[Total proximal reconstruction of chronic aortic dissection in patients with Marfan syndrome].

Authors present two patients suffering from Marfan syndrome. Asymptomatically developed serious cardiovascular complications being characteristic of the syndrome itself were revealed at both cases. Type "A" aortic dissection was found at the first, and type "B" aortic dissection involving even the arch as well revealed at the other patient. They were operated on an elective base. Total proximal aortic reconstructions (aortic root-ascending aorta-arch) were performed under deep hypothermia using total circulatory arrest (35 and 87 min). Both patients are doing well without complaints 3-4 months following the surgical interventions. Control angiographic imagination has found normal anatomic conditions at one of the patients. Anastomosis stenosis at the borderline of the arch and descending aorta was revealed at the other case which should be necessary to operate at an later occasion.

Cytoplasmic Ca2+ signalling and reduction of mitochondrial pyridine nucleotides in adrenal glomerulosa cells in response to K+, angiotensin II and vasopressin.

We have examined the mitochondrial formation of NAD(P)H in rat adrenal glomerulosa cells. A short-term elevation of the K+ concentration from 3.6 to 8.4 mM induced a reversible increase in the formation of reduced pyridine nucleotides. Potassium applied after the addition of rotenone had no further effect, confirming that the redox signal was of mitochondrial origin. Inhibition of aldosterone synthesis by aminoglutethimide in K+-stimulated cells decreased the rate of decay of the NAD(P)H signal upon the termination of stimulation, indicating that the NADPH formed was consumed in aldosterone synthesis. When the NAD(P)H signal was measured simultaneously with the cytoplasmic free Ca2+ concentration ([Ca2+]i), elevation of the K+ concentration to 6.6 or 8.4 mM induced parallel increases in [Ca2+]i and NAD(P)H formation. The rates of increase and decrease of NAD(P)H were lower than for [Ca2+]i, confirming that the redox signal was secondary to the Ca2+ signal. Angiotensin II (100 pM-1 nM) induced an oscillatory NAD(P)H signal which usually returned to a lower baseline concentration, while a sustained signal with superimposed oscillations was observed at higher concentrations. Simultaneous measurements showed that NAD(P)H levels followed the [Ca2+]i pattern evoked by angiotensin II. Vasopressin (100 nM) also induced parallel oscillations of [Ca2+]i and NAD(P)H. A sustained rise in the extramitochondrial Ca2+ concentration to 1 microM induced a sustained elevation of the intramitochondrial Ca2+ concentration in permeabilized cells, as measured with rhod-2. A sustained rise in [Ca2+]i evoked by long-term stimulation with 8.4 mM K+ or 2.5 nM angiotensin II resulted in sustained NAD(P)H production. These Ca2+-dependent changes in the mitochondrial redox state support the biological response, i.e. aldosterone secretion by glomerulosa cells.

New combination for the therapy of canine otitis externa. II. Efficacy in vitro and in vivo.

A new, effective drug combination was developed for the therapy of canine otitis externa (OE) by taking into consideration the microorganisms most frequently isolated from dogs affected with OE, their combination, their drug sensitivity and the type and incidence of ear canal inflammation. The antimycotic active ingredient of the combination is ketoconazole, its antibacterial component is gentamicin sulphate, while its antiphlogistic constituent is mazipredone hydrochloride. Based upon the results of in vitro pharmacodynamic tests, the antifungal activity of the combination is superior to that of ketoconazole used alone at the same concentration. A total of 210 dogs affected with OE were treated with the combination: 94.2 per cent of them became clinically symptomless and microbiologically negative in an average of 8.5 days. No adverse reactions were observed in connection with the use of the drug combination. The therapeutic results can be attributed to the high antifungal efficacy of the combination demonstrated in vitro and to the favourable properties of the solvent mixture.

[Stomatologic implications of Turner syndrome. I. Dental caries and periodontal status].

Turner's syndrome is a chromosomal abnormality with additional deficiency in gonadal and adrenocorticotropic secretions. The most frequent oral findings are: high palatal vault and hypoplastic mandible. The aim of the present study was to investigate the caries prevalence and periodontal condition of patients with Turner's syndrome. The study population consisted of 24 patients with Turner's syndrome and 24 healthy control girls. Mean age were 24.5 years in both groups. Significantly lower DMF-T mean values were found in patients with Turner's syndrome compared with the healthy controls. From among the investigated periodontal parameters probing depth showed no difference between the two groups, but gingival index and plaque index were significantly higher in patients with Turner's syndrome.

[Stomatologic implications of Turner syndrome II. Orthodontic disorders and some characteristics of the temporomandibular joint].

