The positive identification of crystals of calcium oxalate occurring in brain and kidney after xylitol administration is described. Biochemical, histochemical, conventional light and electron microscopical methods, including selected area electron diffraction, were used to characterize the crystals.

Scopulariopsis brevicaulis is a common soil saprobic mould. Occasionally, S. brevicaulis has been reported as an etiologic agent of human disease. The spectrum of human mycoses includes toenail infection, hypersensitivity pneumonitis, fungus ball formation in a preformed pulmonary cavity, and, most recently, documented deep tissue infections in immunocompromised hosts. A case of invasive hyalohyphomycosis of the great toe caused by S. brevicaulis is reported in an allogeneic bone-marrow transplant recipient.
When elements of a parallel network, such as the human brain, are extensively interconnected, the network can exhibit 'cooperative behaviour'. Such behaviour, which is characterized by order-disorder transitions, multi-stable states, and a form of memory called 'hysteresis', has been observed in human stereopsis and has motivated models of stereopsis that incorporate cooperative networks. More recently, cooperative phenomena have also been observed in human visual motion perception. This report strongly supports a cooperative interpretation of motion perception by demonstrating hysteresis in the perception of motion direction. The results agree quantitatively with a mathematical model incorporating nonlinear excitatory and inhibitory interactions among direction-selective elements.

Adult cystic fibrosis (CF) patients are increasingly diagnosed with hepatic cirrhosis, cholecystitis, and cholelithiasis. A continuing diagnostic problem is the early detection of cirrhosis prior to diffuse liver involvement. Sonography has been used in evaluating the pancreas, gallbladder, liver, and spleen in cystic fibrosis patients. We used a real-time mechanical sector scanner to study the portal veins in adult CF patients randomly selected from our CF population and correlated the portal vein diameters with liver function studies. A measurement greater than 12 mm was interpreted as a probable sign of portal hypertension. Of 21 patients studied, 14 had portal veins that measured greater than 12 mm, and 12 patients had (although not necessarily at the time of the exam) elevated serum alkaline phosphatase levels. Three patients had associated splenomegaly and thrombocytopenia, and 10 patients also exhibited cholelithiasis and abnormal gallbladders on sonography. Initial results indicate that portal vein measurement may be a sensitive indicator of early portal hypertension. Sonography may, thus, isolate patients at high risk for possible future complications. Serial sonograms can be performed easily in evaluating the progress of liver disease.
A system is described for the stabilization and evacuation of the critically ill in South Australia, with a discussion of experience in 278 patients. The integration of activities of peripheral medical practitioners, major teaching hospitals, and the ambulance transport authority (St John Ambulance Council) is paramount. Modes of communication are by "urgent line" telephone and radio. Advanced life-support equipment is portable, may be placed in any ambulance vehicle, and is used and maintained principally by the staff of intensive care units. The design characteristics of a suitable road vehicle and fixed-wing aircraft are considered, with emphasis on dimensions for the particular needs of advanced life-support. The cost of transporting such a patient is approximately two-thirds of an average bed day cost in a public hospital, plus the basic ambulance charge. The number of patients who need such measures is 0.2% of the total number of patients carried by ambulance per annum.
