Relation between early parental death and life event ratings among 70-year-olds. A test of a sensitization theory.

A subsample of 151 subjects from the population study "70-year-olds in Gothenburg" were given a questionnaire for life event ratings to be returned by mail. The 24 life events to be rated were events that are fairly common in advanced age. The subjects were instructed to record how an event would affect them personally. The ratings of subjects who had lost a parent by death before they were 16 were compared with those of subjects who had grown up together with both parents until they were 16. Bereaved men had lost their parent at 6.9 years of age and bereaved women at 9.9 years on average. Bereaved men attributed significantly less weight to five of the events and there were tendencies in the same direction for another three events. Bereaved women, on the other hand, attributed similar weights to the events as non-bereaved women. As possible explanations for the relation between bereavement and life event ratings, it is suggested that early bereavement might (a) cause a change in personality, with avoidance of involvement, (b) lead to a strong development of the defence mechanism denial, or (c) result in a very high standard of comparison.

Psychogenic needs in a 70-year-old urban population. Norms for the CMPS and sex and age relations.

Psychogenic needs according to Murray were measured by means of the Cesarec-Marke Personality Schedule on representative samples of men and women taking part in a health control in Gothenburg, Sweden. Means, standard deviations, stanine scores and a comparison with results from 50-year-old men and women and with 16-year-old boys and girls with regard to sex differences are presented. Men in all three age-groups scored higher than women on achievement and dominance, and women in all three age-groups scored higher than men on nurturance and sociability. Seventy-year-old men and women scored lower than the 50-year-olds on exhibition and higher on order and acquiescence. The difference between the sexes was smaller in 70-year-olds than in 50-year-olds with regard to nurturance. The standard deviations were generally smaller in 70-year-olds than in 50-year-olds. Four out of the 11 subscales had distributions that were skewed in the 70-year-olds.

Personality dimensions in a 70-year-old urban population. Norms for the MNT scales and sex and age relations.

The three personality dimensions validity, solidity and stability according to Sjöbring were measured by means of the MNT scales in representative samples of 70-year-old men and women taking part in a health control survey in Gothenburg, Sweden. Means, standard deviations, stanine scores and a comparison with results from other Swedish studies pertaining to representative samples of middle-aged men and women and 16-year-old boys and girls are presented. Seventy-year-old men scored higher than 70-year-old women on validity, as did men in all other age-groups. The difference could, to some extent, be related to the lower prevalence of neurosis and personality deviations in men. There was no difference between the sexes in the 70-year-olds nor in the middle-aged people with regard to solidity and stability. Sixteen-year-old boys scored higher on stability than 16-year-old girls. Seventy-year-olds of both sexes scored lower on validity than 45-year-olds of both sexes and 70-year-old men scored higher on stability than 45-year-old men. The size of the differences of personality dimensions between the sexes was similar in 70-year-olds and 45-year-olds, and the standard deviations of the personality dimensions were also similar between the 70-year-olds and the 45-year-olds.

Ten-year follow-up of subjects with impaired glucose tolerance: prevention of diabetes by tolbutamide and diet regulation.

In a diabetes detection survey carried out between 1962 and 1965, 2477 (1.1%) of 228,883 subjects had Clinistix-positive glucosuria after a carbohydrate-rich luncheon meal. Of these 2477, 578 displayed impaired tolerance to oral glucose without having manifest diabetes. From this group, 267 men were divided into five groups and subjected to the following treatments and controls: (a) diet regulation and 0.5 g tolbutamide t.i.d. (N = 49), annual oral glucose tolerance test (OGTT); (b) diet regulation and one placebo tablet t.i.d. (N = 48), annual OGTT; (c) diet regulation only (N = 50), annual OGTT; (d) no treatment (N = 61), annual OGTT; and (e) no treatment, OGTT at follow-up (N = 59 at follow-up). In addition, a control group was included comprised of men with normal OGTT (N = 52). At follow-up, 29% of those without diet regulation and medication (group e: N = 59) had developed diabetes. Of those on diet regulation, but without active medication (group b plus group c, N = 98), 13% had diabetes. No individual maintaining tolbutamide and diet regulation (N = 23) had progressed to diabetes. In this group, 80% of those later examined (N = 11) had serum tolbutamide concentrations in the therapeutic range. No individual with initially normal OGTT developed diabetes or impaired OGTT. The findings suggest that normal oral glucose tolerance signifies little risk of progress to impaired glucose tolerance and manifest diabetes, whereas impaired glucose tolerance is associated with a high risk of progression to diabetes. In addition, it seems possible that treatment with diet regulation, in combination with tolbutamide, may prevent or postpone progression from impaired glucose tolerance to manifest diabetes.

Long-term treatment of subjects with borderline glucose tolerance.