Turner syndrome is a disease belonging to the group of chromosome disorders, which affect sexual chromosomes. The present study aims at analysing and describing orthodontic and TMI joint disorders in patients with Turner's syndrome, and comparing the results with those healthy controls. Data of 24 patients with Turner's syndrome as well as 24 healthy controls were collected and analysed. No significant alteration in TMI was found in any of the two groups. Both groups presented orthodontic anomalies, however, crowding was the most common anomaly in the control group, whereas in the case of patients with Turner's syndrome, other anomalies (protrusion, open bite, cross bite) could also be found more frequently.

[Does tonsillectomy cause any change in long-term course of IgA nephropathy?].

Tonsillectomy seems to be a logical step in IgA nephropathy owing to the frequent tonsillitis before the beginning and the acute exacerbations of the disease. The usefulness of tonsillectomy is questionable because there are only few and short-term follow-up data after it. We followed up 35 kidney biopsy proven IgA nephropathy patients (25 men and 10 women) for an average of 12.2 yrs (range 7-32) after tonsillectomy. The values of endogenous creatinine clearance, serum creatinine, hematuria and proteinuria were compared at regular intervals during a follow-up with those obtained before tonsillectomy. We considered more than 12,000 data using 2 tailed t test. We found that proteinuria started to decrease significantly already 6 months after tonsillectomy (1.40 +/- 0.27 g/day before tonsillectomy vs 0.92 +/- 0.25 g/day after it, p < 0.05) and was significantly lower under follow-up. The tendency in microhematuria was similar (70.5 +/- 35.0 million RBC/12 hours before and 14.0 +/- 6.5 million RBC/12 hours 6 months after tonsillectomy, p < 0.0001). Furthermore, tonsillectomy stopped gross hematuria appearing in the acute exacerbation of the disease in more than two-thirds of patients. Creatinine clearance did not change in the first 2 years after tonsillectomy, however, from 2.5 years after it is significant slow and continual decrease started (117.0 +/- 9.8 ml/min before and 106.2 +/- 10.8 mil/min 2.5 yrs after tonsillectomy, p < 0.05). End-stage renal failure was detected only in 4 patients out of 35 after 10 years after tonsillectomy, in our non-tonsillectomised control group in 8 patients out of 40.

[Disseminated histiocytosis X in an adult patient].

Patient was a 69 year old white female who presented with diffuse brownish-reddish and normal skin colored nodules of a diameter ranging from 0.5-4 cm. Histologic examination showed histiocytosis X. Despite the retinoid-PUVA, and combination of glucocorticoid and chemotherapy the disease progressed and in 8 months after the appearance of the skin lesions the patient expired. The authors briefly summarise the clinical, histological features of histiocytosis X and the therapeutic possibilities.

[Simultaneous heart valve implantation and coronary bypass following kidney transplantation].

The authors report simultaneous aortic valve replacement and coronary artery bypass grafting surgery successfully performed in a patient with functioning transplanted kidney. The patient's cardiac status was classified as NYHA class III. The indication of the cardiac operation was a heavily calcified, stenotic and insufficient aortic valve and severe coronary artery disease. Renal function and blood biochemistry tests did not show significant changes in the postoperative period. The patient was discharged on the 15. postoperative day after an uneventful postoperative period. He remained asymptotic for two and a half years after operation. His cardiac status judged by postoperative treadmill stress was NYHA class I. The outcome of this case in accordance with results found in the literature further confirms that, if the clinical indications are appropriate, a kidney transplant patient can survive and benefit from cardiac surgery. To our knowledge our patient has been the only one who has survived aortic valve replacement and double coronary artery bypass grafting after kidney transplantation, and this is the first case that has ever been reported in Hungary.

[Ultrasonic screening of neonates].

Till june of 1990 all newborns borned in the authors' hospital were examined with ultrasound before discharge. 7582 newborns were screened during 5 years. 120 urinary abnormalities were found (1.58%). Eleven of 26 hydronephrosis were operated and one of the two multicystic kidneys. 6 unilateral agenesis of kidney, 12 unilateral hypoplasies and 30 duplex kidneys were found. 2 of the laters had hydronephrosis, too. The form of position of 37 kidneys were abnormal. 463 newborns had pyelectasy (6.1%). All were recovered spontaneously but one of them had a gross vesico-ureteric reflux. 31 cerebral malformations were found, one of them was a progressive hydrocephalus. Two of the three babies with cerebral arterio-venous malformation died because of circulatory insufficiency. There were 38 abdominal malformations: 3 hepatic and 7 splenic cysts, 3 bile stones and one neuroblastoma were the most important. 86 delivery injuries were found: 60 adrenal apoplexies, 2 hepatic ruptures and 24 intracranial hemorrhages. All healed. The ultrasound screening in the 19-20th weeks of pregnancy showed only 24% of serious renal and cerebral malformations. The authors think important to screen newborns with ultrasound.

