Carcinoma of the prostate irradiated by combined I125 and external irradiation. Analysis of failure and significance of positive biopsy one year or more after therapy.

Sixty-three patients with cancer of the prostate T2 or T3 were evaluated. The protocol of treatment called for pelvic lymphadenectomy, 10,000 rad from I125 implant and 4000 rad in 20 fractions using a Cobalt60 machine. They were followed for 1 to 5 years with a plan to rebiopsy the prostate 1 to 2 years after therapy. Six of 59 evaluable patients (10%) showed progressive disease. Distinctive prognostic features in the failure group were younger age, larger prostate, more advanced stage, poorer differentiation, more possibility of positive pelvic lymph nodes, and if the nodes were positive, the involvement of more than two pelvic lymph nodes. On the other hand, the patients with controlled disease with or without positive prostatic biopsy on follow-up showed identical features regarding age, size of prostate, stage, differentiation, involvement of pelvic lymph nodes, and if the nodes were positive, only one or two nodes involved. Positive biopsy 1 to 2 years after radical irradiation in otherwise controlled disease is considered of no prognostic value.

Growth achievement in low-birth-weight premature infants: relationship to neurobehavioral outcome at one year.

Previously published studies present conflicting conclusions about the growth of small, premature infants. To provide further data on these infants, a well-defined group of 86 premature infants appropriate for gestational age, weighing less than 1501 gm at birth, was observed until 12 months after their expected dates of delivery. Length, weight, and head circumference were measured at one, three, six, nine, and 12 months postterm. At 12 months postterm, infants were evaluated by neurologic examination and the Bayley Scales of Infant Development. Premature infants with mean Bayley scores greater than or equal to 85 were found to be significantly larger than infants with mean Bayley scores less than 85 by three months postterm. Birthweight, gestational age, duration of hospitalization in the neonatal intensive care unit, pneumonia, hypoglycemia, perinatal neurologic impairment, acidosis, and hypoxemia were found to be associated with reduced growth at one year. Conflicting results in previous studies may be a result of failure to separate infants with good neurobehavioral outcome from those with poor outcome.

Preliminary observations on the results of combined 125iodine seed implantation and external irradiation for carcinoma of the prostate.

Fifty-seven patients with localized carcinoma of the prostate were treated with pelvic lymphadenectomy and a reduced 125iodine implant dosage, supplemented by a moderate dose of external beam radiotherapy to the whole pelvis delivered 4 to 6 weeks later. The incidence of pelvic nodal metastases was 28 per cent and the operative morbidity was 15 per cent. Late radiation sequelae developed in 18 patients, including 15 patients with radiation proctitis (29 per cent), among whom 2 (4.6 per cent) suffered rectal ulceration and required diverting colostomy. Followup has been 2 years or longer (median 33 months) in 26 patients, of whom 22 (85 per cent) are free of disease. Three patients are living with osseous metastases or local disease and there has been 1 death of prostatic carcinoma, for an absolute 2-year survival rate of 95 per cent. Of the 7 patients with poorly differentiated tumor and of the 8 patients with positive pelvic lymph nodes 5 and 6, respectively, remain free of disease after a minimum 2-year followup. Potency has been lost in 20 per cent and reduced significantly in 30 per cent of the patients followed 18 months or longer. Prostatic biopsies on 28 asymptomatic patients 12 to 30 months after completion of therapy showed no tumor in 21 (75 per cent).

Emphysematous pyelonephritis in a solitary kidney.

A middle-aged female diabetic presented with ureteral obstruction, renal failure and emphysematous pyelonephritis in a solitary kidney. She was treated non-operatively with intensive antimicrobial therapy, prolonged ureteral catheter drainage and dialysis. Good recovery of renal architecture and function was eventually obtained. Although the usual indications for surgical extirpation or drainage of the kidney with emphysematous pyelonephritis seem to be well established circumstances may occasionally dictate an alternative course of action.

Level of psychological functioning as a predictor of psychiatric rehabilitation outcome.

Traditional psychological measures, diagnostic levels, and biographical and demographic data have all failed as reliable predictors of the outcome for clients in psychiatric rehabilitation programs. An instrument called the Client Adjustment Rating Scales is an attempt to develop a reliable indicator; it consists of nine subscales on which client's attitudes and behaviors in such psychosocial areas as motivation, self-concept, and family functioning can be rated by clinicians. Analysis of the ratings of 89 clients from Horizon House, a community-based psychiatric rehabilitation program in Philadelphia, shows that those who successfully complete rehabilitation programs have higher (more positive) scores on the scales, while program dropouts have lower scores. The authors conclude that after further testing the CARS may be used both as a reliable predictor of client outcome and as a tool for planning rehabilitation programs.

Differential protection against cytotoxic chemotherapeutic effects on bone marrow CFUs by Wr-2721.

This paper reports on the ability of the radioprotector WR-2721 to protect bone marrow colony forming units from the cell kill of various cytotoxic chemotherapy agents including nitrogen mustard, cyclophosphamide, BCNU, cis-platinum, and 5-fluorouracil. The dose-modifying factors seen with these compounds ranged from 1.5 to 4.6. Protection by WR-2721 seems to be selective for the bone marrow endpoint, in that similar protection against EMT6/SF tumor cell kill was not seen with the same combination of the cytotoxic chemotherapy agents and WR-2721 as measured by regrowth delay. The implications of these data are discussed in the paper.

Absorbable suture materials for vascular anastomoses. Tensile strength and axial pressure studies using polyglycolic acid sutures.

This study attempted to evaluate how vascular healing itself is sufficient enough to provide adequate strength to autogenous vein grafts in small arteries without the use of nonabsorbable sutures. The absorbable suture, Dexon, was used in small canine femoral vein graft anastomoses over a two-month trial period with nonabsorbable Prolene as a control. Tensile strength and resistance to axial leak were measured. Results show that Dexon had 93 per cent of the tensile strength of Prolene immediately; 76 per cent at 14 days, 111 per cent at 21 days, and 110 per cent at 30 days, and 124 per cent at 60 days as an average for all suture lines. The per cent mean resistance to leakage that Dexon had compared to Prolene for all suture lines was 102 per cent at seven days, 97 per cent at 14 days, 107 per cent at 21 days, and 97 per cent at 30 days. There is no evidence of aneurysms, suture line disruptions, dilations, or infections of any anastomoses. Our ultimate conclusion is that the integrity of small autogenous vein graft is dependent on healing rather than on the permanent strength of the suture material and that Dexon maintains vascular integrity long enough to permit this healing to occur.
