Neither basal nor gonadotropin releasing hormone-stimulated secretion of FSH and LH were affected by flunarizine. Twelve healthy men were given placebo and flunarizine (10 mg at bedtime) for 5 days in single-blind fashion. Flunarizine caused a significant increase in prolactin and TSH with no effect on basal gonadotropin and thyroid hormone levels. These results can be accounted for by the calcium blocking effect of the drug, although weak interference with dopaminergic transmission is a further possibility explanation.

Italian multicenter study of reversible cerebral ischemic attacks. Part 5. Risk factors and cerebral atherosclerosis.

As part of a prospective study, the influence of several premorbid and environmental factors on the presence, extent and severity of cerebral vessel atherosclerosis was studied in 462 patients with clinical diagnosis of RIA who underwent cerebral angiography. The extent and severity of atherosclerosis of the cerebral vessels was quantified using extracranial and intracranial cerebrovascular scores (ECS, ICS) based on the number and severity of the lesions in 11 extracranial and 21 intracranial arterial segments. Results of univariate and multivariate analyses indicate that the presence of atherosclerotic changes of cerebral vessels, as shown by angiography, was strongly related with age in both sexes. The lesions were more frequent in males, in particular under age 55. Elevated cholesterol was associated with a higher incidence of atherosclerotic lesions. Smoking was associated with a higher incidence of extracranial lesions. Age, smoking and history of hypertension were the best predictors of the extent and severity of cerebral vessel atherosclerosis.

Continuous epidural analgesia in the heparinized vascular surgical patient: a retrospective review of 912 patients.

Nine hundred twelve patients received continuous epidural analgesia administered through an indwelling plastic catheter while undergoing vascular reconstruction of a lower extremity. During a portion of the operative event, the patients were totally, transiently anticoagulated with heparin. None of the patients had an untoward neurologic event that could be attributed to an epidural hematoma. Our results and those of others show that this form of regional analgesia is safe and far outweighs theoretic contraindications when anticoagulation with heparin is planned as a part of the operative event. In the patients with impaired pulmonary ventilation or a cardiac disorder, this method of analgesia offers many advantages over a general anesthetic, such as obviating aspiration pneumonitis and averting prolonged support in the recovery period after completion of the surgical procedure. The regional vasodilation ensuing from the epidural blockade is an additional advantage in patients undergoing vascular reconstruction for lower extremity ischemia.

Orthotopic transplantation of partially hepatectomized liver in the pig.

One of the major technical obstacles to liver transplantation in children is to find a liver of appropriate size because of the rarity of child donors. To overcome this difficulty an experimental study was carried out using only a portion of the donor liver (right liver) transplanted orthotopically in pigs. A group of 15 allotransplants were performed. A left hepatectomy of the liver graft was performed ex situ and the right liver amounted to 55% of the whole liver. A total of 13 animals survived for more than 5 days (5 to 30 days, with an average of 16). Upon killing, the liver weight was considerably more than that of the part transplanted. The absence of technical complications suggests that this procedure is safe and feasible.

Muscarinic cholinergic and histaminergic H1 and H2 receptors are not involved in the LH response to naloxone in man.

The role of muscarinic-cholinergic and H1-, H2-histaminergic receptors as possible mediators of the LH response to the opioid antagonist naloxone was evaluated in 18 normal men. Subjects were divided in 3 groups of 6 men; the increment of LH in the plasma elicited by naloxone was evaluated after giving naloxone alone or together with dexchlorpheniramine, cimetidine or pirenzepine (respectively H1-, H2-histaminergic and muscarinic-cholinergic receptor antagonists). LH release was significantly stimulated by naloxone in all subjects; this response was not altered by histaminergic or cholinergic blockade. These results confirm the stimulatory effect of naloxone on LH release in man, without evidence of the involvement of H1-, H2-histaminergic or muscarinic-cholinergic pathways.