Enterotoxigenic Escherichia coli, rotavirus, porcine epidemic diarrhoea virus, adenovirus and calici-like virus in porcine postweaning diarrhoea in Hungary.

In order to obtain data about the significance of enterotoxigenic E. coli (ETEC), and of the different enteric viruses in the aetiology of porcine postweaning diarrhoea, bacteriological, electron microscopic and ELISA studies were made on dead and live pigs. E. coli from the small intestine of diarrhoeal weaned pigs that died were tested for serogroups, pili (fimbriae) and toxin-geno-type. The 108 haemolytic E. coli representing 14 farms and 42 pigs were typed as follows: 0149:K88+ETEC (56.5%), OX:K88+ETEC (21.3%), O141:F18ac+ ETEC (4.6%), O147:F18ac+ETEC (3.7%), OX and O157:F18ac+ETEC (9.3%), verotoxigenic O141:F18ac (2%). In another study, when faecal samples of 92 live diarrhoeal weaned pigs (representing 19 farms) were tested, rotaviruses (18.6%), porcine epidemic diarrhoea virus (PED) (5.5%) and calici-like viruses (5.5%), and adenovirus (two pigs) were detected, besides K88+ETEC (12%) and K99+ or 987P+ETEC (one of each). Combined infections were detected in 9% of the samples. Sequential studies of diarrhoeal and nondiarrhoeal weaned and unweaned pigs indicated that PED virus and group A rotavirus were related to diarrhoea but adeno- or calici-like virus were not. It was concluded that K88+ETEC was the overwhelming aetiologic agent of porcine postweaning diarrhoea in Hungary, but F18ac+ETEC, group A rotavirus and PED virus were also significant.

[Cariologic and periodontal screening of children aged 7, 12 and 14 years in the city of Debrecen. II. Periodontal status and need for treatment].

The aim of present survey was to gain epidemiological data on periodontal condition of 7, 12 and 14 yrs old (40 children in 7, 92 children in 12 and 84 children in 14 yrs old groups) schoolchildren as assessed by the CPITN. The peridontal health of children proved to be very poor. Only 33.3% of 7 yrs old, 20.9% of 12 yrs and 13.1% of 14 yrs old children showed healthy gingiva. Bleeding on probing was observed in 66.7% of 7 yrs old group, in 19.8% of 12 and in 16.7% of 14 yrs old groups. The calculus prevalence was 0.0% of 7 yrs old children, 56.0% in 12 and 44.7% in 14 yrs old groups. Oral hygiene instruction was needed by 66.7% of 7, 79.1% of 12 and 86.9% of 14 yrs old children. Scaling was necessary in 11.1% of 7, 59.3% of 12 and 70.2% of 14 yrs old groups. Complex treatment was needed in none of the groups examined.

[Use of hydroxy-propyl-methyl cellulose (methocel) and carboxy-methyl cellulose containing artificial saliva in the symptomatic treatment of xerostomia].

Salivary gland hypofunction is a common sequela of Sjögren's syndrome and irradiation treatment of tumors in head and neck region. The aim of the present study was to evaluate the therapeutic effect of a recently developed hydroxy-propyl-methyl-cellulose (HPMC) containing artificial saliva and to compare it with that of the carboxy-methyl-cellulose (CMC) based saliva substitute. The therapeutic effects of both preparations were assessed by means of self-administered questionnaires which had to be completed before and after the use of each type of artificial saliva after one week treatment. We found significant improvement in symptoms of nocturnal discomfort and difficulty in speech, and results were better in case of HPMC based artificial saliva. The retention time in oral cavity and the frequency of daily administration also were better in case of HPMC containing preparation. It is concluded that the use of HPMC containing artificial saliva can be recommended in the treatment of salivary hypofunction.

[One year results after extracorporeal shock wave lithotripsy of gallbladder stones].

512 patients with gallbladder stones (393 females, mean age 52.7 years; 119 males, mean age 46.9 years) were treated by extracorporeal shock wave lithotripsy with an electrohydraulic Dornier MPL 9000 lithotripter. The Munich criteria were used for patients selection. Midazolam (15 mg im) and piritramid (mean 7.5 mg iv) were administered as analgetics. Stone fragmentation was achieved after an average of 1.92 treatment sessions. In 12 cases (2.3%) there was no fragmentation. Ursodeoxycholic acid (10 mg/kg/day) was administered as adjuvant litholytic therapy until 3 months after total fragment clearance. During a period of a year the patients returned for follow-up investigations in decreasing number. The total fragment clearance rate was 43.3%, for the I. group (single stone of 5 to 20 mm) was 58%, for the II. group (single stone of 21 to 30 mm) was 28.6%, for the III. group (2 or 3 stones of 30 mm maximum diameter) was 21.4%. In 12 cases (1.2%) vasovagal reactions, in 31 cases (3.1%) atrial and ventricular extrasystoles, in 27 cases (2.7%) transient gross hematuria were observed. During a period of a year 18 cholecystitis (3.5%), 8 pancreatitis (1.56%) and 5 obstructive jaundice (0.97%) developed. 28 cholecystectomies (5.4%), 1 necrectomy because of necrotic pancreatitis (0.19%) and 5 endoscopic sphincterotomies (0.97%) were required.

