Benserazide induces migraine attacks. Irrelevance of concomitant hyperprolactinemia.

Migrainous women underwent acute administration of sulpiride (25 mg i.v.) and benserazide (125 mg per os), both drugs increasing prolactin secretion. A similar increase in prolactin levels was found following the administration of the two drugs. A typical migraine attack was observed only in the 4 women given benserazide. These data, though being preliminary, stress the scarce or irrelevant role of prolactin in the pathogenesis of the migraine attack.

[Cryosurgery. Current status. Problems and future prospects].

As matters new stand, cryosurgery has certain definite indications in various fields, either alone or in association with other forms of treatment. It is probably the therapy of choice for 1st-stage haemorrhoids and a series of skin lesions. It is of palliative value in the treatment of tumours and/or an addition to conventional management. The same is true of neoplasm in various sites and stages. As far as the immunological response is concerned, the data available are inconclusive. Further investigation is therefore justified, both experimentally and in man, bearing in mind all the parameters that can be used to assess a possible immune response.

[B-lymphocytes in multiple sclerosis: studies concerning their levels in the blood and cerebrospinal fluid].

In this paper the B lymphocyte population was investigated by fluoresceinated immunoglobulin assay in peripheral blood and cerebrospinal fluid (CSF) in M.S. patients and other neurological diseases taken as control. In peripheral blood there was no significant difference in two groups considered. In CSF there was a higher percentage of B-cells only in M.S. patients. This datum was more evident when the percentage of B cells was simultaneously determined in CSF and blood. It can be hypothesized the existence of subpopulations of B cells synthesizing Ig in the CST.

Ascending aorta-abdominal aorta bypass: indications, technique, and report of 12 patients.

Use of the supraceliac segment of the abdominal aorta for ascending aorta-abdominal aorta bypass (AAAAB) offers a new technique for management of certain difficult surgical problems. Since 1973, we have performed AAAAB in 12 patients: 4 with recurrent coarctation of the thoracic aorta; 4 with coarctation of the thoracic aorta and associated cardiac lesions requiring a concomitant intracardiac procedure; 2 with recurrent aortoiliac occlusive disease (AIOD); 1 with interruption of the aortic arch requiring concomitant pulmonary artery banding; and 1 with coarctation of the abdominal aorta. In 3 of these patients (2 with recurrent AIOD and 1 with coarctation of the abdominal aorta) the distal anastomosis was made to the distal abdominal aorta or femoral arteries. Ten patients (83.3%) experienced satisfactory results; 2 patients (16.6%) died. The technique of AAAAB provides a practical solution to complex situations in which previous procedures preclude a standard operative approach, or when necessary concomitant procedures would otherwise require a two-stage operation.

Hemifacial spasm associated with dolichomegavertebrobasilar anomaly.

3 patients suffering from long-lasting "cryptogenetic" hemifacial spasm in association with dolichomegavertebrobasilar anomaly, are described. The angiographic examinations confirm the anomaly and put in evidence the patholphysiologic value of segment V4 of the vertebral artery at the same side of the spasm. Classical electromyographic examinations, and methods used to study the blink reflex, seem to rule out the hypothesis of a brain stem involvement as the only and sufficient pathologic mechanism. The peripheral pathogenesis is the most likely one.

Comparative Doppler directional sonographic and ophthalmodynamographic study in the occlusion of the internal carotid artery.

In the occlusion of the internal carotid artery, the change occurring in the internal-external carotid pressure gradient may cause flow reversal within the ophthalmic artery and its branches. Ophthalmodynamographic, Doppler directional sonographic and angiographic investigations were performed in 21 cases (11 total occlusions, 6 severe and 4 moderate stenoses of the internal carotid artery). The authors compare the results of these investigations and discuss the practical importance of Doppler directional sonography and the significance of flow reversal in the ophthalmic artery.

[A new surgical technique for treatments of left ventricular outflow tract obstructions. Report of a case and reexamination of our experience (author's transl)].

Aortic valvotomy, aortic valve replacement, resection of the subaortic membrane and aortic patch angioplasty do not always provide satisfactory relief of left ventricular outflow tract obstruction. From August 1975 through January 1977, 15 patients underwent creation of a left ventricular vent by means of a Dacron fabric conduit containing a heterograft valve going from the left ventricle to the supraceliac portion of the abdominal aorta (Cooley's point). All patients had valvular, subvalvular, supravalvular aortic stenosis, or combined defects. One patient had severe hemolytic anemia following aortic valve replacement. Eleven of our 15 patients had undergone one or more previous surgical procedures for relief of aortic stenosis. The operative mortality has been one patient. At postoperative catheterization, there was a normalization of pressure gradients, and the blood flow through the conduit was 40% of the cardiac output. An interesting case of supravalvular aortic stenosis associated with stenosis of the vessels of the arch is presented.

Insulin secretion in migraine: influence on the blood levels of tryptophan.

The blood levels of free tryptophan, glucose and insulin were determined in different groups of patients suffering from headache, initially and after a glucose tolerance and a tolbutamide tolerance test. The control group consisted of subjects without neurological or psychiatric disturbance. A sharp drop of blood tryptophan levels was observed in migraine patients during the tolbutamide tolerance. Migraine attacks may be induced in certain subjects by a disturbance of 5-hydroxytryptamine turnover.

A case of late onset lipid storage myopathy. Electromyographic, histochemical and ultrastructural findings.

A 65-year-old woman showed the clinical picture of a myogenic myopathy, confirmed electromyographically. Detailed histopathological, histochemical and ultrastructural studies were performed on deltoid and quadriceps femoris biopsy specimens. Many muscle fibres were found to contain vacuolar spaces with lipid droplets: perivascular inflammatory changes were also observed. The affected fibres were consistently type I. Under the electron microscope the fibres contained large numbers of lipid droplets, usually next to normal mitochondria. A slight improvement occurred after prednisone therapy.

Cytochemistry of cultured fibroblasts in myotonic muscular dystrophy.

The cytoplasm of cultured skin fibroblasts derived from four unrelated and from three related patients with myotonic muscular dystrophy exhibits high amounts of metachromatic granular material and of alcianophilic material with Alcian blue containing 0.1 M MgCl2. Alcianophilia disappears when the staining solution contains 0.3 M MgCl2, i.e. is probably due only to carboxyl group of glycosaminoglycans.
