[Dental caries and the morbidity and mortality index in populations with endemic fluorosis].

Clinical and statistical research, conducted in collaboration with other workers in some instances, into the behaviour of dental decay and the morbidity and mortality indices in populations with dental fluorosis due to the ingestion of naturally fluorinated water in Calabria and Sicily showed that the fears expressed with respect to the toxic effect of fluorine on the organism can be considered excessive. It can also be asserted that fluorine is at present the only means available of ensuring a real reduction caries following its ingestion via various routes.

[Effects of the combination of magnesium fluosilicate and manganese on experimental carries in white rats].

The pattern of experimental caries was examined in rats treated with magnesium fluosilicate associated with manganese and a cariogenic diet. A 33.34% fall in the incidence of caries was observed with respect to a control series, whereas Mg fluosilicate and manganese administered alone caused 30% and 20% reductions respectively. It was also noted that a slight toxic action, expressed as a 3 g fall in body weight, was found with the Mg fluosilicate dose employed; this effect was attenuated by simultaneous administration of manganese.

Splenectomy in early chronic myeloid leukemia.

The authors report their experience with 25 patients suffering from C.M.L. who underwent splenectomy in an early phase of the disease. There were no operative deaths; seven patients had postoperative complications, three of which required re-operation for intra-abdominal hemorrhage. The follow-up is still too short to permit definite conclusions, but the results so far obtained suggest that splenectomy is useful in reducing leukemic tissue, improving the quality of life, and potentially efficacious in delaying the onset of blastic transformation.

[Sex determination in hair roots without sheath: proposal of a method for preparation of specimens].

Japanese authors have described a procedure for the sex determination of human hair roots without epithelial sheath, paraffin embedded and fluorescent stained for cortical cell nuclei Y or X sex chromatins. This method however is not sufficiently practical for forensic science purposes: it is difficult and not guaranteed to obtain suitable longitudinal hair sections with possible loss of evidence particularly serious when the questioned hair is only one. The authors, researching Y-bodies with Quinacrine, present an alternative technique of preparation of specimens without embedding which is simple and quick, reproducible and satisfying in results and that avoids loss of material.

The original Bassini technique in the surgical treatment of inguinal hernia.

Bassini's technique represents a fundamental step in the treatment of inguinal hernia. It can be asserted that a new era in its surgical therapy started with Bassini. As many difficulties exist in comprehending the archaic language used a century ago by the Italian surgeon, the principles of his technique are analysed. It is to Bassini's credit that he invented a simple and easily performed technique to be applied in each case, respecting all the anatomic structures without using any foreign material. Accordingly, it is believed that by using this technique correctly, good results can still be obtained today.

[Controlled clinical trial of a new antibiotic "CM 9164" (Midecacin) in dental and stomatological practice].

The action of a new antibiotic identified as CM 9164 ("Midecamycin") and provisionally called "Midecacin", has been experimented on patients admitted for dental conditions. The research was carried out on an open basis and erythromycin was used as a comparison. After a short review of the chemicophysical properties and toxicological and pharmacological features of Midecamycin, the purpose, materials and methods of the research, which was done on two groups of patients, are presented. The results demonstrated the therapeutics usefulness of the new antibiotic as its activity is certainly on a par if not greater than that of erythromycin whether considered "historically" or from the viewpoint of the comparison group. The tolerance of the antibiotic was clinically better than that of the other substances. The doses used were much lower than those required with erythromycin. On the basis of these results, the use of Midecamycin is therefore recommended in the treatment of Gram-positive infections in the dental field.

Surgical treatment of cancer of the colon.

The authors give a brief review of the course of surgical treatment for colon cancer in the last two decades, as reported in the medical literature. Between 1960 and 1980 they examined 619 patients. Of these, 599 underwent surgery: 436 (72.8%) had radical surgery and 163 (27.2%) received palliative operations. The morbidity was 12.1% and the mortality 2.7%. Sixty per cent of the patients in Dukes' class "A" and "B" were alive after five years, against 28% of those in class "C" and non in class "D". The results obtained during the 1970 to 1980 period are not significantly better than those obtained from 1960 to 1969. On the basis of this experience, the authors describe their present approach for the treatment of tumors of the colon and explain in detail the preoperative preparation of the patients, the extension of the excisions and of the lymphectomies, according to the site of the cancer and its local or distant invasion, the use of temporary derivative colostomies and the application of additional medical therapies. Guidelines for reaching an early diagnosis of colon carcinoma are given, in order to improve the postoperative survival rate.

[Value of computerized tomography in the early detection of pelvic recurrences in patients subjected to resection of rectal cancer].

Forty eight patients surgically treated for carcinoma of the rectum were by computed tomography (CT) during routine follow up to test the value if this procedure in the early diagnosis of pelvic recurrences. We used a protocol which schedules controls at different intervals for Dukes A e B and for Dukes C patients; CT showed pelvic recurrences in 13.3% of the Dukes A e B and in 50% of Dukes C cases. CT seems to be the most reliable presently available test in detecting early extraluminal recurrence after radical resection for rectal cancer.
