Synovial membrane in rheumatoid arthritis: determination of glycosaminoglycans and age-dependent correlations.

From 16 synovial membranes patients with rheumatoid arthritis morphological parameters as well as the GAG distribution pattern have been determined using 2 methods each. The results were checked by discriminant and regression analysis: The total GAG concentration of the synovialis was significantly correlated with the sum of the parameters "fibrin in organization", "granulocytes" "nad "necroses" (acute activity). A highly significant correlation between the age of patients and the severity of chronic morphological alterations (basic activity, especially proliferation) was found. The percentage of CS plus DS was positively correlated with the age of patients as well as with the basic activity of the disease, especially with the proliferation. - Contrary to this, HA was significantly decreased when related to the "chronic" parameters. No correlation could be demonstrated between the GAG components and criteria of the acute activity. The evaluating system of STIEHL and GEILER is to be preferred for obtaining morphologic-biochemical correlations. Furthermore, a diagram is proposed demonstrating the cumulative progression of rheumatoid arthritis in the synovialis as related to the GAG pattern. Its combination with morphological data (scars, proliferation, exudation) may help to describe the development of the disease more acurately.

Action of chronic peracetic acid (Wofasteril) administration on the rabbit oral mucosa.

Peracetic acid Wofasteril) is increasingly made use of as a disinfecting fluid. Because of its high effectiveness it has found widespread use in medical practice. The action of Wofasteril on the oral mucosa during long-term action is studied by aid of an "oral tank". After a test period of 11 months peracetic acid only caused a slight ballooning of the superficial layers and enhanced vascularization of the submucous tissue. The results of the experiment suggest the intraoral administration of peracetic acid in oral surgery and stomatology.

[Value and results of coloscopic polypectomy].

From August 1976 till the end of February 1979 158 polyp in 83 patients (48 male and 35 female) were removed by means of the rectocoloscopic polypectomy with the high frequency diathermy loop. 66% of the polypi had a diameter of less than 1 cm. Most the polypi were with about 70% in the sigma and in the rectum. With 60.7% the adenomas appeared most frequently, followed by the hyperplastic polypi in 20% and by the inflammatory ones in 13.3% of the cases. With the increase of size of the polypi the proportion of the adenomas clearly increased in comparison to the other kinds of polypi. Severe epithelial atypias (altogether 23.1%) were more frequently in adenomas larger than 2 cm than in smaller ones. An invasive carcinoma was found in no polypus. Thus the endoscopic polypectomy in every case meant a curative therapy with great economic use. In patients a haemorrhage appeared as a complication of the coloscopic polypectomy. Only the removal of the polypi in toto allows an exact histological classification and the answer of the decisive question, whether in one adenoma only severe epithelial atypias are present and thus the polypectomy is therapeutically sufficient or whether there exists an adenocarcinoma, for the removal of which an operation according to the principles of the carcinoma surgery must be carried out. The endoscopic ablation of the adenomas thus means a genuine prophylaxis of cancer.

Leukoplakia induced by repeated deposition of formalin in rabbit oral mucosa. Long-term experiments with a new "oral tank".

Rabbits were exposed repeatedly to formalin irritation using a new oral tank which allows a chronical contact with the oral mucosa. In long-term experiments (11 months) a preneoplastic unrest of the epithelium with dyskeratosis could be produced. This new model of "oral tank" can be used for soluble and insoluble test substances for analyzing pathogenetic processes in the oral cavity.

[Lectin arthritis--a new arthritis model].

By single intraarticular injection of lectin from Lens culinaris in nosenitized rabbits acute and chronic arthritis was produced. By use of fluoresceinisothiocyanate-labelled lectin binding of lectin could be demonstrated in the connective tissue of joint capsule for 4 weeks. The morphological features of lectin-arthritis were described. The results are discussed with respect to the pathogenesis of the arthritis.

Experimental allergic serous rabbit endocarditis and cardiav valvular lesions in rheumatoid arthritis. --A comparative study.

Fibrosis of the chordae tendinae pathogenetically comparable to the rheumatoid arthritis (RA) conditioned alterations in the chordae tendineae was to be induced experimentally using the model of allergic-hyperergic rabbit endocarditis. 2. Furthermore, 45 hearts of RA autopsy cases were compared with 200 control heart. All heart valves and chordae tendineae were examined for occurence of diffuse fibroses, adhesions and rufflings as well as for ulcerations and thrombi. The degree of the pathological changes was classified in ""slight'', ""mean'', and ""severe'' (see table 1). 3. The tissue lesion can either lead to endothelial injury resulting in verruciform endocarditis or it can change over from recurrent edema with subsequent edematous sclerosis to etiologically ambiguous nonspecific fibrosis. 4. The persistent allergic-hypergic irritation in rabbit endocarditis likewise caused fibrosis of the chordae tendineae. During the progress of the disease the lesions are ranging from acute edema over edematous sclerosis to fibrosis. In the relatively short experimental period adhesions of the chordae tendineae and sheat-like depositon of newly formed connective tissue as common in allergic processes in RA were not observed. The pathogenetic reactions due to experimental endocarditis were comparable with those in RA.