Analysis of the oral manifestations of systemic sclerosis.

The oral signs and symptoms in 32 patients with systemic sclerosis were evaluated. Oral mucosal telangiectasia was present in 18 cases (56.3%) and was not restricted to the limited form of systemic sclerosis. The interincisal distance was significantly decreased in the patients with systemic sclerosis compared with the 17 controls (p < 0.001). The distance between the vermillion borders was also significantly decreased when the 14 patients with salivary hypofunction were compared with the 18 cases without decreased salivary secretion (p < 0.05). Twenty-two (69%) of the patients exhibited keratoconjunctivitis sicca, salivary hypofunction, or both. Lip biopsy was performed in 16 cases. Two cases with inflammatory signs characteristic of Sjögren's syndrome were found, and six patients showed the histologic signs of labial gland fibrosis. Five of these cases belonged to the group of limited cutaneous systemic sclerosis that indicates the generalized nature of the fibrotic processes even in systemic sclerosis with less extensive skin involvement. Of the 10 cases investigated by electron microscopy, all but one showed a thickening of the capillary basal lamina, lamellar arrangement within the basement membrane, or capillary endothelial vacuolization. Three of these cases belonged to the patients with disease onset within 2 years, showing that capillary vascular lesion is present in the early cases and that vascular injury affects even those tissues that do not seem to be evidently involved by clinical examination.

[Variations of pathological tooth loss with special reference to tooth erosion--apropos of a case].

Importance of tooth surface loss has increased in dental practice. It is difficult to determine aetiological factors and the exact diagnosis, because the symptoms are often combined. The aim of the present study was to summarise the most important factors of tooth surface loss, mainly of erosion which is a frequent form. Reviewing the literature aetiological factors, symptoms, differential diagnostic problems and therapeutic advice are discussed. Prevention, early diagnosis and treatment are very important in these patients.

[Correlation between meteorologic factors and periostitis of dental origin in Debrecen].

Correlation between bioclimatical and meteorological factors is well documented. The aim of this study was to investigate the correlation between dental periostitis and meteorological fronts, cyclon and anticyclon. We found a positive, statistically significant correlation between occurrence of warm meteorological front, cyclon, anticyclon and prevalence of dental periostitis. It could have important implications in organizing the work of dental teams.

[Cariologic and periodontal screening of children aged 7, 12 and 14 years of age in Debrecen. I. Prevalence of caries and need for treatment].

A WHO survey was carried out earlier on caries prevalence in the 12-year olds. No data are available in the 7- and 14 year old children. The aim of this study was to investigate caries prevalence (DMFT, dmft numbers), percentage of caries free children and treatment needs in these three age-groups. The clinical examination was based on WHO's criteria. DMF numbers were 0,12, 2,11 and 2,51 in 7-, 12- and 14-year old, while dmf numbers were 0,79, 0,51 and 0,05 respectively. Percentage of caries-free children was 25% (aged 7), 3,3% (aged 12) and 3,6% (aged 14) in the primary teeth, 87,5% (aged 7), 3,3% (aged 12) and 0 (aged 14) in the permanent teeth. There is no treatment need in 27,5, 22,8 and 17,9% of studied groups. Extraction need was 0, 4,3 and 3,6% in 7-, 12- and 14-year-old. Restorations need was the following: one surface: 45, 65,2 and 71,4%, two surfaces 57,5%, 37, 52,4%, three surfaces: 35, 8,7, 16,7%, more than three surfaces or crown: 2,5, 1,1 and 2,4% at the age of 7, 12, 14 years. The number of DMF and dmf teeth was lower at the 12-year-old children than the previous Hungarian data.

Clinical significance of amniotic fluid bacteriological cultures taken at caesarean section.

To predict postoperative infection after Caesarean section by bacteriological examination of amniotic fluid samples a prospective analysis was performed on amniotic fluid bacteriological results and infectious morbidity in 266 consecutive Caesarean sections. Culture and sensitivity results were analysed in relation to postoperative febrile complications and their antibiotic treatment. One hundred and twelve samples grew bacteria. There was a significantly higher frequency of postoperative pyrexial complications among those patients with a positive amniotic fluid culture (22.3% vs 14.2%). Eighty per cent of amniotic fluid samples with significant bacterial growth provided useful information when antibiotic treatment had been required. Routine amniotic fluid sampling for bacteriology at Caesarean section is of clinical value in the prediction and management of postoperative pyrexial complications.
