[Health surveillance of workers].

The main purposes of the health surveillance of workers exposed to anaesthetic gases are: a) the finding of disorders either reliable to the exposure or incompatible with the activity in the operating theatre; b) the epidemiological evaluation of the late effects. Simple and rigorous methods to estimate the previous and the current risk are suggested. The main data to be collected during the medical examination are indicated. The registration of the data of the reproductive function by a protocol of recording is proposed. For the health surveillance of workers in Health Care Institution it's advisable to establish an interdisciplinary group, referring to a specialist on occupational medicine.

[HCV+ patients on the waiting list for kidney transplantation. Retrospective analysis of data on the patient population with hepatitis C on the waiting list for kidney transplantation in the setting of the AIRT].

BACKGROUND: HCV infection in hemodialysis is still a matter of debate from an epidemiological and clinical point of view. Evaluation criteria for HCV-infected patients as transplant candidates are still not adequately standardized. Aims of the present study were to investigate: 1. the percentage of HCV positive patients on the waiting list of three Italian regions belonging to the Associazione InterRegionale Trapianti (AIRT); 2. to analyze the clinical approach in the evaluation of these patients in the attempt to define national guidelines for their pre- and post-transplant management. PATIENTS: We evaluated 2045 uremic patients on the waiting lists of four transplant centers (Bari, Bologna, Modena, Novara) belonging to AIRT at 31/12/2002. RESULTS: The overall prevalence of HCV positive patients was 14.2%, with a peak in the Puglia waiting list. The most common screening tests were AST and ALT serum levels and viral load (HCV RNA). Although there is a clear evidence that histological parameters are the main diagnostic and prognostic markers, a liver biopsy was performed in only 9.5% of patients. An even smaller percentage of HCV-infected patients underwent anti-viral therapy. CONCLUSIONS: Our retrospective analysis evidenced the need to improve common clinical strategies in approaching HCV-infected canditates to renal transplantation in the attempt to improve their post-transplant outcome.

[Kinetics of PCBs in a population of 27 exposed individuals].

Because of the casual assumption of PCBs through contaminated foods produced by the family farm (meat, milk and by-product), 27 subjects (14 female and 13 male), whit a mean age of about 31 years (range 2-77), were submitted to a medical check-up. The results confirm the exposure of all the patients, related to the intake-level of contaminated foods, and the presence of a correlation between the age and plasmatic levels of PCBs, about which the authors discuss the possible reasons.

[Cardiovascular assessment in building activities using an ergospirometer].

We report preliminary results of a current study aimed to evaluated peak oxygen consumption and others cardiopulmonary exercise test parameters, in some common and tiring building activities, with the use of a portable ergospirometer stress test (Metamax 3B, Cortex Biophysik). We tested 3 Building School's Teachers of Seriate (Bergamo): the materials handling and transport of cement bricks and to dig with pick and shovel reaches peak anaerobic threshold into few minutes while the activity of make molter permits an aerobic metabolism in all building workers tested.

[Introductional results of a mortality study in 471 ex-exposed workers to PCBs].

This study concerned a group of ex-workers (372 males and 99 females) from a firm situated in Bergamo (Italy) that ceased its activity at the beginning of the Nineties. From an introductory analysis of the results, we found out that there isn't excess in the death-rate for liver and bile ducts cancers among the males of this group. Among them, for what concerns the other tumoral pathologies, such as leukaemia and stomach tumors, the information allowed to verify an excess of mortality. However, this increase hasn't result statistically significant. On the contrary, it came out that there is an excess of risk for intestine tumors: standardized death-rate of 2.58. In accordance with the information collected, for the future it will be necessary to carefully examine the research in order to verify the existence of a direct connection to the exposure at PCB and study the incidence (not only for mortality) of these neoplasms.

[Variation of the judgment of the fitness to specific work in a cohort study in construction industry].

This work shows the results of a study about variation of the judgement of the fitness to specific work in the construction industry in a year. In the 8.6% of the cases, after a year from the previous examination, it was necessary to express new limitations to the job activity mainly because of the presence of illness of the musculoskeletal or cardiovascular system. Considering only workers aged over 35 years, the percentage of variation after a year grows to 12.3%.

[Training and information for competent physicians].

The rule in force and the Guide-Lines concerning the topic point out specific duties for the Occupational Physician; the most important is certainly health survey, which is organized in several formal medical actions; another Occupational Physician's relevant activity is the "collaboration to the risk assessment". Because of the complexity of the construction building industry and because of the central role of the Occupational Physician in health care, the problems which he has to face are two: firstly he has to improve the knowledge about specific risks of the construction building industry; secondly he must adapt his specialistic knowledge, including the methodology acquired during the school training, to the specific organization conditions and risk of the field.

[Training individuals of the construction sector in the province of Bergamo: a shared path division and an operative proposal].