Subjects with borderline glucose tolerance are at a higher risk of suffering and dying from cardiovascular disease than subjects with normal glucose tolerance. Our data on the progression to overt diabetes and our data on cardiovascular morbidity and mortality suggests that tolbutamide treatment reduces these risks. In any case, we have no data supporting the theory that tolbutamide promotes cardiovascular damage.

A model for evaluating the analgesic effect of a new fixed ratio combination analgesic in patients undergoing oral surgery.

A special model designed for evaluating the analgesic effect of oral analgesics was based on a short-time registration period of immediate postoperative pain. One-hour intervals in pain registration and a minimum of 2 h between the tablet intake allowed a good estimation of changes in pain levels. The patient material consisted of 112 patients and from each patient a lower impacted wisdom tooth was removed. The test model was used to compare two analgesic drugs with placebo. The two pharmacologically active preparations were Doleron (dextropropoxyphene, acetylsalicylic acid, phenazone, caffeine and Transergan) and Astra 2167 (dextropropoxyphene and acetylsalicylic acid). The trial was double blind and the tablets were administered according to a crossover design. There was no statistically significant difference in analgesic effect between Astra 2167 and Doleron, and both drugs were superior to placebo. Finally, the trial showed that a reduction of the number of components of a compound analgesic to some degree reduced the pain relieving effect on this particular postoperative pain. This observed reduction was however, not statistically significant.

Factors affecting the experience of the initial psychiatric consultation in outpatients with anxiety states.

The experience of the first consultation was measured by a questionnaire in 42 outpatients with neurotic anxiety tension states. The correlation between this experience and the doctor's level of experience, the sex combinations of doctors and patients and the personality of the patient, as measured by an inventory 1-2 years after the end of the drug study (35 patients completed the inventory), was studied. Patients who had had a more favourable experience had more often been interviewed by doctors with longer psychiatric experience. Female patients tended to report a more favourable experience with male doctors than male patients did. Patients with a less favourable experience deviated more from the norms of the personality inventory with regard to, for instance, passive dependency than patients with a more favourable experience did, but the difference between the groups was not significant.

Factors related to drug compliance and attrition in neurotic outpatients treated with anxiolytics.

Forty-six outpatients with anxiety-tension states took part in a study on the effects of anxiolytic drugs. At the first interview ratings were performed and the patients answered a questionnaire as to their experience of the consultation (n = 42). Based on follow-up examinations after 2, 4 and 8 weeks, three groups were formed: completers, who did not experience side-effects to be of such intensity that they changed the recommended dose (n = 15); deviators, who due to side-effects changed the recommended dose temporarily or permanently (n = 20); and attritors (n = 7). One to two years after the end of the drug study a personality inventory was sent out by post. It was returned by 35 patients. 1) Completers, deviators and attritors were similar with regard to age, education and social class. 2) They were also similar with regard to initial distress level. 3) Completers had the highest scores on scales measuring the personality traits defence of status and guilt-feelings and on the factor inex neurotic self-assertiveness, while attritors had the lowest scores. 4) Completers, deviators and attritors were similar with regard to experience of the first consultation. 5) Patients with a more favourable experience of the first consultation reported taking lower doses of the drugs.

Surgical treatment of nasopalatine duct cysts. A follow-up study.

In a clinical-radiological study, 43 operated and histopathologically verified incisive canal cysts were subjected to follow-up investigation. The clinical investigation revealed no subjective symptoms. Abnormalities of the sensibility to the anterior part of the palate were noted in four cases, hyposensibility in three cases and hypersensibility in the fourth. The radiological investigation showed that bone regeneration occurred 1 year postoperatively at the earliest. After 3 years, two-thirds of the patients showed complete bone regeneration. Large cysts (diameter greater than 10 mm) showed the same tendency for bone regeneration as small cysts. The five patients in whom no bone regeneration had occurred were subjected to reoperation. The histologic investigation of these patients revealed scar tissue.

Mandibular reconstruction with bone grafts.

A total of 23 patients with partly resected mandible were repaired with autogenous bone grafts from the iliac crest and ribs. The reasons for reconstruction are presented in Table 1. The grafts healed without complications in 21 cases. With the exception of two of the cases with gunshot wounds, all patients recovered good mobility of the jaw and satisfactory mandibular contours. The patients had been folloed up for 6 months to 11 years. Radiographic examination at the last examination showed that resorption of the graft had been only slight or moderate. Through marrow-spongious bone grafts are regarded as best from an osteogenetic point of view, our cases showed that good results can be achieved also with solid block grafts. The authors discuss the use of plate osteosynthesis without IMF instead of other types of graft fixation and IMF. In six cases where the area of the graft was loaded with a prosthesis, resorption was not more extensive than in the other cases.
