Bowenoid lesions, Bowen's disease and keratoacanthomas in long-term PUVA-treated patients.

Four patients are described, two with histologically proven multiple foci of bowenoid lesions, one patient with bowenoid lesions and Bowen's disease, and one patient with two keratoacanthomas, most of the lesions occurring in non sun-exposed but photochemically treated areas of the body. These patients had received long-term PUVA treatment for their psoriasis. Two patients had a history of arsenic intake. The possible relationship of these epidermal lesions to light treatment is discussed.

[Pathological-anatomical teaching models in moulages. Manufacturing technic and history].

Wax moulages were used as pathological-anatomical teaching objects in medicine well into this century. On the occasion of the restauration of the moulage collection of the Department of Dermatology, University of Munich, the history of Munich wax artists is reviewed. The historical development of reproductions in wax in previous centuries is described. Several moulages showing various dermatological diseases are pictured. The technical details of moulage manufacture are mentioned.

[Superficial cutaneous nevus lipomatosus (Hoffmann-Zurhelle)].

Neviform hand-sized yellowish or skin colored plaques or linear lesions with flattened papules or folded surface in the gluteal regions, especially in females, point out to nevus lipomatosus superficialis (Hoffmann-Zurhelle). These nevi consisting of mature fat cells among the dermal collagen are rare lesions. The case report of two female patients should remind of this clinical and histological picture.

[Pityriasis lichenoides chronica--a new indication for PUVA therapy? (author's transl)].

2 patients with long-standing and wide-spread pityriasis lichenoides chronica were treated with orally administered 8-methyoxypsoralen and UVA irradiation (PUVA therapy). After 15--21 PUVA treatments the patients were completely cleared. 3 and 4 weeks after the initial PUVA treatment, phase recurrences occurred in both patients. In 1 patient the lesions appeared clinically and histologically as pityriasis lichenoides et varioliformis acuta.

[Nevus of the arrector muscle of the hair].

An eighteen year-old-male patient had an asymptomatic nevoid lesion with small papules over the right lumbosacral region. The nevus was diagnosed histologically as a nevus musculi arrector pili, the origin of which is seen in a nevoid hyperplasia of the smooth muscle bundles (mm. arrector pili) connected with hair follicles.

[Photoaugmentation. a photobiological phenomenon (author's transl)].

The phenomenon of photoaugmentation was investigated in 37 male and female patients. UVA, UVB and UVC was applied with a high intensity grafting monochromator. Photoaugmentation could be induced following UVA plus UVB immediately or dissociated up to 3 h, but not after 24 h (15 J/cm2 lambda 351 less than or equal to 389 nm [UVA] + 13 mJ/cm2 lambda 290 less than or equal to 310 nm [UVB] = 1/2 MED). Simarily the reversed application of UVB plus UVA lead to photoaugmentation. With UVA plus UVC, photoaugmentation could not be elicited in every patient. Moreover, photoaugmentation was demonstrated in albino rabbits using a new light apparatus with a liquid filled light conducting core. Photoaugmentation seems to be an important biological phenomenon in association with sunburn, pigmentation, photoxicity, phototherapy, photochemotherapy, and UV-light induced cutaneous premalignanacies and malignancies.

Photoallergic dermatitis from 8-methoxypsoralen.

A 36 year old woman with psoriasis vulgaris developed generalized photoallergic dermatitis to 8-methoxypsoralen after 16 uneventful treatments with 8-methoxypsoralen und UVA (PUVA). The diagnosis of photoallergy was confirmed by re-exposure to oral 8-methoxypsoralen and total body UVA irradiation; phototests using topical and oral 8-methoxypsoralen with a high intensity monochromator or with a new high intensity light apparatus for the delivery of UVA; and histological studies. Photoallergy occurred only with UVA, but not with UVB or UVC. There was no photoallergy following trimethylpsoralen.

Circumscribed sebaceous gland hyperplasia: autoradiographic and histoplanimetric studies.

Circumscribed sebaceous gland hyperplasia (sebaceous hyperplasia) is a common benign facial tumor found in adults and aged. Seventeen biopsies of sebaceous hyperplasia from 10 patients, females and males aged 57-87 years were secured which were injected 45 min or up to 15 days prior with tritiated thymidine, and prepared for autoradiography. Specimens were evaluated by cell counts (labeling index, labeled total cells), and histoplanimetric techniques (sagittal section areas and circumference of sebaceous hyperplasia, of sebocytes, and of sebocyte nuclei). Controls were uninvolved sebaceous follicles from the same subjects, from an age-matched group, and from young male adults. In sebaceous hyperplasia the labeling index of basal cells is low with 5.7% +/- 2.3 vs uninvolved sebaceous follicles from the same subject (8.6% +/- 2.4) and the younger subjects with a value of 10.1% +/- 2.0). Labeled cells are retained in the acini much longer than in young adults (slowed down transit and turnover time). The glands of sebaceous hyperplasia are very large, their sebocytes are small, and many more basal cells are found per unit basement membrane length.

A new apparatus for the delivery of high intensity UVA and UVA+UVB irradiation, and some dermatological applications.

A new apparatus consisting of a 200 W super pressure mercury lamp and a flexible light guide with a liquid light-conducting core is presented. Its transmission for UVA irradiation is more than threefold as compared to conventional quartz fibre optic bundles. The light guide is capable of transmitting density levels exceeding I W/cm2. UVA and UVB are transmitted. By inserting a filter platelet (WG 345) into the light guide the UVB is cut off. The output of the unfiltered guide is 700 mW (3570 mW/cm2), of the filtered guide 400 mW (2040 mW/cm2), centered mainly at 366 nm. The light apparatus proved useful for several dermatological applications. It replaces in many respects conventional Kromayer lamps, especially because the flexible light guide, which is 160 cm long and weighs only 210 g, can be brought into direct contact with the skin. Results of determining the minimal erythema dose (MED), of phototesting (8-MOP photoallergy, persistent light reactor, erythropoietic protoporphyria) and of photochemotherapy are given.
