Distal renal tubular acidosis in hepatic cirrhosis: clinical and pathogenetic study.

The mechanism of the renal tubular acidosis (RTA) occurring in patients with hepatic cirrhosis remains uncertain although it has been suggested that renal and intrarenal hemodynamic alterations could play a role in its pathogenesis. To verify this hypothesis, renal acidification was studied with an intravenous acid load of arginine hydrochloride in 51 patients with cirrhosis due to various causes. In 22 patients renal and intrarenal blood flow was also measured using the 133-Xe washout technique. RTA was found in 17 of 51 patients (33%) with the greatest incidence in alcoholic cirrhosis. The tubular defect did not appear related either to the degree of liver functional impairment or to the renal and intrarenal hemodynamic alterations.

Post-transfusion hepatitis: antigen/antibody systems correlated with non-A, non-B hepatitis.

In seven patients post-transfusion hepatitis (PTH) was due to non-A, non-B virus(es) (38.9% of all PTH, while 61.1% were due to hepatitis B virus (HBV). No clinical or biochemical differences were observed in non-A, non-B PTH when compared with PTH due to HBV, while incubation period was very ample, from 15 days to nine months (generally 45 days to two months). An antigen/antibody system was shared by five of our patients (their sera showed precipitin lines when assayed by immunodiffusion with known sources of antigen or antibody), while in one patient an antigen/antibody system was detected when onset serum was assayed with self-recovery serum but not when assayed with known sources of antigen and antibodies, nor with sera of the other five patients. Antigen was detected during the first weeks of illness, antibody at recovery, for both systems. The results suggest that there may be at least two antigen/antibody systems correlated to non-A, non-B hepatitis not necessarily linked to incubation period.

Individual serum bile aids in patients with primary hyperlipoproteinemias.

Fasting serum concentrations of the individual bile acids were measured by gas chromatography in 27 patients with primary hyperlipoproteinemia (8 type IIa, 7 type IIb and 12 type IV) and in 14 healthy subjects. Total serum bile acid levels were 1618 +/- 244 ng/ml (SE) in type IIa, 1296 +/- 251 ng/ml in type IIb and 15609 +/- 263 ng/ml in type IV hyperlipoproteinemia. These values did not differ significantly from values in the control group (1505 +/- 200 ng/ml). Serum levels of cholic acid were significantly higher in patients with type IIa (551 +/- 78 ng/ml) than in those with type IIb (190 +/- 57 ng/ml, P < 0.01) and type IV (240 +/- 57 ng/ml, P < 0.02), while intermediate values were recorded in the control group (384 +/- 49 ng/ml). Ursodeoxycholic acid was found in larger amounts in hyperlipidemic patients than in controls. No significant differences with respect to other bile acids were observed between the groups examined. According to the current concepts on the enterohepatic circulation of bile acids, the findings support the hypothesis that the intestinal absorption of cholic acid may differ in various types of hyperlipoproteinemia.

Circulating platelet aggregates in an adult population sample. Relationships with the main coronary risk factors and cardiovascular diseases.

We have determined the reversible circulating platelet aggregates (CPA) in a sample taken from the general population (414 consecutive individuals of both sexes, aged 20--60 years), examined during the initial screening of a multifactor primary prevention trial for coronary heart disease. Our findings show that in the overall population, less than 10% of circulating platelets occur as reversible aggregates. No differences were observed with regard to sex, age or presence of non-acute cardiovascular disease. No significant correlations were found between the extent of CPA and the main coronary risk factors.

Red blood cell fatty acid composition in a sample of Italian middle-aged men on free diet.

Within the framework of a project of primary prevention of coronary heart disease the erythrocyte fatty acid composition was determined in 140 Italian middle-aged men on free diet. The linoleic/oleic ratio ranged from 0.27 to 0.96 pointing out remarkable differences in dietary fat intake. A negative correlation was observed between erythrocyte linoleic/oleic ratio and plasma cholesterol levels. This gives indirect support to the validity of this ratio as an index of adherence to cholesterol-lowering diets in long-term studies.

Erythrocytes and "vinca rosea" alkaloids. An in vitro study.

The reported stereoscan images have proved that overtherapeutic amounts of vincristine induce in normal red cells the same effects as vinblastine, i.e. the onset of spherocytosis (in about 30% of the cells) and spherostomatocytosis (in the remainder) associated with distinct smaller vacuoles surrounding the larger cavity. The lowest deformation-inducing doses have been found to be twice as high for vincristine as for vinblastine (0.6mM); this is probably due to the greater interaction of the latter drug (because of its methyl group) with the bilayer phospholipids. Unlike previous reports, the present findings have revealed that the onset of red cell deformation is energy independent, since the same aspects have been recorded in both young and old erythrocytes. Therefore, such deformations must be considered the morphological consequence of a drug-induced mechanical derangement of the membrane subunits. In drug-incubated normal erythrocytes it was found that (a) neither drug exerts any influence on the intraglobular energy production and/or utilization, (b) osmotic fragility is enhanced, and (c) potassium leak is increased. These findings may be considered convincingly consistent with the assumption of a drug-induced mechanical derangement of the bilayer components.

Blood lipids in Italy. Regional differences.

Serum cholesterol and triglyceride levels have been evaluated in samples from fasting males aged 20--59 in Northern (Brisighella), Central (Rome) and Southern (Pozzuoli) Italy. Regularly performed quality controls between laboratories assured comparability of data. A statisitically significant difference of mean serum cholesterol and triglyceride levels was observed for most age-groups in the 3 different areas, lower values being found in the southern population as compared to the central and northern ones. These results support previous findings and the thesis that large differences in blood lipid levels may still exist even within the same country and that they at least in part may be culturally determined in connection with different dietary habits.

Enzymatic oxidation of mercaptoethanol to isethinic acid and isethionic acid.

The enzymatic oxidation of mercaptoethanol by purified cysteamine oxygenase has been studied. Products were identified by chromatography as isethinic acid (2-hydroxyethan-sulfinic acid) and isethionic acid. Other features of the reaction, as cofactor requirement, the influence of the enzyme concentration on the stage of oxidation, the biological significance of this reaction are also discussed.
