[Efficacy of periodic health surveillance of construction workers visited at the CPT of Bergamo].

The aim of this work is to expose the results obtained with the application of a model about efficacy in occupational medicine proposed by ALMLII. In particular we discuss the validity of some indicators of efficacy used in a large group of construction workers during health surveillance, in order to verify effectiveness and appropriateness of an occupational medicine service, as a part of the whole prevention and safety system.

[A particular case of hard metal disease].

BACKGROUND: The production and the use of hard metal tools have becoming increasingly widespread since the second half of the last century also thanks to the great variety of applications that extends from DIY to the aeronautical industry. It has already been known for many years that occupational exposure to hard metals dusts (which occurs especially in people employed in production of the metals or in the sharpening of tools that contain them) can determine the onset of pulmonary fibrosis, bronchial asthma and contact dermatitis. OBJECTIVES: clinical evaluation of a peculiar case of hard metal disease. Descriptions of cases with single pathological pictures due to hard metals, are, in fact, common in the literature, neither are cases with two different clinical pictures (more frequently asthma and pulmonary fibrosis) rare. However, cases in which all the signs and symptoms appeared simultaneously have never been reported. METHODS: a male worker aged 41 years, employed i a hard metal factory for seven years in sintering, and then in grinding. A year later he developed dry cough, wheeze, and eczematous patches. The diagnosis of hard metal disease was based on the work-related symptoms, clinical evaluation, spirometry, chest x-ray, HRCT and patch tests. RESULTS: during the working period, ventilatory function decreased substantially, and then normalized one month after the patient stopped working. Patch tests confirmed sensitivity to cobalt, and skin lesions improved, as did ventilatory function. Chest x-ray and HRCT showed a pulmonary fibrosis that, at the last radiological examination, was still unchanged. CONCLUSIONS: The particular susceptibility to the development of the diseases, could, in our opinion, be in relationship with the race of the subject, many studies (particularly American) have shown that allergic diseases are more frequent, and often more serious, in African subjects, particularly in immigrants from Africa. In the case of occupational diseases attention also needs to be given to the fact that immigrants are often employed in duties that involve a greater exposure to harmful or sensitizing agents. The occurrence of a whole series of occupational allergic diseases among these workers is therefore to be expected.

[Predisposition and evaluation of sanitary procedures in occupational medicine].

"Sanitary procedures" are used to perform preventive and periodic clinical examinations of workers. Their main purpose is to evaluate health status and function of target organs. Periodic medical procedures are used to evaluate the possible onset of precocious and reversible modifications of the health status induced by exposure to specific risk factors during working activities. Moreover, they must be considered an essential step of a correct management of the quality.

Study of lung reactions in six asymptomatic workers occupationally exposed to hard metal dusts.

The description of one case of pulmonary fibrosis in a group of workers occupationally exposed to hard metal dusts with high concentrations of cobalt (50-90%), followed for five years, prompted us to perform a cytological examination on the bronchoalveolar lavage fluid plus determination of the lymphocytic subpopulations in six asymptomatic workers with the longest exposure. Bronchoalveolar lavage performed 16 hours (3 cases) or 48 hours (3 cases) after the end of the workshift showed a lymphocytosis with inversion of the helper/suppressor ratio in 3 cases. This demonstrates an immune response at deep lung level. No difference was observed between a subject with ascertained interstitial fibrosis and the other subjects. Furthermore, bronchoscopy revealed marked inflammatory reaction of the tracheobronchial mucosa.

[Clinico-anamnestic survey in health professionals exposed to volatile anesthetics].

The authors describe the results of a preliminary clinical survey of a group of 220 health workers, physicians and 151 nursing and auxiliary staff members of a large regional hospital in Lombardia, who are exposed to volatile anaesthetics. Contact dermatitis, varices of the legs and arthrosis, particularly that of the cervical and lumbar tract, are the most frequently reported disease in our population. In 75% of cases these disturbances arose after the beginning of the job working in the operating-theatres. When compared with a control group there was a statistically significant difference for contact dermatitis (p less than 0.01) and cervical tract arthrosis (p less than 0.02). The statistical comparison among the subjects exposed to the anaesthetics gases, who were stratified on the basis of the level of exposure and of seniority, confirms a higher prevalence of symptoms due to a reduction of alertness and of psychological and psychomotorial performances in the more exposed personnel. The comparison between female staff exposed and not-exposed to the gases did not show an increase of abortivity, while a statistically significant difference was evident between the female staff (either exposed or not-exposed) and the wives of male workers not exposed to the anaesthetics gases.

[Sanitary surveillance of workers occupationally exposed to volatile anesthetics: proposal for a computerized program for data collection and processing].

The aim of this work is to present a computerized program for an epidemiological evaluation of the data relevant to workers occupationally exposed to anesthetic gases and of the related occupational risks. The great handiness, the speed of execution, the possibility of carrying out researches by mean of multiple variables correlation and the prompt availability of the data, are the distinguishing features of this program.

[Individual dosimetry and biologic monitoring of workers exposed to low concentrations of aniline].

Low aniline vapours concentration exposure has been studied through either active or passive individual dosimeter, in a group of workmen employed in the production of an antioxidant for the rubber industry. We collected 28 samples, confirming the clear correlation existing between the two methods which was already demonstrated in a previous work. We newly suggest the usefulness of the individual passive dosimeter instead of the traditional vacuum pump system. Meanwhile we have determined bound and free aniline and total aromatic amines, with and without an extractive method on column, on urinary samples collected before and after the workshift. The values so obtained have been compared with the results of the active individual dosimetry. We could demonstrated a statistical significative correlation only between urinary aniline determination and the level of exposure, so confirming the value of this parameter as biological indicator of occupational exposure.

Comparative losses of free amino acids and their plasma variations in artificial substitutive programmes for chronic uraemia.

This study provides a comparative evaluation of the losses of free amino acids which take place in the various artificial substitutive therapies for chronic uraemia (conventional haemodialysis, haemofiltration, haemoperfusion, continuous ambulatory peritoneal dialysis, intermittent peritoneal dialysis). The highest loss was found during haemofiltration sessions, the lowest in continuous ambulatory peritoneal dialysis. The differing loss rates do not influence patients' plasma amino acid patterns either in the short or in the long term (up to 12 months). Moreover, no currently available artificial therapy can correct the basic uraemia-related alterations of plasma amino acid patterns.

[Repetitive movement of the upper limbs: results of exposure evaluation and clinical investigation in wood sanding in Bergamo].

The authors report their findings regarding a risk assessment study and clinical tests carried out among a group of 121 workers whose job was to sandpaper and buff timber products. The results of the risk assessment indicate that the job in question is at significant risk for the development of WMSDs in relation to the principal risk factors (repetitiveness, force, posture, short recovery periods). The clinical tests, undertaken by medical staff, complied with the anamnestic models proposed by EPM. The results showed that 21 workers were above the anamnestic threshold, and were thus referred for specialist examinations. As a result of the latter, the relevant diagnoses were made. A positive correlation was reported between the occupational exposure level and the clinical abnormalities detected.