With this presentation it will be introduced the experience of the work group native of Bergamo who is taken care to plan the formation course for the professional figures of system in building. The group is formed from the greater agencies present on the territory, their work is to ensure the protection of the health and the emergency of the workers in the field of the constructions. The objective is to use the formation course in order to diffuse one culture of the emergency. In the specific one they will come to describe the various planning phases of the course of formation for employers that mean to elect himself Responsible of the Service of Prevention and Protection (RSPP).

[Management of the new first aid service at enterprises and construction sites].

First aid at work organization and management represents a complex and critical aspect of the manifold problems of hygiene and security at work; nevertheless, even in relatively well organized productions, these themes are often neglected, if not completely ignored. In this work the authors analize the laws which regulate the correct first aid at work organization and management and an approach to the problem based on the preliminary risk assessment is suggested.

[First aid training in collaboration with CPT and CPTA of Bergamo].

In this work it's briefly described the experience in planning and realizing of first aid at work courses for workers of the building trade, realized by the UOOML in collaboration with CPT and CPTA of Bergamo. In particular this is a report of the activity of planning and realization of courses in the months from February till September 2004.

[Assessment of the efficiency of the first aid course among construction companies in the Province of Bergamo].

The aim of this project is to value, through a specific questionnaire, the effectiveness and utility of the professional training among the construction workers responsible of the First Aid. 48 construction firms, in Province of Bergamo, were examined and 100 first aid responsible were tested through to the questionnaire. The 21% of the subjects performed a practical first aid intervention. The 84% considers the professional training courses to be globally satisfactory, and one third underlines the necessity of an annual refresher course. This study tends to confirm the validity of this program of professional training, particularly in the construction trade, that is characterized by a high accidents rate.

[Prevalence of risk factors and prevention of coronary disease in a population of construction workers of the Province of Bergamo].

Authors describe the prevalence of cardiovascular risk factors among 1.348 construction workers, enrolled in a five years lasted health surveillance program. Our results confirm both the high prevalence of cardiovascular risk factors and the need of further examinations other the clinical, laboratory, electrocardiogram and chest X, in order to point out a correct diagnosis and evaluate the fitness to job. Furthermore, we underline the usefulness of training in health education for the prevention of cardiovascular disease, considering the low extent to which individuals perceive their own risk.

[Research and formal demonstration of evidence of efficiency in occupational medicine].

There is an increasing pressure on occupational health professionals to ensure that their practice is based on a quality standard and an evidence of appropriateness. The efficacy-effectiveness of their activity strictly depends on their intellectual and professional integrity, their technical and legal competence, but also on the quality of the relations with the company organisation they are able to establish. We asked to some occupational physician when they considered their interventions in the workplace to be efficacious. Then, accordingto our epexrience, we propose some effectiveness indicators, particular concerning health surveillance and health educttaon, that should be evaluated to verify the health professional agccvity agreement to quality standards.

[Efficacy in occupational medicine: proposals of the regional sections of SIMLII of Lombardy and Emilia Romagna].

The efficacy in Occupational Medicine is based on the degree of attainment of workers health and safety care. The Occupational Physician activity, attending on business parts, gives a contribution to the mission of the firm leading to the workers health/safety. The Occupational Physician actions are conditioned by workers features and by the other members of prevention system. It is necessary to verify the efficacy, the efficiency and the adequacy of the Occupational Physician activity finding goals and specific indicators.

[Clinical and risk records assessment].

In the collaboration between the Hospital Occupational Health Department and Public Occupational Service of Bergamo, it has been elaborated a plan finalised to the verification of the quality and of the appropriateness in the compilation of the Clinical and Risk Record in the firm. The study has been lead by evaluation of 409 clinical and risk records (containing the first visit and, in 277 records, a periodic visit) collected from different productive areas (wood, building, health, agriculture). The analysis of the clinical and risk records have shown many critical aspects in their compilation, for this reason it is necessary to improve vigilance and attention from Public Occupational Service, Hospital Occupational Health Department and the Scientific Associations.

[Health surveillance in noise exposure].

The Occupational Health Department of Bergamo Ospedali Riuniti and Occupational Health Public Services set up a project in order to check the health surveillance quality in noise exposed workers. 887 hearing threshold tests, performed between 1998 and 2002, were examined; the audiograms concerned 304 workers exposed to industrial noise higher than 85dB(A), employed in 22 textile, mechanical, wood, stone, building and paper industries. After examining the 887 audiograms we noticed that the health surveillance of noise exposed workers up to now, 14 years after the issuing of the Legislative Decree 277/91, reveals many critical situations, showing an inadequate accuracy and possibility of interpretation of the audiometric tests.

[Study of the health surveillance activities of the C.P.T. of Bergamo].

A "customer satisfaction" research about the approving of the sanitary watching activities provided by the "Comitato paritetico territoriale" of Bergamo and the Department of Occupational Medicine of "Ospedali Riuniti" in Bergamo, comprehensive of the diferent points of view by the different subjects involved in the process, in order to estimate and improve the facilities provided.
