Fetal blood flow velocity waveforms as predictors of growth retardation.

Seventy-five high-risk pregnancies were studied in order to define the clinical value of the analysis of fetal blood flow velocity waveforms in early screening for growth retardation. Recordings were obtained at 26-28 weeks' gestation, in the absence of ultrasonographic signs of growth retardation, using a pulsed duplex Doppler system. The pulsatility index was evaluated at the level of the umbilical artery, descending aorta, and internal carotid artery. Fetuses (N = 23) who developed growth retardation showed higher values of pulsatility index in the umbilical artery (P less than .001) and descending aorta (P less than .05) than fetuses of normal growth. In the internal carotid artery, the pulsatility index was lower (P less than .001) in the fetuses who developed growth retardation than in those with normal growth. The ratio between the pulsatility indexes of the umbilical and internal carotid arteries proved an accurate predictor of growth retardation (specificity 92.3%; sensitivity 78.2%; positive predictive value 81.8%; negative predictive value 90.5%; accuracy 88%).

Plasma protein binding of dicloxacillin: effects of age and diseases.

Dicloxacillin protein binding was investigated in sera from 10 umbilical cords, 20 children (aged between 2 and 21 months) and in the plasma from 8 healthy young subjects, 7 healthy elderlies, 10 patients with liver cirrhosis, 10 patients with renal failure (glomerulonephritis) and 10 chronic uremics maintained on hemodialysis. The percentage unbound fraction (mean +/- SD) of dicloxacillin was 7.3 +/- 0.8 (healthy subjects), 9.8 +/- 0.6 (umbilical cords), 7.4 +/- 2.8 (children), 8.8 +/- 1.0 (elderlies), 11.8 +/- 6.3 (cirrhosis), 10.5 +/- 2.0 (renal failure), 12.7 +/- 2.0 (before hemodialysis) and 11.7 +/- 2.1 (after hemodialysis). Healthy subjects were different from all the groups (0.001 greater than p greater than 0.05) except children (p greater than 0.5) (Student's t-test). Human isolated albumin (45 g/l) bound dicloxacillin at the same degree as the plasma of healthy subjects. The kinetics of dicloxacillin protein binding was studied in three umbilical cord serum, three adult plasma specimens and in human serum albumin. The number of binding sites (n) and the association constant (k) were estimated by Lineweaver-Burk double reciprocal plot.(ABSTRACT TRUNCATED AT 250 WORDS)

Effects of development, aging, and renal and hepatic insufficiency as well as hemodialysis on the plasma concentrations of albumin and alpha 1-acid glycoprotein: implications for binding of drugs.

The concentrations of total proteins, albumin, alpha 1-acid glycoprotein (AGP), and nonesterified fatty acids (NEFA) were measured in the serum of 21 newborn infants and 13 children, aged 1-13 months, and in the plasma of 31 volunteers, 25 patients with renal failure, 27 patients with cirrhosis, 39 uremic patients undergoing hemodialysis, and 20 elderly subjects. The concentration of albumin in the volunteers was higher than in all other groups. The concentration of AGP in the volunteers was higher than in newborn infants but lower than in elderly subjects, patients with renal failure, and those with chronic uremia. The concentration of NEFA in volunteers was higher than in newborn infants and patients with renal failure and lower than in elderly subjects and patients with cirrhosis.

HLA antigens in Italian patients with primary Sjögren's syndrome.

Twenty eight Italian patients with primary Sjögren's syndrome were typed for class I and class II alloantigens of the major histocompatibility complex. Patients with Sjögren's syndrome had higher prevalence of DR3 (46.4% v 14% in the control population, p corrected less than 0.02), while similar prevalence was found for DR2 and DRw52 alloantigens. DR3 correlated with DRw52 (p less than 0.0001), anti-Ro(SSA) (p less than 0.0002) and anti-La(SSB) (p less than 0.02) antibodies, and extraglandular manifestations (p less than 0.02). In addition, extraglandular involvement was associated with anti-Ro antibodies (p less than 0.05) and raised gammaglobulins (p less than 0.02). In Italian patients with primary Sjögren's syndrome DR3 is the genetic marker related to this clinical entity and seems to identify a disease subset characterised by autoantibody production and extraglandular manifestations.

Effects of free fatty acids and L-carnitine on the spontaneous motility of rat isolated uterine horn.

In the present investigation the effects of free fatty acids (FFA) and L-carnitine were investigated on the rat isolated uterine horn. The cumulative addition of FFA and L-carnitine to the isolated organ bath produced a significant increase of isometric developed tension and functional activity, especially evident at 5 mM carnitine concentration. No variations in the frequency of contractions were found. These data suggest a possible role of FFA as energy source in the rat uterus.

A new method for the measurement of pre-ejection period in the human fetus.

The pre-ejection period (PEP) of the cardiac cycle was measured in the human fetus using pulse Doppler in association with transabdominal ECG. The technique permitted the exact localization of the aortic valve, a good recording of its opening and the exact measurement of PEP. Between 32 and 42 weeks of pregnancy there is a strong correlation between PEP and gestational age. This new method seems to be an easy and reliable tool for measuring the PEP during intrauterine life.

True precocious puberty: a long-term complication in children with shunted non-tumoral hydrocephalus.

In 4/37 (10.8%) children, adolescents and young adults with successfully shunted hydrocephalus, puberty occurred or was occurring precociously, at an age ranging from 7.5 to 8.6 years, with a consequent impairment of their effective or predicted adult height as compared to the familial target height. All four patients had undergone a surgical intervention for the insertion of a ventricular-atrial or a ventricular-peritoneal shunt during the first year of life; since the last surgical shunt revision (at the age of 5 years) no relapse of hydrocephalus had been recorded. The authors conclude that precocious puberty is to be regarded as a not infrequent long-term complication in patients with successfully shunted hydrocephalus.
